Planning
• Planning is the phase of the DENTAL HYGIENE PROCESS in which priorities are set, client goals are established and interventions & outcome measures are determined

Planning CARE PLAN
• Its purpose is to develop the plan of care that will target the resolution of an oral health problem amenable to dental hygiene care, the prevention of a problem, or the promotion of oral health

CARE PLAN
• Therefore, the phrase CARE PLANCARE PLAN, rather than TREATMENT PLAN, is consciously used to denote the broad range of preventive, educational, therapeutic, & support services within the scope of dental hygiene practice

CARE PLAN
• Given the human need for responsibility for oral health, the dental hygienist actively involves clients, according to their ability & motivation, in the process & obtains the informed consent of the client (or the client’s guardian) before initiation of care
• The tangible outcome of the planning phase is the dental hygiene care plan

To formulate a care plan, the dental hygienist must:
• Possess a sound background in dental hygiene theory
• Be thoroughly familiar with the parameters or standards of dental hygiene care
• Possess the ability to collect, analyze, & interpret client data
• Develop dental hygiene diagnoses• Formulate client goals

Dental Hygiene Care Plan• Is a written BLUEPRINT that directs the
dental hygienist & client as they work together to meet the client’s oral health goals
• The plan increases the likelihood that the oral healthcare team will work COLLABORATIVELY to deliver client-focused, goal-oriented, individualized care to clients
• The plan facilitates the monitoring of client progress, ensures continuity of care, serves as a vehicle for communication among healthcare professionals & increases the likelihood of quality care

• Once the CARE PLAN is written & shared with the client, the CARE PLAN becomes a legal contact between the dental hygienist and the client
• Once the diagnosis is made, client-centered goals are established by the dental hygienist in conjunction with the client

• From the established client goals, interventions & expected outcomes can be drafted
• With client input, priorities for each goal are established and then appointment planning can begin

• Evaluation allows the clinician’s to measure the short-term achievements of client-centered goals as well as to anticipate the client’s long-term prognosis in maintaining the goals achieved
• Evaluation includes the monitoring or reassessing the client’s self-care, changes in self-care behaviours, and/or parameters of oral health & disease at continued care
