Dependent Coverage If you wish to add a dependent, you must do so during open enrollment.

Mid-year additions require a qualifying event. You may drop a dependent at any time.

Health Assessment for HRA

To receive HRA dollars in 2019 you and your PreMedicare spouse must have completed the health assessment by September 30, 2018.

PreMedicare retirees and their PreMedicare spouses will be required to complete a Health Assessment by September 30, 2019 with their insurance carrier to receive their portion of HRA dollars in 2020.



There is a premium increase to the Dental Plan for 2019.

Medical Coverage

To continue coverage as a surviving spouse and/or dependent, you must elect to continue coverage through Via Benefits.

The cost to continue coverage is 50% of the full medical premium for the survivor of a retiree or employee with 15 or more years of service.

Continued coverage is available until the surviving spouse remarries, dependent children become ineligible and/or coverage is terminated with Sandia.

Dental Coverage

Dental coverage for surviving spouse and/or eligible dependents is discontinued at the end of the month of retiree’s death.

Group coverage may be temporarily continued (COBRA), for up to 36 months by paying the full monthly premium COBRA surviving spouse rate $46 plus the 2% administrative fee.

Other long-term dental plan coverage information for surviving spouse is available through Via Benefits Benefit Advisor.

Surviving Spouses lose dental coverage under Sandia at the end of the month of the retiree’s death. A surviving spouse can continue coverage for up to 36 months by paying the monthly premiums for COBRA coverage. UHC Business Services will send paperwork to the survivor for continuing under COBRA coverage. COBRA – Consolidated Omnibus Budget Reconciliation Act of 1986 requires most employer groups sponsoring health plans to continue temporary health coverage in certain instances where coverage would otherwise end. Coverage may be elected through Via Benefits


Check to ensure that your child meets the eligibility requirements to be enrolled.

Children are covered until age 26.

If your child is no longer eligible, please be sure to contact Via Benefits to disenroll.

You must disenroll dependents within 31 days of qualifying event.

3- months before reaching age 65…

Enroll in Medicare Parts A and B for an effective date the beginning of the month in which you become Medicare-eligible.

Contact Medicare or your local Social Security office for more information.

Approximately 3 months before age 65, you should receive information from

Via Benefits. Call Via Benefits to review health care options (888) 598-7809.

What Do I Do When I Turn 65?

If you or your covered dependent becomes Medicare-eligible due to disability, you or your covered dependent will need to enroll in Medicare Parts A and B.

OK for those of you that are not yet age 65…. You need to enroll in Medicare Parts A and B. EH will send you materials 3-4 months before you reach age 65. Medicare should also be sending you information. You’ll have the current medical plan options that are available to Medicare retirees for 2018 Contact Medicare or your local Social Security office for Medicare enrollment information.


Confirmation/Billing Statements

You will NOT need to make any changes to your automatic debit for premium payment.
