Developing a Questionnaire

• No hard and fast rules• Only guidelines

• Avoid Complexity: use simple, conversational language

• Avoid leading and loaded questions

• Avoid ambiguity: be as specific as possible

• Avoid double-barreled items

• Avoid making assumptions

• Avoid burdensome questions
