In keeping with our mission as “dedicated to working with our partners and providing assistance to the fireservices in the protection of life, property, and the environment through communication, inspection,investigation, certification, and licensing,” the Office of the State Fire Marshal is meeting the challenge.

As you read this report, you will see we have had a very successful year. My dedicated staff works diligently intheir efforts toward fulfilling all of our initiatives; we are meeting our federal and state mandates, and servingthe citizens in the state of Illinois.

ARSON INVESTIGATIONThe mission of the Office of the Illinois StateFire Marshal, Division of Arson Investigation, isto provide fire investigation and law enforce-ment services to the fire service and federal,state, and local law enforcement agenciesthroughout the state of Illinois. This is accom-plished through complete and thorough inves-tigations, evidence collection, and professionalexpert testimony in court proceedings.

The Division is responsible for investigatingsuspicious fires and explosions and providingfire and explosion origin and cause investiga-tions when requested by the fire service andlaw enforcement agencies throughout thestate. Nationally, arson (and suspected arson)is the primary cause of property damage dueto fire in the United States and the secondleading cause of fire deaths. To combat this se-rious problem, certified investigators respondto assist fire departments, law enforcementagencies, communities, and state and federalagencies 24 hours a day, 7 days a week via a24-hour, toll free Arson Hotline number (800)252-2947. When the requesting agency callsthe hotline, they are immediately connectedwith an Arson Duty Investigator. The ArsonDuty Investigator has authority to dispatch anArson Investigator anywhere in the state, any-time of the day or night.

Arson Investigators are sworn peace officersauthorized to conduct fire and/or explosionorigin and cause investigations, interviewwitnesses and/or suspects, conduct follow-upinvestigations, execute search and arrest war-rants, collect evidence, make arrests, and testifyin court. There are six canine teams availablewhich consist of a Certified Arson Investigatorand a certified ignitable liquid detecting canine.Canines can detect minute traces of ignitable

liquids often used in fires and can lead investi-gators to a specific location where physicalsamples can be secured, analyzed, and con-firmed by special laboratories.

Arson Investigators and canine teams investi-gate over 1,000 fires each year. Together withfederal, state, county, and local law enforce-ment agencies, many arsonists are arrested an-nually.

The Division’s Arson Investigators attend manyhours of continuing education classes andseminars each year. This increases their knowl-edge and expertise of fire investigation and lawenforcement training.

2011 Highlightsn The Division of Arson Investigation consists

of 18 personnel; including the DivisionDirector, two Administrative Assistants,and front line field staff consisting of 15Certified Arson Investigators.

n There were 1,011 incident investigations (2explosions and 1,009 fires) conducted byDivision Investigators during 2011, a de-crease of 20 percent from 2010. There were895 structure fires, 97 vehicles, and 18 otherincidents investigated.

n Investigations conducted in 2011 resulted inthe total of 41 cases being closed with arrest.

n Inquiries regarding juvenile fire setters are re-ferred to the Juvenile Fire Setter InterventionHotline number (847) 400-4864. The JuvenileFire Setter Intervention Program is adminis-tered by the Illinois Fire Safety Alliance.

n Arson Investigators continue to receive pro-fessional development training for enhanc-ing their knowledge and skills in new trends,technology, and investigative techniques.

Advanced investigation classes were hostedby OSFM and attended by Division Investi-gators and other investigators from Illinoisand throughout the nation.

n The Division’s web site continues to ex-pand to provide valuable information toother agencies and to the general public.

n The Certified Fire/Arson Investigator Recer-tification criteria was adopted and imple-mented in July 2007, and the Divisioncontinues to assist with inquiries and assis-tance relating to the recertification criteria.

n The Division continues to work with in-terns and Ride-Along participants to pro-vide a hands-on, working knowledge of theDivision and its function and role in the in-vestigation of arsons throughout Illinois.

n In January of 2011, the Division of ArsonInvestigation began using the Bomb ArsonTracking System (BATS). BATS is a secureweb based, state-of-the-art case manage-ment and investigation tool which linksagencies to the United States Bomb DataCenter (USBDC). The USDDC is a nation-wide repository of arson and explosivesincidents.

2011 CaninesIn 2011, there were 227 canine assisted cases.These canines are specifically trained to detectminute traces of accelerants. Accelerants maybe used in arson fires. Currently, there are sixOSFM certified accelerant detecting canineteams located in Illinois.

2011 IncidentsDuring 2011, the Division investigated 1,011incidents. Of those cases, 354 cases weredetermined incendiary, 179 were determinedaccidental, and 476 cases were listed as un-determined or still under investigation. Two

explosions were also investigated. Investiga-tions conducted in 2011 resulted in 41 casesbeing closed with arrest.

The Agency works closely with local, state,federal fire and law enforcement agencies, andthe courts in dealing with the arson problem.

Fire departments in Illinois responded to49,401 reported fire incidents in 2010. (IllinoisNFIRS Data, 1/11)

There was an estimated $397 million in propertydamage in Illinois in 2010. (Illinois NFIRS Data,1/11) This data represents incidents reported byIllinois Fire Departments to the National FireIncident Reporting System. The Illinois Office ofthe State Fire Marshal cannot guarantee thecompleteness or accuracy of this data.

