    GLOSSARY

    Key terms used in this report are defined as follows:

    All Injury Describes the sum of lost times, restricted and medical treatment cases.

    Days Charged or Time Charged This is a fixed number of days allocated to each death, loss of member or

    equivalent loss of function of member. Each fatality is charged sixthousand (6,000) man-days. This is to express the severity of the case.The American Standard Table of Scheduled Charges put out by theAmerican National Standard Institute is used for this purpose.

    Days Lost The number of normal working days the injured employee was unable towork as the result of an injury for which there is no scheduled day charge.The number of days lost is determined by the doctor.

    Lost-Time Injury An injury suffered by a person, which arises out of and during the courseof employment and which results in the absence of the injured from work for at least one (1) day following the day of the injury.

    Exposure Work-time, usually expressed in number of man-hours worked.

    Fatality Any death resulting from lost-time injury regardless of the timeintervening between injury and death.

    Incidence Rate (1) The number of injuries per two hundred thousand (200,000) hours of work or exposure. Incidence case rate and lost work-day incidence rate may bespecified.

    First-Aid Any one-time treatment and subsequent observation of minor scratches, cuts, burns, splinter intrusions, and so forth which do notrequire medical care even though provided by a physician or registered professional personnel.

    Medical Treatment Any treatment (other than first-aid) administered by a physician or by registered professional personnel under the standing orders of a physician.

    Work-related Cases An occupational injury suffered by an employee which resultsfrom a work accident or from exposure involving a single incidentin the work environment, and any illness caused by exposure toenvironmental factors associated with employment.

    OccupationalIllness Any abnormal condition or disorder of an employee, other than

    resulting from an occupational injury, caused by exposure toenvironmental factors associated with employment.

    Recordable Case Any work-related injury case requiring more than first-aid, and alloccupational illnesses. Recordable cases include:

    1) Death regardless of the time between occupational injury or illness and death.

    2) All occupational illnesses.

    3) All occupational injuries resulting in any of the following:

    a) Lost work-day either days away from work or daysrestricted work activity.

    b) Medical treatment other than first-aid.c) Loss of consciousness.d) Restriction of work or motion.e) Temporary or permanent transfer.f) Termination of injured or ill employee.

    Note Any case that involves lost work-days must be recorded since italways involves one or more of the criteria for recordability.

    Occupational Injury Any injury such as cut, fracture, amputation, etc., that results froma work accident or from an exposure involving a single incident inthe work environment.

    Lost Work-day DaysAway From Work Days away from work are those work-days (consecutive or not) on

    which the employee would have worked but could not because of occupational injury or illness. The number of lost work-daysshould not include the day of injury or onset of illness.

    Lost Work-day Days Days of restricted work activity are those workdays (consecutiveRestricted Work or not) on which, because of the occupational injury or illness,Activity the employee was assigned to another job on a temporary basis,

    Lost Work-day Case Any recordable case that results in lost work-days away from work or work-days of restricted activity.

    DATE OFRECORDABILITY : Occupational Injuries:

    The date of recordability for occupational injuries is the date onwhich work accident or single incident exposure occurred.

    Occupational Illness:

    The dates of recordability for occupational illness is the date of initial diagnosis as an occupational illness, or, if the diagnosisfollows a period during which a person has been unable to performfor unknown reasons, the date should be the first day of absenceattributed to the illness which was later diagnosed or recognized.

    Nature of Injury This refers to the type of injury suffered by the person involved.

    Part of Body This refers to that part of the person injured that was damaged or affected due to the accident.

    Location Location refers to the actual area in which the accident occurred.Accident Source The equipment or thing that contributed to the accident is known

    as the accident source.

    Accident Cause This relates to unsafe action or unsafe condition which resulted inthe accident.

    Medical Treatment This means the number of medical treatment cases per twoRates hundred thousand (200,000) man-hours of exposure or work.

    Restricted work This means the number of restricted work cases per twoCase Rate hundred thousand (200,000) man-hours of exposure or work.

    Lost Time This means the number of lost time injuries per two hundredCase Rate thousand (200,000) man-hours of exposure or work.

    Lost Work Day Rate This means the number of lost work days per two hundredthousand (200,000) man-hours of exposure or work.

    Restricted Work DayRate This means the number of restricted work days per two hundred

    thousand (200,000) man-hours of exposure or work.

    "PAR' Shall be half the sum of the National Average Index and theindividual plant's average index for three (3) preceding years.
