RESULTS Crosscutting projects in primary/tertiary healthcare and public health some of which are indicated in the schematic. Data analyses and reports on population health trends in Jacksonville, Florida. Leadership roles in Community initiatives. Informatics capacity assessment at local health department. Clinical informatics project management and workforce development. National Informatics policy analyses and presentations for community mobilization. CONCLUSIONS The collaborative model of an Academic Health Department maximizes cross cutting opportunities like the APHIF for Public health Informatics infrastructural development Delivery of core public Health functions and essential services Providing opportunities for a true integration of Public Health and Primary Care Courtesy: CDC/CSTE/ASTHO APHIF