Arson Facts(Source: Fire Loss in the United States during2010 NFPA, 8/11)

n U.S. fire departments responded to anestimated 1,331,500 fires in 2010, a slightdecrease of 1.3% from the year before.

n 482,000 fires occurred in structures, adecrease of 0.3%.

n 384,000 fires or 80% of all structure firesoccurred in residential properties.

n 215,000 fires occurred in vehicles, a decreaseof 1.6% from the year before.

n 634,000 fires occurred in outside properties,a decrease of 2.3%.

n What do these fire frequencies abovemean? Every 24 seconds, a fire departmentresponds to a fire somewhere in the nation.A fire occurs in a structure at the rate of 1every 65 seconds, and in particular, a resi-dential fire occurs every 82 seconds. Firesoccur in vehicles at the rate of 1 every 146

seconds, and there’s a fire in an outsideproperty every 50 seconds.

n 3,120 civilian fire deaths occurred in 2010,an increase of 3.7%.

n About 85% of all fire deaths occurred in thehome.

n Nationwide, there was a civilian fire deathevery 169 minutes and a civilian fire injuryevery 30 minutes in 2010.

n An estimated 27,500 intentionally setstructure fires occurred in 2010, an increaseof 3.8%.

n These intentionally set structure fires re-sulted in an estimated 200 civilian deaths,an increase of 17.7%.

n Intentionally set structure fires also resultedin $585 million in property loss, a decreaseof 14.5%.

n Also in 2010, there were an estimated14,000 intentionally set vehicle fires, a de-crease of 6.7% from a year ago, whichcaused $89 million in property damage, adecrease of 17.6% from a year ago.

BOILER ANDPRESSURE VESSEL SAFETY

2011 Highlightsn Performed 45,622 boiler and pressure vessel

inspections.

n Wrote 1,665 violations for corrective action.

n Conducted 32 audits and joint reviews forOwner-User organizations, Illinois Author-izations, and National Board “R” Stampholders.

n Issued 40 new Certificates of Competencyto inspectors and renewed 275 to previ-ously certified inspectors.

n Issued 202 permits for low pressure boilerrepairs to authorized repair organizations.

n Collected $4.1 million in boiler and pres-sure vessel fees.

n Issued 13 new and renewal licenses to au-thorized repair firms.

Boilers are used to heat schools, hospitals, nurs-ing homes, and other public and private build-ings. All are inspected to prevent accidentscaused by explosions. Pressure vessels are rou-tinely inspected in manufacturing facilities,chemical plants, and at other vital locationswhich fuel the Illinois economy. Inspectionsidentify hazards and provide an opportunityfor repair and measures to ensure public safety.

The Division of Boiler and Pressure VesselSafety regulates the construction, installation,repair, use, and operation of boilers and pres-sure vessels as mandated by the Illinois BoilerSafety Act. The Division’s main responsibilityis conducting in-service inspections of pres-sure equipment. Currently, there are 97,690 ac-tive boilers and pressure vessels in thisprogram which require inspections at differentintervals. Professional safety inspections areperformed on boilers and pressure vessels inschools, churches, hospitals, nursing homes,office buildings, places of public assembly, re-tail, apartment buildings, manufacturing facil-ities, utility plants, refineries, chemical plants,and any other type of location in Illinois wherepressure equipment is utilized.

Boiler and Pressure VesselRepairer ActAnother area of responsibility is the administra-tion of the Illinois Boiler and Pressure VesselRepairer Act. There are a total of 172 companiesauthorized under this Act to perform repairs

and alterations to pressure equipment. Thesecompanies are required to be audited everythree years under this program. The Divisionconducted 21 audits during 2011 to ensurelicensed repair companies were complyingwith rules and regulations as prescribed by theRepairer Act.

Boiler FeesDuring 2011, the Division had receipts of$4.1 million for inspections, certificates, repairlicenses, commissions, and auditing fees.

2011 InspectionsThis joint government/industry inspectionprogram is an effective partnership. During2011, a total of 45,622 inspections were con-ducted. OSFM Boiler Safety Specialists per-formed 24,200 inspections, while insurancecompany inspectors performed 21,370 in-spections, and 52 inspections were performedby owner-user inspectors on their own boil-ers. Inspectors reported 1,665 violations,which required corrective action.

ELEVATOR SAFETY

2011 HighlightsPA 92-0873, cited as the Elevator Safety and Reg-ulation Act was originally created and approvedby the Illinois General Assembly on January 3,2003, in order to provide for public safety andawareness in relation to conveyances locatedwithin the state of Illinois. The Act requires thedesign, construction operation, inspection, test-ing, maintenance, alteration, and repair of eleva-tors, dumbwaiters, escalators, moving sidewalks,platform lifts, stairway chairlifts, and automatedpeople conveyances are in accordance with allapplicable statutes and rules. The Office of theIllinois State Fire Marshal (OSFM) is named asthe administrator of the program and is respon-sible for ensuring enforcement.

The Elevator Safety Review Board was also cre-ated by Public Act 92-0873 to adopt rules con-sistent with the provisions of the Act and forthe administration and enforcement of theAct. The Board has the authority to hear ap-peals on exceptions and variances and de-pending on the outcome, grant or deny anexception or variance; hold hearings; establishfee schedules; approve continuing educationprograms.

Since the inception of PA 92-0873, the ElevatorSafety and Regulation Act has been amendedon four occasions. PA 95-0573 refined the Act,and PA 95-0767 moved mandated conveyanceupgrade compliance to 2013 from 2009 and2011. These were signed on July 29, 2008. PA 96-0054 updated the Act and moved compliancedates to 2015. This was signed on July 23, 2009.PA 96-0342 removed special purpose personnelelevators within the scope of ASME A17.1, andused only by authorized personnel from the Actwas signed on August 11, 2009.

Public Act 097-0310 was enacted on August11, 2011. This gives locations for religious wor-ship that have only one conveyance and travelone floor the ability to renew their certificateson a triennial basis rather than on an annualbasis. If a place of worship has more than oneconveyance, they must continue to renewtheir certificate on an annual basis.

Activity within the Division for 2011Registrations, Permits, Certificates ofOperation, Municipalities, and Licensing forCalendar Year 2011:

n The Division now has 32,834 registeredconveyances. Of this total, 13,384 are under

state jurisdiction, and 19,450 are underthe jurisdiction of municipalities with anagreement with the OSFM.

n 442 permits issued.

n 9,149 Certificates of Operation processed.

n 1,185 conveyances registered.

n 1,477 licenses processed, and the followingare active licenses:

98 licensed contractors.

