Lower (Standard)Tightens the lower face’s un-derlying structures that slacken with age. Also trims excess skin, smoothing it to eliminate facial creases, and giving a visual lift to the bottom third of the face.

Mid-FaceliftAs people age, the cheeks and eyelids tend to sag. This proce-dure restores a youthful, rested look to the lower eyelids and cheeks.

The Mini “Weekend” Performed quickly with a shorter recovery period, the procedure enhances key facial areas. How-ever, the “weekend facelift” is a minor surgical procedure. Results are much less impressive than a standard facelift.

Thread FaceliftA less invasive approach that has come under criticism for not producing the results as promoted. I do not offer this technique because of the questionable level of improvement.

TYPES OF FACELIFTS.

The effects of gravity start to take their toll between the ages of 40 and 50, so the skin and soft tissues of the face start to lose their elasticity. This all results in bags developing under the eyes, and a sag-ging from the cheekbone, which causes a deepening of the folds from the nose to the corner of the mouth. The jawline becomes less clean and bands begin to develop in the neck. Most patients requesting evaluation for a facelift feel that their facial features do not reflect their youthful spirit and energy level. That’s when they turn to me for a rejuvenation plan. Facelift techniques (rhytidectomy) have evolved significantly over the last several years. Early facelift operations tightened only the skin. Today, the facelift addresses

facial muscle laxity and removes excess skin and fat found in the lower face and neck. The procedure can enhance the con-tour of your jaw and soften deep lines. There are so many different procedures, it can be confusing for a patient. Just the word “facelift” can be intimidating because it implies that the whole face is lifted. In the majority of facial cosmetic procedures that I have performed, the most com-mon areas treated are the cheek and neck only. The main objective has always been to produce a natural result and to make the patient look like a younger version of themselves, with an avoidance of the dreaded “pulled” look.

The Procedure: 85% of the facial cos-metic procedures I perform are done in our office operating suite, on an outpatient basis. The procedure is done under heavy sedation or a general anesthetic. We find the office procedure to be more beneficial than the hospital experience because of the higher level of confidentiality and also the cost savings.

Facelift Recovery and Results: Most patients rate the pain as 3 to 4, on a scale of 1 to 10. Bruising usually resolves in 10 to 12 days. There is one small, well-concealed scar in front of the ear and the rest are hid-den in the hair line. They fade significantly within a 3 to 4 month period.

While we can often disguise protruding ears with different hairstyles and soften the appearance of our eyes with makeup, the nose is the most noticible facial feature and is more difficult to hide or disguise. In the 500+ Rhinoplasty procedures that I have performed, I have employed a highly focused individualized approach which avoids the dreaded “operated on” or “done” appearance.

The Top 5 Nose Complaints.Sound familiar?

FUNCTIONAL nasal problems usually arise from an injury and result in a disfigurement of the nasal shape with com-plaints of obstructed nasal breathing. Most functional Rhinoplasty candidates have sustained an injury to the nose that has resulted in a possible nasal fracture and/or displacement of the internal partition of the nose, which separates one nostril from the other. The most common concerns - difficultly breathing through the nose and severe headaches. The nose may appear disfigured, often identified by a hump at the midportion of the bridge. An internal examination may

confirm that there is, in fact, a deviation of the nasal partition resulting in a partial blockage of nasal airflow. We then would develop an operative plan to restore the displaced nasal structures to their original position, and to rearrange the displaced nasal partition in an effort to restore normal nasal airflow. The reconstruction of a functional nasal problem is usually covered by one’s insurance.

COSMETIC nasal problems include the above complaints of “my nose is too large”, “I hate my hump” or “the tip is too large”. These are characterized as cosmetic

because their improvements do not affect the function of the nose and the ultimate goal is to produce a better appearance of the nose. I refer to this type as an “aesthetic” Rhinoplasty. It is important to understand that the shape of the nose is dictated by the bone and cartilage framework beneath the skin. It is therefore the goal of the cos-metic nasal surgeon to reshape the frame-work beneath the skin to reduce the size and shape of the nose, without producing a result that looks “done”. In the case of an aesthetic Rhinoplasty, I feel the most important aspect of the initial consultation is to determine whether the

results of the procedure can realistically meet the patient’s expectations. Difficulties can arise when a patient states, “I want my nose to look exactly like this,” and presents a photo of a celebrity. This type of result is often not realis-tic, firstly, due to other facial features contributing to the nasal appearance, and secondly, because the nasal frame-work may not allow a specific result. However, certain characteristics of someone else’s nose may be incorporated into each individualized Rhinoplasty. A detailed examina-tion of the candidate is performed and an operative plan to address the patient’s concerns is formulated. Because this is a purely cosmetic procedure, it is not covered by insurance.

