A Message To The Parents

Dear Future Campers Parent,

The Gladiators Academy of Lafayette Little Ninjas Super Summer Martial Arts Camp focuses

on discipline, teamwork, confidence, and functional fitness. The Little Ninjas Program is for

students ages 712 years old. Through the Little Ninjas Super Summer Martial Arts Camp

your child will be encouraged to explore and be creative inside and outside of the academy.

Activities and drills ranging from obstacle courses, trips to the zoo and library, along with

weekly trips to the pool and morning walks will allow your Little Ninja to apply martial arts

skills and techniques to everyday life. Watch as your child grows in confidence, learns how to

deal with bullies, practices Stranger Danger, and begins their journey to Black Belt. Weguarantee that your child will improve leadership qualities and a positive attitude after

attending a summer camp at Gladiators Academy of Lafayette. Thank You for your support of

our program and we look forward to seeing your child to the Black Belt level and beyond.

V.I.P (Very Important Parent) Details

o Parents MUST Sign in and Sign out students every day. All certified pick ups must be on

the verified transportation list and must present ID in order to pick up child.ABSOLUTELY NO EXCEPTIONS!!!

o Before care starts at 7 AM and Camp Activities officially start at 9 AM. Summer Camp

ends at 3:30 PM and aftercare ends at 5:30 PM. Both before and after care are included

in the weekly tuition.

o Parents will be charged $1 per minute after 5:30 PM

o If your child will be picked up before the 3:30 PM dismissal time, a written notification

must be made at least 24 hours before the next camp day.

o On assigned field trip days students are to be at the academy at

8:45 AM. Students who are not there for the final head count will

not be permitted to attend the field trip.

o All campers must bring a lunch every day. There are no peanutproducts or shellfish allowed in the Academy due to possible

allergic reactions from kids. Lunches that require heating should not be packed on Field

Trip days.

o Gladiators will not disperse any medications. Students who take daily medications (i.e.

allergies, asthma, behavioral, etc.) must take without the assistance of a Gladiators

Academy staff member. All daily medication should be dropped at the front desk only.

Do not pack them in your childs belongings or lunch.

o Martial Arts Uniforms and gear should be packed for camp every day. Uniforms include

top, bottom, and belt. Each camper will be assigned 1 official Gladiators Academy Camp

Uniform. Additional uniforms can be purchased at the front desk for $45.00. o Camp T-Shirts are to be worn to the Academy on all field trip days. Each camper will be

assigned one official camp t-shirt. Additional camp t-shirts can be purchased at the front

desk for $20.00

o Parents who volunteer for activities and field trips should notify staff at least 1 week

prior.

o All Summer Camp birthday parties should be scheduled at the front desk at least 2 weeks

Camp Packet

The Camp Information Packet contains critical information such as drop-off and pick-up times, what

your camper needs to bring to camp, etc. The Camp Information packet will be mailed to your address

of record 2 weeks prior to the start of camp. It will also be available for download on the web site or at

the front desk starting on May 20 th. This years Summer Camp will only be offered to 50 students due to

the high demand that we are receiving. We are only interested in providing the highest quality

experience to our students and not just packing a bunch of kids into the camp. We advise you to please

get your deposit and information into our front desk ASAP. Our Camp is filling up fast and we would

love to have your child as a part of this exclusive and unique experience.

Waiver for Summer Camp

I understand that Gladiators Academy of Lafayette assumes no responsibility for injuries orillnesses, which my child may sustain as a result of her/his participation in day camp, athleticactivities, sport programs, the use of any equipment, exercise or other activities. I expresslyacknowledge that I assume the risk for any and all injuries and illnesses which may result from her/hisparticipation in these activities. In consideration of the privilege of participating at Gladiator Academyof Lafayette, I hereby voluntarily release and discharge , its agents, servants and employees from

any and all claims for injury, illness, death, loss or damage which my child may suffer as a result ofher/his participation in these activities. A parent/legal guardian must discuss with GladiatorsAcademy Kids Camp Director director any special conditions or circumstances involving their child.

This must be completed prior to registration.I hereby give my permission to the medical personnel selected by the activity director to order X-rays,routine tests, treatment; to release any records necessary for insurance purposes; and to provide orarrange necessary related transportation for me or my child. In the event that I cannot be reached inan emergency, I hereby give permission to the physician selected by the camp director to secure andadminister treatment, including hospitalization for my child.

I understand that no accident or medical insurance is provided with this activity.I give permission to Gladiators Academy of Lafayette, without limitation or obligation, to usephotographs, film footage or tape recordings, which may include my childs image or voice forpurposes or promoting or interpreting Gladiators Academy of Lafayette programs and release thecamp from any claim of liability to that use. I give my consent for my child to leave GladiatorsAcademy of Lafayette site, participate in authorized Gladiators Academy of Lafayette Academy tripsand to ride in authorized vehicles for the purpose of transportation in connection with the GladiatorsAcademy of Lafayette Summer Camp.

Registration DatesCamper registration is processed on a first-come basis.

Registration begins March 4th, 2013.

Camp Fees: $259 per week per camper, $199 per week per camper for our 4 Week

Block and $189 per week per camper for our 8 week block. No discount for the 2nd child. 50%

discount for any additional sibling. Camp Registration is for 8 weeks, 4 weeks or 1 Week

session of the Martial Arts Kids Camp. We are only offering 50 spots for campers and the

camp will be reserved for people interested in participating in the entire camp. Camp fees

cover the cost of all field trips, camp t-shirt and a uniform for all non-students. Both early

Drop-Off (7:30-9 AM) and Late Pick-Up (3:30 5:30 PM) are included in the price of the Camp