6 limited contractor licenses.

191 licensed inspectors.

1,474 licensed mechanics.

17 licensed limited mechanics.

695 registered apprentices/helpers.

47 licensed inspection companies.

n 181 municipalities have signed an agree-ment with the OSFM indicating they willcontinue to run their own elevator programand adopt the codes and rules used by theElevator Safety Division of the OSFM.

Division Complementn The processing work for the Division (the re-

view and issuance of Certificates of Opera-tion, licensing, and permits) continues tobe handled in the Chicago Office. In 2011,registrations were transferred to the ChicagoOffice.

n The Springfield Office continues toprocess Elevator Safety files to DocumentManagement.

n One clerical staff member was added to theDivision complement in 2009 to process

Certificates of Operation. This remains afull-time position and will continue to beas 13,384 conveyances need to be inspectedand issued certificates on an annual basis.

n We are currently running the Division withfour field inspectors. One inspector spendsone to two days in the office processingand reviewing permit applications.

Work Performedn Inspectors are in the field making random

checks for registration tags, checking thework of licensed inspectors, and checkingmechanic licenses. Permits are generatedthrough the Chicago Office, and these arereviewed and issued by one field inspector.All incidents and reported accidents aregiven the highest priority, and reports aregenerated as a result of investigations andkept on file. Any complaint received is alsogiven priority attention and, if necessary, sitevisits are conducted, and additional reportsare generated and kept on file. We con-stantly inform the public of the ElevatorSafety Act, our current rules and regulations,and information on conveyances.

n The Division Director frequently appearsbefore various groups informing them ofthe current Elevator Safety rules and regu-lations, changes to the Act, and updatedcode information.

n Various meetings with legislators were at-tended once again in 2011, and these meet-ings resulted in new Elevator Safety rules.

n Meetings of the Elevator Safety ReviewBoard were scheduled every other month in2011, with one emergency meeting held inJune 2011. They were attended by staff withthe Division Director holding the positionof Administrator to the Board.

n We continue to refine and upgrade our dataentry program, Jurisdiction Online. Up-grades and additional reports are neededand have been developed as we redefineour processes to simplify data entry and toaccommodate the additional work beingperformed by the Division.

n Email addresses continue to be collectedfor all new licensed contractors, inspectioncompanies, and inspectors in order to keepall parties informed about changes to thecode, OSFM requirements, Board news up-dates, and any information pertinent to thesmooth operation of the Division. Emailaddresses have also been obtained for allmunicipalities that have an agreement withthe OSFM. This is a great cost-saving effortsince monies are not expended on massmailings to contractors, inspectors, andinspection companies.

n Our website is continually updated withnews and information from the ElevatorSafety Review Board and new informationfrom the Division.

n All licenses are renewed every two years.September 2012 will begin the new re-newal period for the majority of licenseholders. The Division will process 13,384certificates of operation in 2012. The fieldinspectors will have to monitor these con-veyances to ensure the Act and the Rulesare followed. They will conduct random in-spections and visit municipalities underagreement to make certain they are enforc-ing the Act and Rules. The Division alsoconducts investigations and complaintsfrom the public.

n The Director was appointed to the ASMERegulatory Committee in the summerof 2011.

FIRE PREVENTION

2011 Highlightsn Continued statewide enforcement of Illi-

nois’ administrative rules pertaining to firesafety. The Division continued enforcementof the 2000 edition of the National Fire Pro-tection Association’s Life Safety Code® as thestatewide rules for fire prevention.

n Conducted 12,535 on-site inspections atfacilities including public and privateschools, day care centers, day care homes,residential board and care homes, com-munity integrated living arrangements,hotel/motels, places of public assembly,aboveground storage tank installationsites, and state-owned/operated buildingsincluding correctional centers.

n Continued the practice of conducting fireprevention inspections at the Illinois andDuQuoin State Fairs to ensure fire safetycompliance at exhibits, concession stands,tents, and special events during the fairs.

n Conducted a survey of Illinois’ hospitals todetermine the number and nature of in-juries sustained in fireworks accidents dur-ing a four-week period that included theFourth of July holiday.

n Continued the regulation of Pyrotechnic Dis-tributors and Operators as required by theamended Fireworks Use Act (425 ILCS 35)and the Pyrotechnic Distributor and Opera-tor Licensing Act (225 ILCS 227). At the endof 2011, the Agency provided licenses for39 companies (Distributor licenses) and593 individual Operator licenses.

n Continued the regulation of the SprinklerContractor Licensing program which re-quires all Sprinkler contractors to be licensedby the Agency in order to do business in thestate of Illinois. At the end of 2011, therewere 231 licensed contractors.

n Continued the regulation of the state’s fireequipment contractors and their employeesin accordance with the Fire EquipmentDistributor and Employee Regulation Act.At the close of 2011, the Agency licensed177 companies and 563 employees forpurposes of performing fire equipmentinstallation and service in Illinois.

n Provided approximately 306 hours of train-ing for Division of Fire Prevention personnelrelative to the application of the NationalFire Protection Association’s Life SafetyCode® as well as other laws and rules adoptedand enforced by the Division includingcomputer skills and usage.

The Division of Fire Prevention, by legislativemandate, is charged with ensuring no buildingendangers persons or property by reason offaulty construction, age, lack of repair, or anyother cause that would make it especially liableto fire. Under authority granted in 425 ILCS 25,the Division conducts fire safety inspections inaccordance with the applicable laws and ad-ministrative rules adopted to ensure such firesafety. These inspections primarily concentrateon state-owned or regulated facilities includingday care centers and group day care homes asrequired by the Department of Children andFamily Services, residential facility programs atthe request of the Department of Human Serv-ices, correctional centers in accordance with aninteragency agreement with the Department ofCorrections, public schools in cooperationwith the Illinois State Board of Education, racetracks as required by the Illinois Racing Board,permanently moored gaming facilities regu-lated by the Illinois Gaming Board, and telecom-munications switching offices as prescribedby administrative rule. Enforcement also ensurescompliance with regulations pertaining toLP-gas tanks larger than 2,000 gallons in capacity

or at any installation intended for purposes ofdispensing LP-gas, and inspection of above-ground bulk and dispensing tank installations.