The Nasal (Rhinoplasty) Procedure: Both functional and cosmetic procedures are done under a general anesthetic and on an outpatient basis. Each procedure takes about two hours to perform. Most patients rate the pain on a scale of 1 to 10 as a 4 to 6. There is a dressing in place for 6 to 7 days, and upon its removal the patient’s result is noted with a moderate amount of swell-ing. Patients typically feel comfortable being in public after Rhinoplasty in seven to eight days.

1. Laser Skin Lift, Skin Tightening or Laser Resurfacing: This approach is well accepted with patients that do not have significant folds and sagging of tissues, or desire not to undergo a surgical procedure.

5 FACELIFT FACTORS TO CONSIDER 1. Sunken Eyes. Gravity begins to pull the fat under the eyes down, which results in a sunken appearance. Typically a blepharoplasty is addressed at the time of the facelift.

2. Loss of Volume. A loss of fat in the face results in droopiness. Volume can be restored through injectibles, fillers, fat, or a combination during or after your facelift.

3. Wrinkly Skin. Various factors over time can cause the skin to age and wrinkle. Think sun exposure, pollution and free-radical damage.

4. Deep Folds in Skin. This is due to the bone density changing over time. Excess skin is removed and remaining skin is pulled back.

5. A Saggy Jawline. As time goes on, bones become less dense and gravity begins to wear on the skin. Eventually, only surgery can correct and tighten the jawline.

AT HOME• Heel raises are perfect while washing dishes, blowdrying your hair, folding laundry or talking on the phone!

• Try squats, alternating lunges, jumping jacks, or push-ups while waiting for food to heat up in the microwave. See how many reps you can accomplish of any of the above listed exercises in the 60 seconds (or whatever time) it takes to heat your food. These can also be per-formed while waiting for the coffee to brew!

AT THE OFFICEBy getting up a few minutes more often, you’re less likely to have lower back issues, tight hamstrings (which is a cause of lower back issues), tightness of the shoulders and neck and, hopefully, less yawning!

• Switch your office chair for an exercise ball. Merely sitting on the ball is going to activate your core muscles to balance on the ball.

• If you have an office or a cubicle, set your watch or timer on your phone to go off every 30 to 45 minutes so that you can get up and move! Great exercises would be squats, push-ups, lunges or crunches on the ball.

• Keep a couple of dumbbells under your desk so that you can perform seated (on the ball) shoulder presses, arm curls, arm frontal and lateral raises, etc.

No Time? No Excuse! You can tone muscles any-where! When meeting with clients during the initial meeting, one of the top five excuses I hear is, “I don’t have enough time!” If you ask any one of my small group training clients they will tell you that some of their toughest, yet most worthwhile, workouts can be done in less than five minutes! Even if you can’t get in an intense five minute workout, there are several movements that you can perform while doing everyday tasks, which will help to “tone” your muscles and burn a few extra calories!

IN THE CARStopped at a stoplight or stuck in traffic? Challenge yourself!

• Chest squeeze. Place left palm on steering wheel at 9 o’clock position and your right palm at the 3 o’clock position. Press your hands into the steer-ing wheel and hold for 10-15 seconds. Keep breathing. You are working the muscles of your chest, shoulders and arms.

• Reverse squeeze. Place hands at same position, but inside of the steering wheel instead. Press hands or fists out and again hold 10-15 seconds.

• Glute squeezes. Yes, I said it! Contract your glutes and hold for 10-15 seconds.

• Some of my clients even have a couple of 5lb dumbbells in their car so they can do arm curls while waiting at the lights. Plus, if you end up at a park for a walk or run, you can incorporate some resistance exercises too!
