This Guidebook contains Voice Intrastate Rates and Charges for services offered on a Detariffed basis effective October 1, 2013.

Under certain circumstances, funds provided under the American Recovery and Reinvestment Act of 2009 (“ARRA”) may be subject to certain restrictions, requirements and reporting obligations. AT&T may be subject to

some of these restrictions, requirements and reporting obligations when Services and Service Components are purchased with ARRA funds. In order to comply with the restrictions, requirements and reporting obligations

associated with the use of ARRA funds (if any), AT&T must be apprised of them before provisioning the Services or Service Components. Accordingly, the Services and Service Components provided under this Guidebook shall

not be used to support the performance of any portion of a project or program which has been funded in whole or in part with grants, loans or payments made pursuant to the ARRA, without the prior written agreement of AT&T and Customer regarding any specifically applicable terms, conditions and requirements. Customer shall provide AT&T

with prior written notice before placing any order that may be funded in whole or in part with ARRA funds. If Customer fails to provide such prior written notice of ARRA funding; or if the parties cannot agree on

the terms and conditions (if any) applicable to an ARRA funded order; or if any terms, conditions or requirements (other than those to which AT&T specifically agrees in such separate writing) are found to be

applicable, then AT&T may, in its sole discretion, reject such order or immediately terminate provision of any affected Service or Service Component without further liability or obligation.

Calling card usage rates for Customers that subscribe to any of the Company's High Volume Calling plans are the same as the usage rates that apply to 1+ outbound calls originating via Switched Access and billed under the High Volume Calling optional calling plan selected by the Customer. For all other Operator Toll Assistance Services calls completed via a Group 2 Toll Free Access Number, the usage rates and per call charges are the same as the usage rates and per call charges described in Section 4.1.2 of this Guidebook.

SECTION 5 - MISCELLANEOUS CHARGES

5.1 Charges Related To Customer Activity

5.1.1 Return Check Charge

The Company will assess the Customer a return check charge for any check that is returned for any reason by the financial institution on which it is drawn for up to the maximum amount allowed by law.

5.1.2 Late Payment Charge

LEC Billing Arrangement - the rules and regulations applying to rendering and payment of bill and late charges are the same as covered in the applicable LEC tariff and/or Service Guide

Other Billing Arrangements - If the bill is not paid within thirty (30) days from the invoice date, the Company may impose a late charge on the delinquent amount. A late charge applies to any past due balance. The Company may charge a late charge of $5.00 or 1.5% per month, whichever is greater. The one-time penalty shall apply on the undisputed amount or on the disputed amount if a dispute is resolved in favor of the Company. When another telecommunications carrier provides the billing function on behalf of the Company, the other carrier's late payment charge applies.

5.1.3 Deposits

See Section 2 of the Company’s Voice Product and Reference Pricing Guidebook for rules and regulations governing deposits.

5.4 PIC Change Rebate

If a local telephone company is requested to change the subscriber's PIC from one long distance service provider to another long distance service provider, the local telephone company may charge the Customer for the PIC change. If a Customer incurs such a charge from its local telephone company for changing the PIC to the Company, the Company will rebate that charge to the Customer. The rebate will be in the form of a credit on the Customer's bill. The credit will appear within two (2) billing cycles after the Customer provides the Company proof that the local telephone company billed the Customer for the PIC change.