The Division also concentrates its efforts oncertain “target” hazards including the inspec-tion of state and county fairgrounds; hotel/motel occupancies; and any occupancy wherea fire hazard has been reported by the localfire authority or the general public. The LifeSafety Code®, published by the National FireProtection Association, has been adopted asIllinois’ rules for fire prevention and safety andserves as the primary enforcement tool of theDivision of Fire Prevention when conductingbuilding inspections. Separate administrativecode sections are applicable to volatile liquidstorage, handling and storage of liquefied pe-troleum gases, furniture fire safety regulation,and fireworks storage.

Fire Equipment Distributor andEmployee RegulationThe Division has responsibility for the enforce-ment of the Fire Equipment Distributor andEmployee Regulation Act. This Act, and theresulting administrative rules applied by theDivision of Fire Prevention, requires the licens-ing of all companies and their employees pro-viding fire equipment installation or servicework in Illinois. While not applicable to firesprinkler systems, the rules and subsequentenforcement do concentrate on the workdone recharging and inspecting portable fireextinguishers as well as the installation andservicing of fixed chemical fire suppressionsystems including those protecting cookingsurfaces in commercial establishments.

Currently, the Agency licenses 177 companiesand 563 employees for purposes of perform-ing fire equipment installation and service in

Illinois in accordance with the Act and subse-quently developed administrative rules.

Sprinkler ContractorLicensing RegulationThe Division of Fire Prevention issues licensesfor fire sprinkler contractors performing workin Illinois. Illinois requires any person, partner-ship, corporation, or other business entity thatinstalls or repairs any fire sprinkler system in Illi-nois to be licensed by the Office of the StateFire Marshal (OSFM). The Fire Sprinkler Con-tractor Licensing Act [225 ILCS 317/1 et seq.]establishes the minimum standards for licen-sure of fire sprinkler installation contractors forthe purpose of protecting the public health andgeneral welfare of the citizens of Illinois. At theend of 2011, the Agency issued 231 licenses.

Pyrotechnic Distributor & OperatorLicensing RegulationThe Division processes applications for Py-rotechnic Distributors and Operators as re-quired by the amended Fireworks Use Act (425ILCS 35) and the Pyrotechnic Distributor andOperator Licensing Act (225 ILCS 227), effectiveJanuary 1, 2006.

Pursuant to the Fireworks Use Act, no personmay act as a consumer, distributor, retailer, ad-vertise, or use any title implying the person is aconsumer, distributor, or retailer unless registeredwith the OSFM. No consumer fireworks may bedistributed, sold, transferred, or provided free ofcharge to an individual who has not been issueda permit in accordance with the Act or has notregistered with the OSFM.

At the end of 2011, the Agency has processedapplications and provided licenses for 39 com-panies (Distributor licenses) and 593 Operatorlicenses.

Fireworks SafetyIn June 2011, the Division of Fire Preventionrequested data from 317 hospitals within theState to determine various facts about injuriesassociated with fireworks occurring during afour-week time period, which included theJuly 4th holiday. Some of the information onour form was not filled out, so we cannot give100% accurate information.

The number of responding hospitals is upfrom last year’s 22% to 78 (25%) respon-dents. Of the 78 hospitals responding to oursurvey, 10 (3%) had no fireworks related in-juries during the time period of June 23through July 20, 2011.

There were 155 persons injured by fireworksthis year. Injuries to males were 104 (67%) andto females 49 (32%). Bottle Rockets (which arebanned fireworks) caused a high number of in-juries at 19%. Injuries affecting the eyes werethe highest percent this year, with a total of20%. The second highest body parts affectedby fireworks were hand injuries at 15% this year.Burns were the leading type of injury at 54%.

Furniture Fire Safety ActResponsibility for enforcement of the FurnitureFire Safety Act is also within the purview of theDivision of Fire Prevention. The Act, and subse-quently promulgated administrative rules en-forced by the Division regulate the manufacture,sale, and use of upholstered seating furniture inspecific occupancies including day care centers,prisons, hotels/motels, health care facilities, andpublic assembly occupancies. The combinationof upholstered furniture and the careless use ofsmoking materials have been identified as thecause of several past fatal fires.

InspectionsFire Prevention Inspectors conducted a total of12,535 inspections in 2011. These inspectionsprotect the public by uncovering building andfire safety hazards before tragedy strikes.

State & County FairsThe Division continued its practice of conduct-ing inspections at the Illinois State Fair and theDuQuoin State Fair. During the Illinois State Fair,Division of Fire Prevention Inspectors conductedfire inspections at buildings, exhibits, concessionareas, and tents prior to and during the fair.These inspections focused on public assemblyareas, beer tents, livestock barns, and thegrandstand coliseum. Inspectors also checkedthe fireworks display area and grandstandstage area before and during each show. Siteswere inspected and re-inspected before andduring the fair. The Springfield State Fair wasagain incident-free from a fire safety standpoint.

Inspectors also conducted similar work at theDuQuoin State Fair. There, the Division staffconducted inspections and carried out numer-ous other special assignments. As in Springfield,the DuQuoin State Fair was incident-free froma fire safety standpoint.

HOMELAND SECURITY

2011 HighlightsThe Office of the State Fire Marshal HomelandSecurity Division continues to ensure theAgency enacts emergency management prin-ciples and has trained staff available so effi-cient operations and services are available tolocal government.

