Interviewer Instructions
Question Objectives Wave 1
The question numbers are those used on the paper questionnaire.
4.TWINTRIPIf the sampled baby is one of twins or triplets, theremaining questions should be asked with respect to thebaby that was born first.
7.BIRTHDATIf the date of birth given by the mother at the interviewdoes not agree with the date on the birth register, acceptthe date given by the mother.
10a-10e.Household BoxPlease enter every member of the household except the babyin the household box. The baby's name, age, sex and date ofbirth have already been recorded at NAME1 to BIRTHDAT.
10a.NAMEYou will need to refer to each member of the household asyou fill in the household box, so enter their names or inlarge households with names which are difficult to spell,you might find it simplest just to record theirrelationship to the mother as the identifier.
10c.AGEThe age of the respondent and her husband/partner and theirchildren should be given accurately, but it is acceptableto give an estimate of the ages of other family members.
12.ETHNICYou may have already asked this question at the doorstep-sift. If so, just record the answer.

28-29.MTOKEN1, MTOKEXCHFamilies receiving income support are eligible for freeinfant formula for babies under a year old. Breastfeedingmothers may take their baby's entitlement of infant formulain the form of liquid milk to drink themselves. Families inreceipt of Family Credit are entitled to purchase infantformula at a reduced price.
In most of the country, mothers get free or reduced pricedried milk from a maternity or child health clinic. Somehealth authorities have delegated the distribution of driedmilk to high street pharmacists. In remote areasdistribution points are sometimes church halls etc. Liquidmilk is available under the Welfare Food Scheme frommilkmen and some shops.
31.HOWSTERIf the answer is Milton, probe whether tablets or fluid areused.
37.BOTLATERFeeds should be coded as kept for later if they are keptfor an hour or more. Do not count feeds which are kept andthen thrown away.

176.DRUGEVRThese are Asian drugs most likely to be used in Bengalisociety. Shupari is betelnut. Chown is lime. Paan is amixture of green leaves, lime (chown), betal nuts and othernuts and grains and is taken after meals. When chewed themixture turns a brilliant red colour. Shadda is a chewingtobacco.
179-180.MUMEDEND-MUMEDSTThese questions are asked so that a comparable measure - number of years of full time education - can be calculated
for respondents educated in different countries. Pleaseexclude nursery education.

226-242.BABY MEASUREMENTSThe anthropometric measurements are described below. Werecommend that the measurements are entered on themeasurement sheet and that you input the data later so thatthe computer is not left running while you takemeasurements. Some mothers will ask for their baby'smeasurements. A chart (Show Card D) is provided so that youcan convert kilos into lbs. and ozs.
248-249.You are asked to code whether an interpreter waspresent, as this may affect the length of the call, and whothe interpreter was, which may be useful to you (or anotherinterviewer) at subsequent waves of the survey.

2. Household Box
The questionnaire begins by looking at changes in the householdsince Wave 1. The household box from Wave1 will be brought up onthe screen. Every member of the household is entered in thehousehold box except the baby. For each person in the householdbox (except the mother) you will ask/check whether they arestill living in the household. Their sex, age and marital statuscannot be changed. It is possible to change relationship tomother on the screen, although changes will not be recorded onthe database.
Then there is a question asking if there are any new people whohave moved in since Wave 1. If there are, you will fill in a newbox.
After completing the household box(es), enter the number ofpersons in the household including the baby at NPERSON2. This ischecked against NOINHH which is computed in the program from thehousehold boxes(plus the baby).
3. Feeding (Q 23-48)
Many of the feeding questions are similar to Wave1. There arenew questions about- number of times a day baby is breast/bottle fed (NFEEDS)- how much bottled milk baby drinks a day. If baby has
different amounts at each feed, try to work out an averageover the day. This can be approximate. (NOUNCE)
- some new infant formulas in MILKTYP2 question- how old the baby was when mother started using the current
powdered milk (this could be from day 1) (STARTBOT)
4. Solids (Q 49-63)
The solids table will be more important at this Wave. We need toknow all the solid foods that the mother gave to her babyyesterday and what kind of foods they were. Please give theamount in teaspoons. Use Show Card E to help the mother convertamounts from dessert spoon or jar to teaspoons.
