Bridges on atrophied and jaws with low heightThe use of implants placed at an angle, narrow implants and narrow abutments allows to make optimum use of local bone, reduce the risk of implant placement and enable the fabrication of favorably-priced high-quality fixed restorations on just a few implants.

Removable restorationsComplicated bone augmentation measures can be avoided when fixing prostheses using narrow implants or implants placed at an angle, which securely fixes the prosthesis in the local bone. Excellent posterior support provides a high level of wearing com-fort.

The Off-Peak characteristic enables insertion of these abutments immediately after implant placement/exposure to that repeated traumatization of the soft tissue can be avoided.

Narrow gaps Narrow gaps can be closed in an esthetically appealing manner using the new narrowSKY implants and corresponding SKY es-thetic S and SKY elegance S abutments.

Highly esthetic ceramic restorations Prefabricated, anatomically shaped ceramic abutments are avail-able for highly esthetic restorations and inexpensive treatment with implants. Special cases can be restored using laboratory-fabricated individual abutments, which feature a titanium base.

Improving the bone quality

Compared to conical connections with press fit, the flat connection with protection against ro-tations provides major advantages for prosthetic restorations since the vertical height of the abut-ments is defined, i.e. it is much easier and takes less time

Thanks to the very long tube-in-tube connection (3.5 mm), masticatory forces are directed from the abutment into the implant in a perfect way:

Indication

Prosthetics for every implant position

The crest-like design of the SKY classic implant is particularly suitable for trans-gingival healing, as both the epithelial tissue and the connective tissue are optimally attached. The bone is maintained to a high degree by the blasted and etched surface.

Through the trans-gingival positioning, this implant is also excellently suitable for use as a short implant in combination with other longer implants, which can also easily be placed at bone level. In this way, it is now possible to anchor the 8 mm SKY classic implants at a depth of 6 to 6.5 mm, so that major and elaborate surgery can be avoided. These implants are available with a diameter of 4.0 and 4.5 mm.

Furthermore, the new SKY classic implants simplify implant insertion if the gingiva is only die-cut. In this case, the insertion depth can be controlled more easily than when implants are placed at bone level.

The classic SKY implant is ideal to avoid milling off bone in narrow or uneven ridges.

The machined section avoids accumu-lation of plaque to exposed implant necks.

In transgingival implant insertion, depth control is made easier.

In the case of isocrestal positioning of the implant, the shape of the abutment gains particular importance, in order to achieve optimum attachment of the soft tissue.

The SKY esthetic abutments offer the ideal prerequisite for this. With the concave-convex hybrid form and the built-in platform switch, the surgical need for greater clearance above the implant is satisfied to achieve optimum attachment of the soft tissue, on the one hand, and this form provides the dental technician an optimum prerequisite to achieve optimal aesthetic results, on the other. The shaping of the gingival proportions is performed either using the SKY esthetic gingiva former or the SKY temp abutment with a provisional restoration, or as an individual gingiva former.

Both have the same diameter as the SKY implant platform, so that soft-tissue can neither get caught between the implant and the augmentation while making impressions, nor when integrating the abutments, and thus the precision of the impression and the long-term stability of the implant-augmentation interface are assured.

Moreover the SKY esthetic line offers precision-manufactured impression abutments with corresponding diameters for open and closed impressions. This is an impression system comprising a titanium abutment and a plastic cap which has long and short screws for the respective impressions and which exels thanks to the precision during impression taking.

It is recommended that the transition area between hard and medium-hard bone is initially prepared using the drills D2-D4. If the torque exceeds 45 Ncm when the implant is screwed in, the implant should be removed again and the cavity extend-ed using D1 drills. This procedure ensures primary stability in every case and avoids overloading the bone.

Detachable drill stops

The detachable drill stops are arranged so that they can be easily taken up with the drill and fastened with one hand thanks to the cavities in the OP-Tray insert. The ruler on the compartment for small parts helps to select the correct drill stop easily and quickly. The drilling depth is approx. 0.7 mm greater than the implant length.

Insertion concept

blueSKY - fast, reliable and atraumatic implant placement

Open the new double sterile packaging. Remove the implant with the insertion instrument for the ratchet or the contra-angle.

Use the same instrument to remove the cover screw ...

... and screw it directly into the implant. Thanks to the smooth cone the screw needs to be only slightly tightened and will not get jammed. Recommended max. torque: 10 Ncm.

The conical Torx® allows to hold the implant safely.

blueSKY and SKY classic implants come in double-sterile packaging. They are supplied in a colour-coded carrier with details of the applicable length so that mistakes can be avoided. After opening the Tyvek foil, it can be directly removed using the SKY TK insertion instrument.