OSFM Emergency OperationsOSFM has staff dedicated and trained to re-spond to homeland security and disaster related

emergencies in the State. In addition tostaff, the State Fire Marshal is an active keyplayer who ensures we have the ability torespond to the public and the fire service.This is accomplished through:

n Homeland Security Briefings – staff con-tinues to participate in the monthly home-land security briefings held in the StateEmergency Operations Center (SEOC).

n OSFM and State Emergency OperationsPlan – the Agency works with IllinoisEmergency Management Agency (IEMA)to ensure all agency resources and experi-ence is available.

n Homeland Security Notifications– staff arein constant communication with intelligenceorganizations such as the State TerrorismIntelligence Center (STIC), the FBI, the USAttorney’s Office, Department of HomelandSecurity, and the Illinois Terrorism Task Force(ITTF). OSFM will notify the fire service ofhomeland security information from thoseagencies as appropriate.

EMnet – the system will permit OSFM,IEMA, and the Mutual Aid Box AlarmSystem (MABAS) to communicate tothe fire service on matters related tolarge scale deployments and intelligencepertinent to the fire service.

n Agency Training – all field staff and iden-tified central office staff are trained in theNational Incident Management System(NIMS) as well as other programs to ensureour capabilities are ready to be utilized.

n Emergency Response

Trained Emergency Responders – OSFMstaff has performed and are available torespond as liaisons to local fire depart-ments/districts as well as have served as

Incident Commander, Unified Area Com-mand IC, and Deputy ICs. NumerousOSFM staff serves as members of theState Incident Management Team (IL-IMT) and have many Position TaskBooks completed.

State Emergency Operations Center(SEOC) – OSFM has staff dedicated towork at a moment’s notice in the StateEmergency Operations Center (SEOC).

The OSFM staffed the State IncidentResponse Center (SIRC) in relationto the southern Illinois floods fromApril 26 – May 10, 2011.

The OSFM staffed the SIRC duringthe State Level Exercise (SLE) held onNovember 15 – 17. SLE simulated anearthquake in southern Illinois.

OSFM SIRC liaisons participated inthe Illinois Plan for Radiological Emer-gencies exercises during the year.

The OSFM was represented at everymonthly briefing at the SEOC forSIRC agency liaisons.

The OSFM continues to maintain pro-fessional staff (8 liaisons) to be availablefor service at a moment’s notice.

State Terrorism Intelligence Center(STIC) – OSFM supported the Fire Intel-ligence Officer position at the STIC andhas worked closely with them to enactfire service based intelligence which sup-ports the fusion center’s goals.

Mutual Aid Box Alarm System (MABAS)– OSFM supports the MABAS conceptand staff works closely with our MABAScounterparts in the SEOC.

Radio Systems – OSFM utilizes Star-com21, VHF, and UHF systems in orderto communicate with other emergencyresponders. OSFM works with otherstate partners in ensuring the State Tac-tical Interoperability CommunicationsPlan (TICP) remains current.

Statewide Committee Participation The Division continues to represent the FireMarshal on the following task forces andcommittees:

n Statewide Interoperability ExecutiveCommittee (SIEC) – the purpose of thisstatewide committee is based on ILCS2615/10 (public safety radio interoperabil-ity) to improve public safety radio interop-erability in the state of Illinois by managingall public safety radio spectrum that hasbeen allocated to public safety agencieswithin the State.

n Mutual Aid Box Alarm System (MABAS)– MABAS is prepared to respond upon theauthorized direction of the state. OSFMcontinues to work with MABAS as a part-ner in the SEOC. Staff continues to attendMABAS Executive Board meetings, confer-ences, and appreciates the opportunity toserve on numerous MABAS committees.

n Illinois Terrorism Task Force (ITTF) – staffparticipates on numerous committees andprojects that help support the mutualgoals of the fire service and the ITTF. Thefollowing summarizes those committeeswe participate in:

The Ready Illinois website (www.ready.illinois.gov) continues to be relied on as a“one-stop” resource for emergency pre-paredness and news in Illinois. The siteoffers comprehensive information onsteps people can take before emergencieshappen, what to do once a disaster hasoccurred, and tips for recovery after theevent. In order to keep our citizens up-to-date on current events and situations,the main page of Ready Illinois is usedto announce new preparedness initia-tives and campaigns. In addition, duringa large-scale disaster, up-to-the-minuteinformation and details about such topicsas shelter locations and road closures canbe posted on the site.

2011 Preparedness Campaign – theIllinois Emergency Management Agencycontinued to utilize the web for effec-tive preparedness messaging throughthe Ready Illinois Website, Facebook,and Twitter accounts. IEMA focusedon a different preparedness topic each

Cyber Security – The Cyber section ofthe Ready Illinois Website went througha major update.

American Sign Language (ASL) – Thecommittee recognized the critical im-portance of being able to communi-cate to all persons in Illinois regardlessof their functional abilities. SinceReady Illinois provides a focused loca-tion for persons to receive prepared-ness information, it was natural for thecommittee to focus on making thewebsite able to communicate wellwith the Functional Needs Commu-nity. With this in mind, the Public In-formation Committee Co-Chairs metwith the Illinois Deaf and Hard ofHearing Commission (IDHHC) withinterest to have the main messagepages of Ready Illinois translated intoAmerican Sign Language (ASL). TheCommission embraced the project en-thusiastically working closely with thecommittee to finalize a product. Releaseof this product is planned for the 1stquarter of 2012.

The School and Campus SecurityTraining Program (SCSTP) is a programdesigned to increase the capacity ofschools, higher education institutions,and first responder agencies to miti-gate, prevent, plan, respond, and recoverfrom disasters and critical incidents.Twelve awareness level and prepared-ness courses are offered to enhance theknowledge, skills, and ability for theplanning, developing, exercising, and

assessing school and higher educationinstitution all-hazards emergency op-erations plans.

Program activities reached an all-timehigh with the delivery of 64 courses to1,834 participants this year. Interest anddemand for continued delivery of plan-ning, preparedness, and technical assis-tance to schools and higher educationinstitutions remains high with manycourses scheduled through 2012.

