Antibacterial Data Testing

Test measurement of the effective removal of particles with SpiroTectTM antibacterial treated HME filter. DIN EN 143 test method using NaCl salt particle aerosol.

Test to establish the efficacy of SpiroTectTM HME filter, test aerosol particles were 99% under 1µm (1 micron) in size and 70% under 0.1µm in size.

The majority of pathogens and viruses are approximately sized between 0.01µm and 0.3µm so the filter removed the majority of these sized particulates by retaining them within the filter attached to the antibacterial coating.

During the test, the NaCl salt particulates reaching the atomic spectroscope are measured to determine the SpiroTectTM HME antibacterial bacteria and viral pathogen removal efficiency.

Results conclusively show over 70% very high average efficiency of the SpiroTectTM antibacterial filter in helping to protect patients from potential infection by removing particulates from their incoming breath.

Benefits of using Heat and Moisture Exchangers (HME)

Introduction

Laryngectomees are statistically more prone to lower respiratory tract infections, including common colds, flu, bronchitis, pleurisy and pneumonia. Pneumonia has been found to be the most frequent medical complication after total laryngectomy. Lower respiratory tract infections including pneumonia represent a significant burden of illness for both the patient and the NHS.

How HMEs work

HMEs are designed to address the loss of pulmonary function following a total laryngectomy and have been in clinical use for over 30 years. Consistent daily use of an HME has been shown to significantly reduce the severity of post-laryngectomy pulmonary disorders.

The natural humidification and filtration process (through the nose andmouth) is bypassed in laryngectomees and the air that enters the stoma is cool, dry and unfiltered which can irritate the lungs. Therefore, artificial airway humidification and filtration is required to help improve lung function by increasing heat and moisture levels and in turn filter particulates as the laryngectomee breathes (refer to the diagram below).

For an HME to achieve its optimal performance it is necessary for it to be as airtight against the tracheostoma as possible. Products such as tubes, baseplates, plasters or buttons can be used to help with this. This will ensure that no air will pass the HME or cause for a loss of warmth and moisture.

• Moisture retention - A person who does not have a laryngectomy loses approximately 250ml from their lungs each day whilst breathing normal. The effect of moisture loss in a laryngectomee who does not use an HME is approximately 500ml a day. Daily use of an HME can reduce moisture loss to approximately 250-300ml per day. This will result in less coughing, dryness and mucus production. Clinical studies also argue that it can improve shortness of breath, fatigue, sleep problems and depression.

• Less pulmonary complaints - Use of an HME increases temperature and humidity. Without artificial airway humidification, the air entering the lower respiratory tract will lack sufficient heat and moisture, leading to increased pulmonary complaints such as coughing or mucus production.

• Better quality of air - It is essential for a laryngectomee to use a HME to maintain healthy respiratory tract and to breathe in air that is filtered from pollution.

• Improved quality of life - Regular use of HMEs can give a laryngectomee the confidence to lead a normal life. HMEs appear to have a positive influence on voice quality and have shown to help restore lost airway resistance. It has also been argued that it may play a role in improving normal blood oxygen levels. Constant HME users tend to make less use of external humidifiers and vaporisers, have better pulmonary status and lower health-care costs.

The NHS Reference Costs for 2013/14 indicate that treatment for a single episode of an acute lower respiratory tract infection can cost in excess of £7,000.The SpiroTectTM antibacterial HME System is available on NHS prescription and costs from £5.27 per day* (based on the daily use of a SpiroTectTM HME antibacterial cassette and SpiroTectTM baseplate), at a cost to the NHS perannum of £1923.55.Equivalent leading brand’s HME system available on prescription costs from £7.89 per day*, at a cost to NHS per annum of £2879.85.*Based on August 2015 Drug Tariff prices

All SpiroTectTM HME Cassettes contain antibacterial coated foam which, literature shows, can help to reduce infection and filter the inhaled air. Furthermore, the cassettes have high porous foam which increases airflow and reduces breathing resistance, helping to restore some of the normal respiratory functions. All cassettes have a fully tinted design, are small, discreet, easy to use and are interchangeable with other devices on the market. SpiroTectTM cassettes offer excellent filtration, moistening and warming of air to protect the trachea during day and night.

All SpiroTect™ Adhesive baseplates are hypoallergenic and have been developed to secure HME Cassettes and Valves at the Laryngectomy stoma site. Designed to mould to body contours, these baseplates are available in both round and oval shapes. The baseplates surface area has been designed to suit different preferences and stoma sizes, with the addition of side tabs to allow for quick, hassle free removal. The SpiroTectTM Laryngectomy Tube is inserted via the stoma into the trachea and secured around the neck with a tube holder or a baseplate with tube connectors.
