Terms and ConditionsThis mail-in rebate offer is valid on any set of Philips VisionPlus, CrystalVision Ultra, or X-treme Power headlight bulbs between August 26,2012 and October 27, 2012, while supplies last, at participating retailers in the United States displaying the original Philips Upgrade Headlight Bulbs Rebate in-store promotional materials. Open to individual residents of the United States only (not available to businesses or organizations). All Claims must be postmarked by November 11, 2012 and received no later than December 15, 2012. To submit a Claim, send via United States Postal Service or courier the original sales receipt showing proof of purchase of Philips VisionPlus, CrystalVision Ultra, or X-treme Power headlight bulbs from a participating retailer in the United States of America, and the original UPC found on the product package, and this original mail-in rebate coupon with your name and address below (collectively, “Claim”) to address listed below. Rebates of $5.00 are given on qualified purchases of a set (pair) of VisionPlus. Rebates of $8.00 are given on qualified purchases of a set (pair) of CrystalVision Ultra. Rebates of $15.00 are given on qualified purchases of a set (pair) of X-treme Power. Any reproductions, mechanical or otherwise, of the rebate coupon will be disqualified.

Late, lost, incomplete, postage-due, illegible or misdirected Claims or Claims that are irregular, have been submitted through illicit means or do not conform with these Terms and Conditions will not be accepted. Fax or email will not be accepted. All Claims become property of Philips and will not be returned. Rebate cannot be combined or used in conjunction with any other offers. Offer is invalid where prohibited. This offer is subject to all applicable federal and state laws and regulations. Offer is limited to two rebate claims per household. Rebate Payment: If Claim is complete and otherwise complies with these Terms and Conditions a rebate in the form of a check will be mailed to the mailing address provided. Please allow 6-8 weeks from receipt of Claim by Philips to receive your check. The customer is required to cash any rebate check within sixty (60) days of the check issuance date. This is an essential eligibility requirement of this offer and Philips has no rebate obligation to the customer if he or she is unable, unwilling, or otherwise does not perform and fulfill the requirement.