Publications – The ITTF Public Infor-mation Committee continues to offernumerous documents, making themavailable to the public and first respon-ders. All documents are available fromthe Ready Illinois Website with manyavailable in Spanish.

INFORMATION SERVICES

2011 HighlightsThe Information Services Division provides op-erational and technical support for all Agencycomputer systems. This section also performsall network management for the Agency com-puter network and supports all the data basemanagement of the Agency.

Illinois state law (425 ILCS 25/1 et seq.) requiresall Illinois fire chiefs to investigate and reportall fires to the Office of the Illinois State FireMarshal (OSFM). The rules section of this

statute requires this reporting be in compliancewith the National Fire Incident ReportingSystem (NFIRS) standards. All fire departmentsare required to be in compliance with theirNFIRS reporting to be eligible for any state orfederal grants.

The OSFM exclusively utilizes the UnitedStates Fire Administration NFIRS reportingsoftware and data base. Illinois fire depart-ments may data enter data directly into thefederal system using the new on-line reportingtool (DEBI) or using the Data Entry Tool. Firedepartments may also use third party softwareapplications to compile and export their data.The OSFM maintains a copy of the federaldata base on a state server. This data ware-house is used for program management, dataquality checks, and statistical reporting. Thedata warehouse is up-dated on the 5th and20th of each month. If those days occur on aweekend or holiday, the update is done thenext business day. A bi-monthly statistical re-port is generated for all fires in Illinois for thelast three years. Fire departments can checktheir reporting status by referring to the fireincident report generated bi-monthly. OSFMsends fire departments prior notification ofcompliance before any grant applications aremailed to the fire chiefs. Other electronicmeans are being used to notify departmentsof their NFIRS compliance.

PERSONNEL STANDARDSAND EDUCATIONThe Division of Personnel Standards and Educa-tion (PS&E) is responsible for the standardiza-tion and enhancement in the levels of educationand training for firefighters and professionals inIllinois with the highest priorities placed onsafety and career advancement. PS&E operatesunder the mandate of the Illinois Fire ProtectionTraining Act [50ILCS 740/1 et. Seq.], which pro-vides for the promotion of standards to protectthe health, safety, and welfare of the public.

The Division continues to update certificationprograms to comply with current National FireProtection Association Standards and applicableneeds of the fire service.

During 2011, PS&E dispersed $950,000 for localfire departments and fire protection districtstraining reimbursement claims. The Divisionprocessed 2,000 claims for a total of 116,170hours of training to 76 departments.

The Division also processed 11,901 certifica-tions, 13,718 requests for examinations, andadministered approximately 11,823 written ex-aminations to Illinois firefighters in 47 levels ofcertification throughout the state.

PETROLEUM ANDCHEMICAL SAFETYThe Division of Petroleum and ChemicalSafety (DPCS) regulates underground storagetanks (USTs) containing petroleum and haz-ardous substances to protect public healthand safety. The Division works under a com-prehensive program combining new and ex-isting federal and state rules and legislation tohandle tank management, emergency re-sponse, and financial responsibility require-ments. The mission of the DPCS is to protectagainst threats to human safety and contam-ination of the environment that can occur bythe underground storage of petroleum prod-ucts and other hazardous substances throughprevention, education, and enforcement.

2011 Highlightsn Reviewed and processed 456 eligibility and

deductibility applications.

n Storage Tank Safety Specialists performed3,151 initial facility certification auditsperformed.

n The Division processed 3,017 Freedom ofInformation Act requests.

Under the supervision of the DPCS, theChicago Fire Department and EnvironmentalDepartment conducted 318 permitted under-ground storage tank inspections and respondedto 10 emergencies. The Chicago EnvironmentalDepartment issued 249 permits for tank instal-lations, removals, upgrades/repairs, relines, andabandonments-in-place. Chicago also respondedto 36 field investigations and conducted 358initial certification audits.

ExperiencesAt the end of calendar year 2011, the Divisionmaintained registration on a total of 21,040

active petroleum and hazardous substancetanks. The Division also maintains more than40,000 facility files regarding USTs in Illinois.During the year, DPCS conducted 6,767 USTinspections. These inspections include permit-ted activity, multiple certification audits, outof service verification, tank testing verification,and re-inspection of Notice of Violation(NOV) issues. The Division conducted 538UST field investigations, issued 2,701 NOVs,and responded to 12 emergency responses.The Division also inspected 3,790 Motor FuelDispensing facilities and issued 1,199 NOVs. Asthe result of the Notice of Violation process,the Division Red Tagged more than 528 tanksin 2011. Red Tagging prohibits the deposit ofproduct into the UST until all deficiencies arecorrected.

PUBLIC EDUCATION ANDMANAGEMENT SERVICES Fire Equipment Exchange ProgramThe Fire Equipment Exchange Program beganin October 2003. This program connects surplusfire equipment with Illinois fire departments inneed. Since its inception, over 380 departmentshave listed equipment on the Exchange, andmore than 12,000 pieces of equipment havebeen donated to more than 220 departments.During 2009, the Fire Equipment ExchangeProgram was expanded to allow fire depart-ments the opportunity to list equipment onthe exchange for donation, sale, or trade.

In 2011, over 200 items were listed on the ex-change which included the following: 4 firetrucks, over 50 self-contained breathing appa-ratus, a rescue boat, more than 100 sectionsof hose, rescue tools, and numerous otherpieces of fire equipment. The total value wasin excess of $3 million dollars if the equipmentwas purchased new.

Smoke Alarm/Battery Give-AwayOver 60 fire departments participated in theOffice of the Illinois State Fire Marshal (OSFM)Smoke Alarm Program in 2011. Through thisprogram, fire departments received and dis-tributed approximately 1,000 smoke alarms intheir communities.

In addition to smoke alarms, OSFM distrib-uted over 1,000 new batteries for existingsmoke alarms through the generosity ofEnergizer Batteries.