After this is question 57 (MILKMIX) which asks if the motherever uses milk to mix up solid food. This is sometimes specifiedin directions on e.g. a cereal packet. Baby milk is infantformula.
There are some questions (Q 59 SOLIDOLD to Q62a XFACTORS) whichare only asked if the mother regularly gives her baby threemeals of solid food a day. They ask how old the baby was whenthe mother started giving three meals a day, and what factorsthe mother takes into account when choosing food.
FAMDIET (Q 63) asks whether most people living in the householdeat only a vegetarian diet.
5. Drinks (Q64-69a)
There are more questions at this wave about water given to thebaby. We especially want to find out about mineral water whichwill be coded at Q66 (WATERTYP), and the brand at Q82(MINWATER).
There is a new code at Q65 (WATER2) for herbal drinks. If themother does give herbal drinks you will ask about these anddrinks other than plain water or mineral water in the drinksgrid.
6. Advice/checkups (Q 78-104)
We ask about the advice the mother has received since Wave1. Ifshe doesn't understand English very well or not at all, we askabout the language used by the health visitor/doctor/nurse.There are questions on whether the mother attends a local groupfor mothers and babies.
7. Public Places (Q 105-108a)
These questions ask whether the mother takes the baby out andwhether she has problems feeding him/her when out. A publicplace is anywhere the mother goes with her baby in public.Visiting a friend/family at their home would not be a publicplace.
8. Employment and accommodation (Q 109-134)
These are asked again if mother/husband/partner's job situationhas changed or if she has moved house.
Question Objectives Wave 3
1. Recall questions on starting breastfeeding
Respondents who first started breastfeeding more than 24 hoursafter the birth will be asked to recall whether this was beforeor after leaving hospital. If it was after, we ask the reasonfor the delay. We are especially interested in any culturalreasons.
2. Bottle feeding
There is a new question asking how mothers choose the type ofinfant formula that they use. Mothers who give their babies cowsmilk will be asked when they first did so.
3. Solids
As before, we ask the mother to tell us all the solid food shegave the baby yesterday. We suggest that it is easiest if you gothrough the day chronologically.
Rather than typing in all the details of the food, you will beasked to code it. PLEASE READ THE SECTION ON COMPUTER ASSISTEDCODING AND PRACTISE BEFORE GOING INTO THE FIELD
In order to code the food you will need to find out from themother whether it was a commercial baby food, fresh or homemadefood or other purchased food. The hardest distinction is betweenfresh/homemade food and other food. As a general rule, any foodwhich requires cooking or preparation (as opposed to simplyheating up) is homemade, but food that is ready to eat from apacket or tin etc is in the other category. Because this is nota nutrition survey, those of you who have worked on ToddlersDiets will find that we do not require so much detail.
We have altered the question on amount of food given. Whilstmost quantities will still be entered in teaspoons, there isalso an option to code rusks, pieces of fruit or vegetables,slices of bread separately. For example, select rusk, then atnext question select how much of it was eaten eg. 1/2. Showcard'H', gives the number of teaspoons for various baby food jars.
4. Food frequency table
After the questions about solid food, there is a table askinghow often the mother gives the baby a list of different types offood. Please use showcard 'F' (where the respondent readsEnglish), to speed up the response, because it can becomerepetitive. You will also be asked whether the mother usuallygives the baby commercial baby food, homemade food etc. In somecases where the answer is obvious we have imputed it, and forthese foods, you can simply press <Enter> without having to askthe question.