The implant is inserted and can be screwed in immediately.

SOLUTIONS

The success of individual and group prophylaxis throughout the world will radically change the spectrum of treatment in dentists’ practices in the coming years. Young patients, who for various reasons suffer tooth loss, will mostly have healthy adjacent teeth, so that a conventional bridge is not indicated for ethical reasons.

Furthermore, the number of young patients in developed countries will decrease with demographic change. In Germany, the number of under-20 year olds will go down from 30 % in 1970 to 15 % in 2050.

With the increasing awareness for oral health, the demands placed on aesthetic and functional results in dental care will also increase. In this case, the provider is required to clarify and take the wishes and fears of the patient into consideration in the therapy plan before treatment begins, and to harmonise these with the financial possibilities available.

Wide-based SKY titanium abutments are ideal for supra-crest implants, as they form a relatively wide crown base at the gingival emergence. With the three available angulations

SKY elegance abutment – White-Shade and Off-Peak

Off-Peak characteristic:

Maximum loading of the implant is reduced due to the cushion-ing effects of the High Performance Polymer, BioHPP. The abut-ment can be inserted as a temporary abutment immediately after the implant has been inserted, and does not subsequently need to be removed. The attached gingiva in the sulcus is not dis-turbed – the optimum basis for long-term bone preservation.

The individual design options and white color of BioHPP provide perfect preconditions for esthetic reconstructions. Using con-ventional dental methods, esthetic, individual abutments can be molded and pressed quickly and safely in the for 2 press unit. The low investment costs of the for 2 press unit and the familiar processing steps enable the use in every laboratory. Moreover, prefabricated abutments in the design of the SKY es-thetic line will be available from autumn 2013. These abutments can be easily modified by the user in accordance with the gin-gival line.

All types of crown and bridge restorations can be placed on the individual and the prefabricated abutments, from BioHPP res-torations with veneers from the visio.lign veneering system to ceramic bridges and crowns. The Off-Peack characteristics of BioHPP enable the fabrication of reliable and durable restora-tions for the entire range of indications mentioned above; such restorations offer patients superior comfort since stress is re-duced considerably during chewing.

Clinical experience with the SKY uni.fit abutment has shown that the bonded joint in the dental laboratory functions very well. Thus, the SKY zirconium abutments have been further developed according to the same concept.

• A prefabricated zirconium abutment is bonded to a titanium base• The form of the zirconium abutment is based on the SKY esthetic Line, however, it has a vestibular sunken shoulder, in order to avoid extensive etching of the zirconium. Costly grinding of the zirconium is eliminated. • The titanium bonding base is also available as a 30° swivelled Torx–R version

so that 12 positions are available for prosthetic restorations. The R-version is marked yellow.

• The bonding base has a reduced platform diameter so that a platform switch results, whereby aesthetic results in narrow gaps and narrow implants are improved.

• Dentine materials are colored.

Important:The SKY zirconium abutments must not be subjected to grinding as the minimum wall thickness is required in order to guarantee long-term stability.

Therapeutic concept... tempting...

Patients who are about to lose all their teeth are a highly interesting customer group for any dental practice. As various scientific studies have shown, these patients are very dissatisfied with their oral health. They often have either a strongly reduced residual dentition, or have severe periodontal damage to the teeth.

Periodontosis is the new widespread disease with a dramatic growth rate in the last few years, which mainly affects patients in the 40 to 60-year old age group, who are in the prime of life.

The incidence of patients with strongly reduced residual dentition is wide-spread. In Germany, the generation “50 plus” has only 4 to 5 teeth per jaw on average and is already edentulous at 64.

These patients are very afraid of losing all their teeth. On the one hand, they still feel quite young, on the other hand, a removable prosthesis means “You are old!”; for this reason, even implant-supported removable prostheses do not meet all patient needs – they want a snug-fitting restoration right from the start, if at all possible.

With SKY fast & fixed, you are certain to be able to satisfy these wishes. Extraction, implantation and provisional restoration with an immediately stable bridge are performed in one day.

This means that patients will no longer have to be without any teeth, even for just a day, and they will always have a secure restoration in their mouth. Psychologically, this is very important. With the provisional restoration, patients already feel a definite improvement in their oral situation and are then often prepared to spend more money for the definitive restoration. These patients are also very pleased to further recommend this form of treatment by word of mouth.

Through the reduced number of implants and the avoidance of augmentation in conjunction with a high degree of standardisation of procedures, both in dental practice and laboratory, SKY fast & fixed is a therapy concept, which, on the one hand, is financially accessible for very many patients and, on the other hand, generates a high contribution per practice hour for both practice and laboratory.