Illinois Fire MuseumThe Illinois Fire Museum has been open to thepublic since 1994 and available for tours byappointment. The Museum is located in a firestation built in 1938 for use by the Illinois StateFair Fire Department at Main and CentralAvenues on the state fairgrounds in Springfield.It houses an authentic 1857 horse drawn handpumper, complete with water buckets. It ishome to one of the nation’s largest fire depart-ment patch collections; this exhibit includespatches from throughout Illinois, the UnitedStates, and some foreign countries. Additionally,authentic fire scene photos, fire helmets, an-tique and contemporary uniforms, call boxes,nozzles, and fire extinguishers each have theirown story to tell as they decorate the walls andexhibits of the Museum.

Children can test their fire safety knowledgein the children’s interactive, educational com-puter room. Here, children learn about hazardsin the home, fire safety behaviors, and tips onhow to react if a fire occurs.

In 2010, the Wenona Fire Department loaneda horse drawn ladder wagon to the museumfor display. The ladder wagon was used inWenona for several years during the late 1800’s

to the early 1900’s. The ladder wagon was builtby the Chicago Extinguisher Company. Twobronze horses to complement the ladderwagon were added to the display this year.During 2011, the Museum was host to over3,000 visitors, most of which visited duringtheir trips to the state fair in August.

Remembering When™ Remembering When™ is a fire and fall preven-tion program for older adults. RememberingWhen™ teaches protection from fires and fallsthrough trivia quizzes, humor, and nostalgicinteractive games. The state of Illinois, as wellas the nation as a whole, has brought issuesregarding our senior citizens to the forefront,and the Remembering When™ program playsan integral part of the process. Our intent isto work with the fire service to keep our olderadults living independently in their own homesfor as long as possible, thereby saving consid-erable tax dollars. The OSFM has assigned astaff member as the Illinois RememberingWhen Coordinator.

n The annual Remembering When Conferencewas held in Urbana in June 2011. Over 50Remembering When participants attendedthe conference to learn new practices,coalition building, and community training.

n The Remembering When Coordinatorhosted two New Community Trainings.A total of 53 participants attended thetrainings.

n In 2011, Remembering When gained 28communities, growing the program 28%.

n Several conferences were attended by theRemembering When Coordinator to pro-mote the program.

Risk Watch™Risk Watch™ is a comprehensive educationalprogram. Risk Watch™ gives children and theirfamilies the information and support theyneed to make positive, health-promotingchoices about their personal safety and well-being.  The Illinois Risk Watch Champion Man-agement Team (CMT), and the Illinois RiskWatch Ad-Hoc Committee is coordinated byOSFM. The OSFM has assigned a staff memberas the Illinois Risk Watch Coordinator.

n The annual Illinois Risk Watch conferencewas held in Urbana in June 2011. Duringthis annual conference, Risk Watch com-munities gathered for a day of idea sharing,key note speakers, new Risk Watch initia-tives, and progress of the program throughNFPA. Over 50 Risk Watch participants at-tended the conference.

n Several Risk Watch teacher trainings wereconducted throughout the state in variousRisk Watch communities.

n Several conferences were attended by theIllinois Risk Watch Coordinator to promotethe Risk Watch program and gain newcommunities.

n Smart Technology Trainings were heldacross the state by the Risk Watch Coordi-nator. Seven were administered with a totalof 78 participants.

n Three New Community Trainings wereheld in 2011. Attending were 38 partici-pants, with 15 new communities added tothe Risk Watch program.

n Illinois Risk Watch Statistics:

Illinois Risk Watch communities grewover 25%.

Over 40 Risk Watch teacher trainingswere held by communities.

Over 20 promotional events were at-tended by the Illinois Risk Watch StateCoordinator and CMT, including con-ferences and speaking events.

Illinois Risk Watch was promoted atboth State Fairs.

Look Up!Look Up! is a campaign designed for collegestudents living in both off-campus and on-campus housing. The main statement LookUp! makes is to remind students to make surethey have a working smoke detector. TheOSFM has teamed up with Kathleen Moritzin implementing this campaign. An OSFMemployee has been assigned to this campaign.

n September was proclaimed College FireSafety Month, and September 22nd wasproclaimed “Look Up-Pay It Forward Day”by Governor Pat Quinn.

n To date, over 250 colleges, universities, andhigh schools around the state have re-ceived Look Up! posters to display aroundtheir campuses and housing units.

n During College Fire Safety Month (Septem-ber), the Look Up coordinator, along withother OSFM employees and KathleenMoritz held a Look Up! coordinated effortat DePaul University in Chicago. This in-cluded canvassing of neighborhoods, amedia junket, and smoke alarm give-away to needy students. Over 300 smokealarms were distributed along with 1,000informational door hangers.

18th Annual Illinois FallenFirefighter Memorial, FirefighterMedal of Honor Awards Ceremony Firefighters from across the state gathered inSpringfield to remember nine firefighters who

died in service and honor 36 other firefightersfor their outstanding bravery and service to theircommunities during the 18th Annual IllinoisFallen Firefighter Memorial and FirefightingMedal of Honor Ceremony.

The Medal of Honor award, the highest awardgiven to firefighters by the state of Illinois, waspresented to seven firefighters who were se-lected for their acts of outstanding bravery orheroism, by which they demonstrated selfless-ness and personal courage above and beyondthe call of duty, under adverse conditions, withthe possibility of extreme personal risk.

Nine firefighters, who died in, or as a resultof, the performance of duty, were posthu-mously honored during the ceremonies astheir families were presented with the DutyDeath Gold Badge by the State Fire Marshal.Certificate of Recognition awards were givento six firefighters acknowledging their dedi-cation to the principles and traditions of thefire service. Eleven firefighters received theFirefighter Excellence Award for an act ofservice in the line of duty, demonstrating ex-cellence and professionalism in service to thecitizens of the state of Illinois. In addition, 13firefighters were recognized for acts of hero-ism or bravery that clearly demonstratedcourage and dedication in the face of danger;they were awarded the Medal of Valor.