5. Drinks
There are fewer drinks questions in Wave3, but we would like youto code all drinks that the mother regularly gives the baby,using the same computer assisted coding procedure as for thesolids table. We have tried to develop a coding frame thatrequires minimal coding for the interviewer, so you will findthat for some drinks you are only required to enter one digit.However due to particular interest in herbal drinks at theDepartment of Health, for these you will need to code whetherthey are homemade or commercial, the flavour, and if commercial,which brand. If the code for any of these questions is other,you will be asked to give details at the next question.
As part of the same table we ask how frequently the mother givesthe drink, so please use showcard 'F' (same codes as forsolids).
To try and encourage respondents to think why they give drinks,we have produced showcard 'G', with the usual precodes. If yourrespondent does not read English, you or the interpreter mayprompt some of these codes.
6. Family diet and feeding practice
There is a whole section covering some wider feeding issues:
1 Family diet, and whether Muslims eat Halal meat.
2 Who does the shopping and where the food is bought. We wantto know where respondents buy most of the food for both thebaby and the family (one code only). If the distinctionbetween supermarket and general food store is unclear,please use the 'Other' category. In general a supermarketis a large, chain store; and food stores are smaller with amore limited range of goods (but can still be self serviceand even part of a chain).
3 Who does the cooking and whether the baby eats with thefamily.
4 Whether the baby has started using a cup or feedinghis/herself.
5 How the mother chooses what food to give to the baby, andwhether she avoids particular ingredients. We would like totry and focus on ingredients here rather than specificfoods. You will already know whether they give the babymeat, fish and other foods (hence the optional start to thequestion), but here we want to know about sugar, additivesetc, as well as other specific foods.
7. Advice
These questions are similar to previous waves, but we've alteredthem slightly to try and encourage respondents to give us afuller response.
8. Employment
If the mother is working, there are questions on childcare.Before asking about the husbands employment there is a questionbeforehand to check whether he is living there. You only have toactually ask it if you have not been able to establish thisduring the interview.
9. Interpretation
There is a new code in the final questions for interviewers whodo the interview in another language.
10. Measurements
There are some additional codes for factors that may haveaffected the measurements.
Babies will now probably be old enough to sit on the scales,rather than lying in the scoop.
11. Interpreters
We are aware that some interpreters have been prompting answersto certain questions. Before you go out with an interpreter,please could you talk to them about the importance of askingquestions the same way to all respondents, and ask them notprompt unless the question says so.
Question Objectives Wave 4
1. General points
Coding frames
Some of the coding frames have been revised to take account ofthe most common answers given in the 'other' category. Pleaseread through them before you start interviewing. They might looka bit daunting at first, but we hope that it will save you timetyping in text.
Food and drinks diaries
A number of mothers will be working at this Wave, and may notknow what foods and drinks the child has consumed the day beforethe interview. Where you contact the mother beforehand, we wouldlike you to ask them to ask their childminder to write down whatthe child eats and drinks the day before. You may also ask othermothers (who are not working) to do the same, if you feel theywill be happy to do this, but this is optional.
2. Milk
If the child is still given milk, we want a rough estimate ofhow much they drink each day. If the child drinks from a bottle,we ask how many bottles they drink; and if the child drinks froma cup we ask how many cups they drink. If both, then we askabout bottles and cups. If either answer is less than one a day,please code '0'.
When we ask whether the child uses a cup or glass, this alsoincludes beakers, mugs and anything else that is similar to acup or glass.
We are interested in the age at which the child starts drinkingcows milk. This question is asked of mothers who were not givingthis at Wave3, but who say they are at Wave4.
3. Drinks
The Drinks questions are now asked after the questions on milk.
At this Wave, rather than asking about all drinks they givetheir child, we ask about drinks given the day before. We hopethis will enable mothers to remember the information that youneed to code. If the same drink has been given more than once,please code it each time.