• Occlusal or transversal screw retention with just one abutment. For example, the long-term temporary restoration can firstly be screwed from the occlusal and then the definitive restoration from the transversal direction. The new SKY fast & fixed abutment offers you unlimited flexibility. • Perfectly suited for the following substructure materials: titanium, NPM, gold, zirconium oxide and BioHPP.• Passive fit is always achieved, regardless of the material, as the substructure is bonded in situ.

Self-centering transversal screw retentionTransversal screw retention is a bolting principle. The thread for the bolt screw is located in the bridge framework. The bolt screw (A) and the cylindrical surfaces (B and C) form a compact unit. The prosthetic coping is fixed by triple point attachment with the bolt screw (A) and the short cylindrical surfaces (B and C) to avoid tilting. At the same time, the short cylindrical surfaces ensure that the coping is self-centered (passive fit) when it is placed on. Thanks to the slightly inclined position of the bolt screw, the prosthetic coping is pressed on the abutment platform and gap formation is avoided when it is tightened.

Simple applicationA click that can be easily heard and felt indicates reliable fit when the framework is attached or removed. Reliable retention thanks to passive fit. The design enables the user to remove and reposition the restoration without removing the bolt screw from the framework, which significantly reduces the risk of swallowing the bolt screws. The design also guarantees fast and simple removal and insertion of the restoration.

Improved prosthetic copingsWithin the scope of our ongoing market analysis, we have optimized the prosthetic copings. In the cervical area they have a short cylindrical section with an index to be used as the basis for the temporary restoration. When fabricating the definitive restoration, the cylindrical section can be easily customized to achieve better aesthetic results.

All abutments have the same cone, which can reconcile divergences of +/- 10°.

In addition to the SKY fast & fixed abutment system, we now also offer the SKY uni.cone system with a reduced diameter of 4.5 mm, so that now high-quality esthetic restorations - both occlusal and vertical screw retention types - can also be easily and quickly fabricated and used for atrophied jaws.

The SKY uni.cone abutment system is copper-color anodized for better distinction in the patient's mouh and in the laboratory.

A typical application for SKY uni.cone is the combination with SKY fast & fixed in the lower jaw. Implants placed at an angle are attached to the normal SKY fast & fixed abutments and the SKY uni.cone abutments are placed on the straight abutments in the corresponding height.

Study of 1,224 corpses > 60 years of age in Hamburg, Germany

One would actually expect that most people will become increasingly dissatisfied with their oral situation with a decreasing number of teeth and increasing age. This is not the case, however, as has been demonstrated by the OHIP value, whereby increasing satisfaction has been shown with increasing age. This result is explained in the scientific literature by the fact that in addition to becoming gradually accustomed to an inadequate oral situation, resignation also occurs to a large degree, as dental measures often only have a short-window of success, as the strongly decreasing frequency of consultations to dental practices has also confirmed. In addition, senior citizens frequently have considerably more serious diseases to contend with, such as e.g. cardiovascular disease, diabetes, etc. which is highlighted by the increased frequency of visits to the doctor.

A study of oral health by Dr. Saal in Hamburg on 1,024 corpses at the crematorium brought the alarming result that only 37% of the deceased possessed a good oral state of health, but that 63% had died with a poor or even desolate state of restorations. Incidentally, only 1.3% of the dead persons had implants.

The prosthesis could, however, be stabilised and fixed in older edentulous patients by a few implants and the corresponding retaining elements, such as e.g. locator, stud-head or bar. In patients, who have been edentulous for a longer time, a lot can also be achieved using mini-implants without much surgical effort.

The difficulty with these patients lies in addressing them. On the one hand, they come more seldom to the practice and, on the other hand, it is necessary to convince them that it still pays for them to invest in their oral health and to bring them out of their resignation. A lot can also be achieved by speaking to both the children and the grandchildren. The positive effects on the general state of health, which is brought about by better nutrition and is made possible with a fixed prosthesis, should be particularly emphasized. In this circle of customers, however, recommendation plays a considerable role, as senior citizens like to talk about their health. A successfully treated patient can lead to many more new patients.

In two posters presented at the SKY Meeting 2012, Dr. Wentaschek from the University of Mainz illustrated how Pretension can be significantly reduced by repeatedly tightening the screw. Pretension is the decisive crucial factor for the implant-abutment connection. He therefore rec-ommends the use of different screws in the laboratory and the clinic to avoid screw loosening.

Use miniSKY implants to ensure reliable fixation of the denture and improve the quality of life of your older patients who lost their teeth quite some time ago.

All clinical studies 1 and tests have shown that inadequate fit of restorations reduces the quality of life of patients considerably.

