Dear Colleague,

Welcome to our first ever GP Service Guide, which we have produced to help you refer patients into our mental health services across Walsall.

You may be aware that we have recently made changes to our Adult Community mental health services. This guide contains everything you need to know, including up-to-date information about our services, details on referral criteria and key contacts.

Please keep this guide handy. It has been designed so that, as information about our services changes or is updated, we will send you a new page that can be easily inserted into the ring binder.

Our Vision

Dudley and Walsall Mental Health Partnership NHS Trust was established in October 2008 and specialises in the treatment of both common and complex mental health conditions in children, adults and older people. It provides a comprehensive range of mental health and social care services including primary care mental health, community based services and inpatient facilities. In 2009 the Trust was selected by the National Commissioning Group to be one of four centres in England to deliver a national Deaf service for children.

Serving a population of approximately 306,600 in Dudley and 255,900 in Walsall, services are provided from a number of sites across the boroughs. Most are owned or leased by the Trust; some services are provided from within GP practices, health and social care centres and other community venues. The Trust also provides services directly to people in their homes or in residential and care homes.

In the past four years the Trust has established itself as a provider of high quality, safe and effective mental health services for the populations of Dudley and Walsall. This is evidenced by unconditional Care Quality Commission (CQC) registration, NHS Litigation Authority (NHSLA) compliance, Accreditation for Inpatient Mental health Services (AIMS) and successful performance in a variety of regional quality, safety and governance assessments and evaluations.

The Trust’s vision is one of providing recovery oriented services. This vision has been guided by a growing emphasis on the wellbeing of the population and a focus on prevention and early intervention. It guides our future plans for mental health services.

What is the Primary Mental Health and Talking Therapies Service?

The Primary Mental Health and Talking Therapies Service offers access to primary care based mental health screening, assessment and interventions.

The team provide a flexible and innovative service that meets the various needs of adults aged 17 years and upwards presenting with mild to moderate mental health problems associated with low risks and care clustered within the range 1 to 4.

Who can refer to this service?

The service can be accessed via a GP referral or self referral, however in cases of self referral, consent will be gained to discuss the case with their GP.

How to make a referral

Please issue the service user with a patient information leaflet which provides information about the service and how to make an appointment.

What is the Early Access Service?

The Early Access Service (EAS) delivers a single point of entry into secondary mental health services for all adults (17 to 65) and provides mental health screening and comprehensive mental health assessments.

Who can refer to this service?

The service accepts referrals for patients presenting with a moderate to severe mental illness made by:

GPs Mental Health Act Mental Health Primary Care Teams Patients Transferring from CAMHs New transfers from other mental health trusts

The Early Access Service is commissioned to accept referrals for patients presenting with a moderate to severe mental illness. Patients who, following assessment, do not meet the criteria for secondary mental health services will either be signposted to other services or referred back to the referrer.

How to make a referral

To make a referral please write to the address below detailing the presenting mental health problems and a short medical history of the patient.

The service will aim to offer the patient a routine appointment within 15 working days from receipt of the referral.

Urgent Referrals

If the referral is urgent and a same day response is required, please ring the service number (01922 608850) and discuss the case with the Duty Worker who will support the appropriate response process. (Please note posted referrals will not be classed as urgent).

Please note: In exceptional circumstances referrals may be accepted from outside of the above list. In such circumstances EAS will contact the GP and seek authorisation to proceed.

The service will aim to acknowledge receipt of all referrals and advise the referrer of the appointment date offered to the patient. The service will write to the service user’s GP following assessment detailing outcomes and any initial plan of care.

Community Mental Health Team for Older Adults

The Community Mental Health Team for Older Adults CMHTOA provides a single point of access into Secondary Mental Health Services for Adults over the age of 65.

The team provides a multi-agency and multi-disciplinary service specialising in the assessment and treatment of mental health needs associated with older adults. The service is also available to younger people with dementia.

Who can refer to this service?

The CMHTOA Duty Desk provides a gateway function to secondary mental health services for Older Adults. The service can be accessed via referrals from GPs.

How to make a referral

Please complete a service referral form and send this via fax to: 01922 607220 or post this to Duty Desk, Community Mental Health Team for Older Adults, Blakenall Village Centre, Thames Road, Walsall, WS3 1LZ.

