And Interfaith Medical Center isn’t your typical hospital ED. Situated in the heart of the Bedford-Stuyvesant section of Brooklyn, Interfaith sees close to 50,000 ED visits per year. Not too bad for a mid-sized, 287 bed hospital. Interfaith recently partnered with T-System to enhance the T-Sheets, their paper template solution, which went live early in 2009. Following the implementation of T-Sheets, Interfaith’s ED team and T-System worked very closely to roll out “T on Demand” at their first beta site. T on Demand is the new template printing solution, built on the ExitCare discharge instruction platform. Interfaith went live with an in-patient enterprise medical record in October 2009. Implementation of the Emergency Department component of the enterprise HIS, however, is not scheduled until late 2010 at the earliest. This implementation schedule opened the door to for T-System to build upon an already well received ExitCare patient discharge solution while addressing Interfaith’s desire to electronically print T-Sheets rather than store them on the normal shelving units. Furthermore, Interfaith wanted to have patient information from their HIS populate the T-Sheets at the time of printing. Enter the new T on Demand solution from T-System, which expands on the functionality of the ExitCare application to print all of the organizations T-Sheets and most commonly used forms from an ExitCare database and pulls selected information from the HIS via a HL7 interface. The goal: simplify and streamline operations, improve efficiency, improve revenue through better documentation. The implementation process: optimistic and challenging. Interfaith challenged T-System with an aggressive implementation timeline. This required the creation of a phased implementation schedule to address all of the desired system functionalities. Phase I included the T on Demand functionalities for both the physician and nursing notes and integration with the HIS to pull data into the ExitCare import list and patient dashboard. Phase II incorporates a NPR report from the HIS to ExitCare which will build upon the data stream created in Phase I to include the information required for the triage component of ExitCare. The end result of information brought over from the HIS will be two-fold. First, Phase II will automate the data pull to the ExitCare triage screen to eliminate nursing work duplication. Second, we will use that information to automate the selection of the T-Sheets through the use of an algorithmic T-Sheet selection, and automatically populate the T-Sheets with the data. Phase II is expected to go live by the early part of Q2 2010. During the implementation, many lessons were learned that would help both T-System and Interfaith develop best practices for development, implementation and workflow at other sites. Staff training should be carefully evaluated and planned to address challenges from both the operational and financial impact of the projects successful deployment. Interfaith’s ED nurses all required training on both the use of the ExitCare platform and the specific functionality of their role specific use of the system. The amount of training required and the limited availability of the staff for formal training were challenging and potentially could have impacted the success of the project. That is when the experienced folks in the T-System training division proposed mandatory online training prior to any live sessions. Interfaith ran with this approach and was able to deliver an almost perfect attendance record!
