Our Patient, Family & Public Advisory Council was

formed in the fall of 2016 to advise senior leaders,

health care providers, staff and physicians on

policies, practices, planning and delivery of patient

and family-centered care. It is a place to ensure

patients, families and communities are meaningful

partners at all levels of service planning, allowing

their perspectives to be heard and thoroughly

considered in decision making.

Care Where You Live

NSHA supports an evidenced-based,

person-centered philosophy called

Home First.

When a person is admitted to hospital,

we start planning for discharge by

asking “What can we do to help this

person return home?”

We know most people want to live at

home with family and friends as long as

possible. Our goal is to make that

happen for more people.

Care where you live.

Home First

• Supports individuals to return and stay home whenever

possible

• Involves individuals and families in setting realistic care plans

• Supports decision making about life-changing options, such as

a move to long-term care, from home

• Allows individuals to wait at home for long term care placement

to their preferred facility, reducing transfers

• Reduces risk of hospital-acquired infections and deconditioning

through timely discharges