The awards presented recognized heroic ac-tions during 2010. Nominations were reviewedand winners selected by the ten-memberIllinois Firefighting Medal of Honor Commit-tee. Due to a large increase in the number ofparticipants in the ceremony, the ceremonywas moved to a larger venue, the Prairie CapitalConvention Center.

SPECIAL PROJECTS

Small Fire-Fighting and AmbulanceService Equipment Grant ProgramThe Small Fire-Fighting and Ambulance Serv-ice Equipment Grant Program provides grantsup to $26,000 for the purchase of small fire-fighting and ambulance equipment by a firedepartment, fire protection district, or town-ship fire department that is a unit of local gov-ernment or a volunteer, non-profit, standalone ambulance service. Applicants mustprovide fire suppression or emergency medicalservice in a geographical area within the stateof Illinois. In Fiscal Year ‘11, this program wasnot offered.

Subject to the availability of funds, the Officeof the Illinois State Fire Marshal (OSFM) willissue an e-mail notification to all fire chiefs inthe state of Illinois announcing an open ap-plication period. Ambulance Services maysubmit contact information to our office fornotification. During an open application pe-riod, applications will be available on theOSFM website and portal. Applicants who arerequired by the Fire Investigation Act [425ILCS 25/6] to report fires to the OSFM mustalso have fully participated in the NationalFire Incident Reporting System (NFIRS) for aminimum of two years immediately prior tothe date of the grant application.

Eligible applications shall be assessed by blindreview, meaning the Committee shall not seethe name, address, or any specific informationwhich identifies the applicant. The Committeeshall review and rank the applications based onassessment of need and information providedin the grant application.

provides interest-free loans, up to 20 yearsand $250,000 for the purchase of fire trucksby a fire department, fire protection dis-trict, or township fire department that is aunit of local government. Applicants mustprovide fire suppression in a geographicalarea within the state of Illinois. In Fiscal Year'11, this program was not offered.

n The Ambulance Revolving Loan Programprovides interest-free loans up to 10 yearsand $100,000 for the purchase of ambu-lances by a fire department, fire protectiondistrict, or township fire department thatis a unit of local government or a volunteer,non-profit, stand alone ambulance service.Applicants must provide emergency med-ical service in a geographical area withinthe state of Illinois. In Fiscal Year ‘11, thisprogram was not offered.

n The Fire Station Revolving Loan Programprovides interest-free loans up to 25 yearsand $2,000,000 for the construction, reha-bilitation, remodeling, or expansion of a firestation or the acquisition of land for theconstruction or expansion of a fire stationby a fire department, fire protection dis-trict, or township fire department that is aunit of local government. Applicants mustprovide fire suppression in a geographicalarea within the state of Illinois. To date, thisprogram has not been funded.

TECHNICAL SERVICES

2011 HighlightsThe Technical Services Division of the Officeof the State Fire Marshal serves to supportother operating divisions within the Agency(primarily the Divisions of Fire Prevention andPetroleum and Chemical Safety) as well as thefire service and general public with technicaland engineering expertise in the form of planand application review for regulated occupan-cies and installations. Technical Services alsoprovides expert advice in the Agency’s rule-making processes for a variety of subjects in-cluding: underground and aboveground tankstorage regulation, LP-gas regulation, motorfuel dispensing facility operation, Life SafetyCode application, and myriad other fire pre-vention and petroleum and chemical safetyrelated issues.

This Division is managed by an Illinois regis-tered professional fire protection engineer(P.E.) and staffed by a variety of personnel in-cluding engineers, engineering technicians,and fire protection specialists. Personnel in thisDivision conduct plan reviews, assist with in-terpretations of applicable codes, assist oper-ating divisions with instruction of fieldpersonnel at training sessions, and performspecial functions for the Agency including rep-resentation on state and national code com-mittees and task forces as well as makingpresentations to the public and the regulatedcommunity relative to technical issues. Thework of the Technical Services Division is con-ducted from both the Chicago and Springfieldoffice locations.

n Analysis of submitted “Fire Plans” for Illinois’correctional facilities.

n Preparation and delivery of training pro-grams and materials for the Agency’s fieldinspection workforce to familiarize themwith applicable codes and changing tech-nologies in an effort to ensure consistencyin code enforcement across the state.

n Preparation and delivery of training pro-grams for local fire service personnel rela-tive to public school fire safety inspectionsas mandated by state statute.

n Assisting operating divisions with the de-velopment of administrative rules, informa-tion reports, policy statements, and/or webpage content to clarify technical and coderequirements. This includes developmentof technical content for “Frequently AskedQuestions” documents posted on theAgency’s website.

n Representing the Agency on state and na-tional task forces and code developmentcommittees relative to fire prevention andpetroleum and chemical safety issues. Thisincludes representation on National FireProtection Association (NFPA) and Inter-national Code Council (ICC) committees.

n Providing expert testimony at appeal hear-ings and court cases.

Possibly most indicative of the work of the Technical Services Division was the handling of telephone, e-mail, and written inquiries from occupancyowners, local enforcement agencies, contractors, architects, and engineers relative to applicable rules, interpretation of codes, and explanationof OSFM policies and procedures. Fire EquipmentDistributor Advisory BoardThis Board serves in an advisory capacity tothe Office of the Illinois State Fire Marshal. TheBoard is made up of a variety of members withan array of expertise and knowledge in theirindustry. They work with the Agency in pro-posing rule amendments, providing guidanceand consultation as it regards the industry andmake suggestions for improving standards inlicensing. (225 ILCS 216).

Elevator Safety Review BoardThe Elevator Safety Review Board was createdby Public Act 92-0873 to adopt rules consis-tent with the provisions of the Act and for theadministration and enforcement of the Act.The Board has the authority to grant excep-tions and variances, hear appeals, hold hear-ings, establish fee schedules, and authorizedcontractor, mechanic, and inspector licenses.