The drinks codes have been simplified, especially for codingfruit drinks. When you go into this section you have to selectwhether it is pure/fresh fruit juice; a concentrated baby drink;or another concentrated drink (eg. squash) with/without addedsugar. We hope this is easier than at Wave3. The instructions onhow to code are included here at Appendix A. The coding framesare included in the paper questionnaire.
There are new categories for fizzy drinks, and milk-based drinkssuch as hot chocolate or horlicks. Please code all chocolatedrinks as 81, even if milk is not added. Drinks such as yoghurtdrinks and lassi should be coded 83 - other milk-based drinks.
There are new questions about vitamin drinks given at meals(because iron is digested better when taken with vitamins), andalso about drinks given when the child goes to bed.
After the 'drinks diary', we have introduced a small tableasking how often certain types of drinks are given (as forfood). There is a new code 'three or more times a week'replacing 'most days'. Please use the revised Showcard F forthese questions and for the food frequency table.
4. Food
We have also revised the food codes. Again this is to simplifythem and add in new foods. There is a section on Asian foods.This is designed to help you code these foods easily, notbecause we want to make any distinction between what is andisn't Asian food. If you don't find it helpful, then you don'thave to use it. A suggestion for coding Asian foods that you arenot familiar with, is to take along a menu from a local curryhouse which gives the ingredients in English.
If a food is eaten several times a day eg. biscuit, please codeeach time it is eaten.
Where possible, we have tried to design the coding frame so thatyou can code both meals and snacks with one code only.
Foods that are hard to codeBelow is a list of foods which you have mentioned as hard tocode, and the codes that you should use:
Breakfast

We hope you will be pleased that the much loathed 'Amount'question has been replaced. We still need a rough indicator ofwhether or not the child had just a 'taste' of the food. Thequestion now asks whether the child had one or two mouthfuls ofthe food, or more. (Question 50d)
5. Choosing and preparing the child's food
Again we ask some of the questions about whether the child eatswith the family, and foods which mothers don't give the child.We are aware that some respondents find these questionsdifficult to understand, but please persevere because theanswers we have from previous Waves are useful. At the end ofthis section there are a few questions from the Toddlers Dietssurvey about how easy the child is to feed.
For the question 'compared to other children of the same age,how easy or difficult do you find it to feed your child?', ifthe mother doesn't know any children of a similar age, pleasecode 'don't know'.
There is a question about foods the child will not eat. Pleasecode the TYPE of food that s/he won't eat eg. raw carrot = freshvegetables. We will not interpret this to mean that the childwill not eat any fresh vegetables.
6. Vitamins
We only ask about whether the child is taking extra vitamins,not about mothers.
7. Advice
Who gives help and advice.As for Wave3, we ask firstly about friends and family (Q76) andthen about others (Q78). Please do not code friends and familyin the second question (Q78).
Checkups, going for advice (Q81)For mothers who read English, there is a new showcard, 'I', forplaces where they may have taken the child for a checkup etc
We also ask why they went. We expect some mothers to have takenthe child for a one-year checkup since we last interviewed them.
Leaflets, books, magazines & videosIf mothers still mention 'bounty packs' we would like you tocode them according to where they came from eg. hospital orcommercial organisation. By commercial organisation, we mean acompany like Boots or Milupa which produce information formothers. Books from a bookshop would be coded as 'other'.
In the section on problems and advice, we have some newquestions looking back over the 15 months since the child wasborn. We also ask about whether mothers would have like moreadvice about feeding. Please try to answer this question forfeeding issues only, even if they mention other areas where theywould have like advice.
8. Work
There are new questions about who feeds the baby when the motheris at work.
9. More children
Some mothers have already given birth again or are pregnant, orplanning to have another child. We ask whether they are feedingor plan to feed the next child any differently.
10. Interpreters
WhoIntr4 (Question 136)An additional code "other paid interpreter" has been added tothe question on who interpreted. This is to cover thoseinterpreters who are paid on the yellow claims forms - ieinterpreters you found yourselves as opposed to interpretersrecruited by HQ or interpreters from agencies or councils.
