2.01 Approve a budget adjustment in the Public Safety (COPS) Department (Dept. 8509) increasing revenues and expenditures by $11. Subject to routing as to form and content.Fiscal Impact: No impact to the General Fund.

2.02 Approve a budget adjustment in the Tax Revenue Anticipation Note Department (Dept. 9883) increasing Transfers Out by $11,336; and approve a budget adjustment in the Contributions to Others Department (Dept. 1990) increasing Transfers In by $11,336. Subject to routing as to form and content.Fiscal Impact: Increase in General Fund revenue in the amount of $11,336.

Behavioral Health Services

2.03 Approve amendment no. 2 with Department of Alcohol and Drug Program (ADP) and Department of Health Care Services (DHCS) for FY 12-13. Subject to routing as to form and content.Fiscal Impact: Increase in revenue of $393,447 per Fiscal Year for 2012/13 and 2013/14.

Behavorial Health Services

2.04 Approve an agreement with Kings View Corporation to provide computerized Management Information Services (MIS), Electronic Health Records (EHR) information services, and all pay sources billing services. Subject to routing as to form and content.Fiscal Impact: The total cost for services under this contract is $159,484, FY 13-14. Source of funding is

Behavioral Health’s Medi-Cal Realignment and MHSA funds.

2.05 Approve an agreement with Kings View Corporation to provide Telepsychiatry Services to Medi-Cal beneficiaries of Trinity County and provide a Medical Director for TCBHS. Subject to routing as to form and content.Fiscal Impact: The Maximum Cost to County shall not exceed $191,040 and shall be paid from Medi-Cal

and Behavioral Health's allocation of Realignment Funds.

2.08 Find no objection to the issuance of a daily alcoholic beverage license to the Weaverville Lions Club to serve alcohol during the annual Fourth of July activities in Lowden Park, Weavervill, CA from July 3 through July 6, 2013.Fiscal Impact: No fiscal impact.

2.09 Approve a budget adjustment in the Five County COHO Department (Dept. 2710) increasing revenues and expenditures by $186. Subject to routing as to form and content.Fiscal Impact: No impact to the General Fund.

2.10 Approve a budget adjustment in the OPEB Department (Dept. 6300) decreasing Salaries & Benefits and increasing Services & Supplies by $194. Subject to routing as to form and content.Fiscal Impact: No impact to the General Fund.

2.11 Approve amendment no. 3 to the agreement with Nor-Cal EMS extending the term to June 30, 2014 for Local Agency EMS services. Subject to routing as to form and content.Fiscal Impact: $25,000 per fiscal year

2.12 Authorize the recruitment and hiring of a Deputy Director of Clinical Services at range M244 Step A for the Department of Behavioral Health effective June 1, 2013 due to a recent resignation.Fiscal Impact: Approximate cost in salary and benefits per month is $8,692.

2.13 Authorize the recruitment and hiring of two Accounting Technician I/II in the Auditor's Office to fill recent vacancies.Fiscal Impact: Approximate cost in salary and benefits per month for an Accounting Technician I is $4,804

and for an Accounting Technician II is $5,156 per position.

Health & Human Services

2.14 Approve amendment no. 3 to the agreement with Kenneth Cutler, M.D., MPH extending the term to June 30, 2016 to serve as Public Health Officer. Subject to routing as to form and content.Fiscal Impact: No impact to the Genearl Fund, $46,800 to Health and Human Services over the 3 year

extension period.

2.15 Approve amendment no. 4 to the agreement with Sam and Ila Carroll and Rebecca Freeman to extend the term of the lease for 60 days for property in the Trinity Alps Industrial Park. Subject to routing as to form and content.Fiscal Impact: $14,723.12 per month

2.16 Approve establishing a Special Revenue Fund - Fund# 599; approve establishing a budget for FY 2012/13 in the Prison Rape Elimination Department (Dept. 8599) appropriation of $28,203 in revenues, expenditures and Transfers In; and approve a budget adjustment in the Evidence Based Probation Supervision Department (Dept. 2425) increasing Transfers Out by $28,203. Subject to routing as to form and content.Fiscal Impact: No impact to the General Fund.

Current Cash Balance if the Evidence Based Probation Supervision Department is $217,617.

2.17 Approve a budget adjustment in the Emergency Operations Center Department (Dept. 2247) decreasing Fixed Assets and increasing Services & Supplies by $1,271. Subject to routing as to form and content.Fiscal Impact: No impact to the General Fund.

Transportation

2.18 Approve an agreement with Human Response Network to administer the Transportation Assistance Program for persons needing transportation to medical providers and social services in the amount of $40,000; subject to Transportation Commission allocation of sufficient funds to cover cost of services. Subject to routing as to form and content.Fiscal Impact: No impact to the general fund. Funds for this program come from Local Transportation

Funds.

2.19 Approve an agreement with Southern Trinity Health Services (STHS) to provide public transportation for citizens of the Southern Trinity and Hayfork areasfor public transportation, medical, and social services purposes in an amount not to exceed $25,000. Subject to routing as to form and content.Fiscal Impact: There is no impact to the General Fund.

County Administrative Officer

5.01 Conduct a public hearing to consider comments and take action regarding the number of units assessed to a given parcel.

Fiscal Impact: Reduction of $100 per equivalent unit to Solid Waste Enterprise Fund.

County MattersAll contracts submitted to the Board are subject to routing for approval as to form and content. County matters will not necessarily be heard in the order listed below. Any department head or member of the public desiring an item be considered out of order may go to the podium at this time and request the item be given priority.

Auditor/Controller

6.01 Accept Umpqua's offer to extend credit on the terms and conditions stated within the Commitment Letter for the Fiscal Year 2013-14 Short Term Note in the amount of $3,000,000; and authorize the Chairman to sign the Commitment Letter dated May 8, 2013.

Fiscal Impact: Provides operating funds to meet cash flow needs.

Behavorial Health Services

6.02 Approve the Behavioral Health Mental Health Services (MHSA) Plan for Fiscal Year 2013-14.

Fiscal Impact: The Trinity County Behavioral Health Services Agency recommends that these new funds, totaling $1,748,909 be deposited into our County Behavioral Health Services Agency Account for use in Fiscal Year 13/14 for the purpose of operating the specific plans outlined in the MHSA Plan. These plans were developed after Focus Groups were conducted in the community.

6.03 Approve an agreement with Noel J. O'Neill, LMFT to serve as the Director of Behavioral Health Services.