Please note that the Referral Form includes guidance as to what information is required.

Once the team has received the referral form they will contact theservice user or carer to arrange an assessment appointment.

Referrals out of hours

Please contact Dorothy Pattison Hospital Switchboard on 01922 607000 and ask for the Crisis Resolution/Home Treatment Team.

Walsall Memory Assessment Service

The Memory Service offers assessment, diagnosis and treatment to persons over 50 years of age for whom memory problems are the primary symptom. In addition, the Service offers pre and post diagnostic counselling, advice, education, guidance and signposting to appropriate services.

Who can refer to this service?

The Service can be accessed by Referral Form from a GP, Community Mental Health Teams and Walsall Manor Hospital. The Referral Form includes guidance as to what information is required.

GPs please note that patients are referred to the Memory Service under Shared Care Protocol.

How to make a referral

To refer a patient to this service please complete a referral form and fax to: 01922 607220 or post this to Memory Assessment Service, 1st Floor, Blakenall Village Centre, Thames Road, Walsall, WS3 1LZ.

Following receipt of a referral the referral will be screened and the patient or carer will be contacted to arrange an assessment appointment.

The referrer will be informed in writing about the outcome of the assessment, the diagnosis and details of the treatment and management plans.

What is the Child and Adolescent Mental Health Service (CAMHs)?

The service provides a multi-agency and multi-disciplinary service specialising in the assessment and treatment of moderate to severe mental health difficulties that children and young people up to their 17th Birthday.

Our team of child and adolescent mental health specialists cover a wide range of skills and include Child and Adolescent Psychiatrists, Child & Adolescent Psychotherapists, Clinical Psychologists, Nurses, Specialist Social Workers, Family Therapists and Speech & Language Therapists.

If you are unsure whether Child and Adolescent Mental Health Services are appropriate please do not hesitate to ring the service number (01922 608777) for further guidance.

Who can refer to this service?

The service accepts referrals for patients presenting with a moderate to severe mental illness made by: GPs Paediatricians Social Workers School Health and Youth Offending services It is important that front line services, such as counselling services, mediation services, family centres and bereavement services are tried first before a referral is made to the CAMH Service.

How to make a referral

Please make a written referral to the address below detailing the presenting mental health problems and details on any previous interventions including the outcome.

Lantern House Community Detox Team

Lantern House is a community drug and alcohol treatment service (Adults 18 +) assisting individuals and families in their recovery from chemical addictions. There are three elements to the service:

• StructuredDrugTreatmentTeam-The Structured Drug Treatment Team provides an access point into drug treatment by undertaking comprehensive drug treatment assessments and individual packages of care. • CommunityAlcoholTeam-The Community Alcohol Team provides medically supervised community alcohol detoxification by undertaking comprehensive alcohol treatment assessments and individual packages of care.

• SharedCareTeam-The Shared Care Team provides ongoing support for those who have achieved a level of stability in their lifestyle but who may require ongoing replacement prescribing programmes. Medical prescribing is undertaken by GPs and a key worker continues to monitor and provide motivational interventions.

Who can refer to these services?

• StructuredDrugTreatment- Referrals are accepted from GPs, professionals, self and the criminal justice sector for structured drug treatment programmes.

• CommunityAlcoholTeam-Referrals are accepted from GPs and self referrals for community alcohol detoxification.

• SharedCare-Referrals are accepted from GPs.

How to make a referral

This service operates out of hours during the evenings, weekends and bank holidays in both the Dudley and Walsall localities.

If a person experiences a Mental Health crisis out of hours and the GP believes a mental health assessment is required then initially the Crisis Team should be contacted by telephone 01922 607000 and the crisis worker will advise on the way forward.

The same process is to be followed if a Mental Health Act Assessment is required. Again, the crisis worker will advise on appropriate assessments, treatment and access to services if appropriate.

The Service Experience Desk (SED) is the central point of contact for all concerns and enquiries from service users, families, carers and all healthcare professionals whether these are formal or informal, compliments, suggestions or complaints.

Compliments, concerns and complaints can be made directly to staff or to the Service Experience Desk.

The Service Experience Desk will liaise with staff managers and health professionals to handle concerns and sort problems as quickly as possible. The team also provides information on NHS Services and advice and support to service users, their families and carers.