Persons at the age of 70 or older have been edentulous over numerous years and, as a result, the ridge has become very narrow and placement of stan-dard implants always requires complex surgical procedures, such as bone splinting, etc. Moreover, frequently the general state of health of these patients does not permit major surgical treatment. The diameter-reduced miniSKY implants are perfectly suited for these patients since they allow minimally invasive placement. In numerous cases not more than a small incision to control the bone width is required.

The soft line denture worn during the healing phase enables the patient to enjoy enhanced retention of the denture.

At the end of treatment, the happy patient is able to eat, speak and enjoy life without any limitations.

The use of diameter-reduced implants is strongly recommended for certain indications.

With miniSKY we offer you an implant system that is perfectly matched to these indications and results in better quality of life for your patients with just a few components, a modern surface and an advanced and simple prosthetic concept.

miniSKY can be used as a one-piece implant with ball head or as a two-piece implant as well.

The mini2SKY offers a choice of Ø 2.8 and 3.2.

Quality of life in later years

Atraumatic insertion and permanent primary stability make the miniSKY implants ideal for splinting the whiteSKY im-plants during osseointegration to avoid any micromovement in free-end situations. In most cases, our patients are willing to accept the temporary titanium implant.

Splinting is done using composite layered on glass fiber bands and can be compared to a plaster cast.

All of the instruments can be easily assig-ned and identified by means of the images and order numbers.

It is small enough to fit onto any operating table, and is of course easy to clean and sterilize.

Presentation of a problem

Initial situation When making an impression of the mini1SKY im-plants, the o-ring (SKY-OR55) is placed over the ball head

At the same time, the impression material brecision implant heavy (580 BH38 0) is added to an impres-sion tray and applied to the area being molded.

Once the impression material has cured, it is possible to carefully remove the impression tray. The o-rings remain in the impression. The torx of the mini1SKY implants is clearly recognizable and aids reposition-ing of the laboratory analog.

When repositioning the implant analog (m1SKYXIA), it is necessary to bear in mind that the torx geometry in the impression material corresponds with the ori-entation of the torx from the implant analog.

The mini²SKY gingiva former completely covers the high preci-sion torx and supports and ideally molds the soft tissue.

The use of the slimline mini²SKY impression abutment enables simple and fast impressions even of very narrow gaps. For the impression of mini²SKY implants, the mini²SKY impression abut-ment, open tray, (m2SKYPA1) is screwed on the implants and the impression is taken according to the standard procedure for the open tray technique.

In order to produce the master model, the mini²SKY implant ana-log (m2SKYXIA) is screwed to the mini²SKY impression abutment and the model is manufactured from super-hard stone.

The locator impression coping is used to make an impression of the mini²SKY Locator®.

The miniSKY Locator® abutments are fixed to the implants using the miniSKY retaining screw. The prosthesis is then manufactured using conventional dental procedures. The Locator® retention elements up to 20° are used for retention.

Narrow jaws represent a challenge when fixing removable res-torations in older, edentulous patients. These patients frequently refuse augmentative measures, or their general state of health requires a procedure that does not involve major surgery. The use of miniSKY Locator® allows minimally invasive implant place-ment and healing without loading and insertion after osseointe-gration using the proven Locator®.

In combination with the blueSKY placed at an angle, SKY classic posterior implants, and the SKY Locator® angled, it is possible to fabricate implant-supported restorations which offer superior comfort to patients.

The mini²SKY Locator® is available in heights of 2 mm and 4 mm and provides special protection against rotation for simple place-ment of the mini²SKY Locator® on the external precision torx of the mini²SKY implant to avoid screw loosening.

The appropriate retaining screw is used for fixation. This ensures that the load is distributed via the precision torx in the implant and the screw is protected from overloading. The original Loca-tor® retention inserts 20° can be used for this procedure.

Reliable fixation of the scan and drilling templates in situ is a key prerequisite for precise transfer of 3D implant planning from the PC to the mouth of the patient.

Bone-supported drilling templates are expensive and cause major trauma owing to extensive flap procedures.

Everything you need

ImplantsThe mini1SKY implants of lengths 6 mm and 10 mm can be in-serted firmly in any bone.

SurgeryThe implants are inserted using the insertion instrument after the cavity has been pre-drilled using the twist drill from the SKY OP tray.

Impression and model fabricationWhen making an impression, only the o-ring (SKY-OR55) is at-tached. The implant analog is pushed into the impression and the model is manufactured.

Scan and drilling templateThe planning matrix fixes the scan and drilling templates se-curely and precisely.

Temporary restorationDuring the planning phase, the prosthesis in the lower jaw may be further stabilized by using either the SKY precious metal ma-trix or the SKY o-ring. In the upper jaw, the prosthesis should only be ground out and equipped with a soft relining.
