    ForwardThe intent of this project is to design a Polytrauma Rehabilitation Center (PRC) for returning veterans in higher education on the campus of Carle Hospital in Urbana, Illinois. The PRC will become the entry point for wounded veterans into the proposed University of Illinois Continuum of Care for Wounded Veterans in Higher Education. This system, the first of its kind, would provide continuous care for wounded veterans who will transition from the PRC to the new UIUC Center for Wound Veterans in Higher Education and eventually into independent living, matriculation, and graduation.

    Professor William Worn,was the Director: Master of Architecture in Health Design at the University of Illinois at Chicago. In 2010, he joined the University of Illinois at Urbana-Champaign as an Associate Professor, while simultaneously continuing his role as President of Worn Jerabek Architects P.C. in Chicago.

    The twin cites of Urbana-Champaign are located in central Illinois, 140 miles south of Chicago. They serve a population of over 230,000 and are home to the University of Illinois at Urbana-Champaign. In addition to the university, Urbana-Champaign is host to the Carle Foundation Hospital, a Level I Trauma Center. The cities unique relationship with the university and Carle Hospital make them a perfect location to advance the study of Traumatic Brain Injury (TBI).

    Carle FoUndation hospitalThe 345-bed regional care hospital has achieved Magnet designation, the nations highest honor for nursing care and was recently named to the 100 Top Hospitals by Thomson Reuters. It offers a more advanced level of clinical expertise and technology than any area hospital, housing the areas only Level I Trauma Center as well as Level III perinatal services. The Hospital admitted more than 22,000 patients and treated more than 70,500 patients in the emergency room during 2012. The Mills Breast Cancer Institute, Carle Cancer Center, Carle Spine Institute and Carle Heart and Vascular Institute bring additional advanced diagnostic and treatment options(via www.carle.org).

    A new nine story bed tower (lower left) is currently under construction and marks the end of the Campus Modernization Project at Carles main campus in Urbana. The new tower will house 136 new private rooms and provide expanded services to cardiovascular, neurological, and intensive care patients. It is also the new home of the Carle Heart and Vascular Institute.

    Due to the completion of the Campus Modernization Project, Carle is exploring new opportunities for future planning. Some of the future areas of interest include expanding neurological research and services and consolidating resources among campuses.

    Located near Lincoln and Nevada in Urbana, the Center for Wounded Veterans in Higher Education will provide comprehensive, integrated educational services to veterans with severe and multiple disabilities as they pursue degrees at the University of Illinois at Urbana-Champaign. The Centers architectural design will provide transitional residential accommodations for 12 to 14 veterans at a time, and non-residential services for up to 150 veterans and their families (via www.illinois.edu).

    The addition of a PRC to the Center for Wounded Veterans in Higher Education would allow Urbana-Champaign to offer the first full continuum of care for veterans with TBI and polytrauma. Veterans would have the proper rehabilitation and therapy facilities to move from injury to independent living.

    Roger S. Ulrich and a number of other experts in Evidenced Based Design have complied a study through the Georgia Tech College of Architecture and the Center for Health Design to analyze how the hospital environment can be improved. It is important to look at the way healthcare spaces are designed in order to create a better environment for everyone that experiences these spaces. Some key strategies derived for Evidence Based Design are helping to improve patient safety, patient outcomes, and staff outcomes. The research shows that some of the most successful strategies include single-bed rooms, reduction of noise, visual and physical access to nature, clear wayfinding, and improved ventilation systems. These strategies not only benefit patients, but also promote a better work environment for nurses and physicians which leads to a reduction in medical errors. The following diagrams map the important relationships and design strategies associated with Evidence Based Design.

    In designing the five existing PRCs, the VA developed a design guide that includes program recommendations, example patient rooms, and technical considerations. Based on recommendations from the VA, it was determined that either a 12 or 18 bed PRC would be the optimum size. These numbers are based on a nurse to patient ratio of 1:6 and the amount of patients being treated at the five existing PRCs.

    Using the VA Program as a base, each team designed their own program based on their own research and observations. As precedent studies, the programs of the National Intrepid Center of Excellence in Bethesda, MD and the Rehabilitation Institute of Chicago were analyzed and drawn from.

    MISSIONTo create a cohesive building which maximizes patient welfare and encourages clinical research and education.

    CLINICALTo create a renovated, interdependent facility with melds the two current Carle clinics to make one large cooperative facility. The coordination of the current Carle clinics with the new PRC would provide a range of treatments with the highest quality of care for the pa-tient.

    RESEARCHThe new facility will offer a variety of opportunities to advance research and improve tech-nologies used to treat TBI patients. The interdisciplinary research will benefit patient care, Carle Foundation, and the University of Illinois.

    EDUCATIONThe goal of the PRC would be to collaborate with the University of Illinois to provide a place for students to practice and research. The patients of the hospital will also learn how to retrain their bodies and can begin on an educational track related to the University. The patient will profit from the interaction between the University and Carle by getting the most recent, ad-vanced treatment possible while reacclimating to society.

    ADMINISTRATIONThe administration pavilion is the most southern part of this arrangement and it is located right next to the pain clinic. It contains offices and conference rooms for the administration of the hospital, but furthermore for the veterans association too. The open courtyard in the middle of the building provides the building with natural lighting in every room and gives the employes a separate place to meet outside.

    LIBRARYThe library gives patients, physicians and visitors the possibility to look up things of interest and also provides the patients with entertaining literature. The research offices on the upper floor can be entered from the clinical balcony. Furthermore the Library pavilion contains a vend-ing area, a coffee shop and also a gift shop. Because of its location right next to the entry it is a very public space.

    VETERANS MEMORIALThe proposal for the memorial is to remind visi-tors of the existence and the importance of the military in the country. This space is de-signed to emphasize the individual solider as well as their military groups. The subtractions VOIDS represent the fallen soldiers, one for each category of military. The addition next to it symbolizes the troops which are still serving. These two elements are connected by a two and a half story black granite wall which is vis-ible from every floor and is accessible from the front of the building.

    The voids of the memorial serve a secondary purpose of creating natural light and venti-lation in the parking garage below grade. The punctures in the earth create an inviting space within the two garage levels and allow visitors to view the memorial from every level.

    AREASThe patient room is divided into three zones: The clinical zone at the entrance, the patient zone in and around the bed and the family space facing the patient at the other end of the room.

    VIEWSThe layout of the room allows the maxi-mum views for both the patient and the caretakers. The patient has the full view out of the window into nature, over to the family space, to the bathroom, to door and the nursing window.

    HEAD WALLThe head wall behind the patient is serv-ing the physicians and the nurses with all the necessary utensils and gases. They are well hidden behind panels which open with a single touch by a magnetic lock.

    THERAPY WINDOW / FRAMEThe therapy window / frame provides the patient with light therapy and differ-ent forms of shading and patterns on the window itself.

    HEALTHCARE BOULEVARDThe central axis of the polytrauma rehabilitation center acts as the col-laborative and active epicenter of the hospital. The axis provides divided pathways for clinical, public, and service circulation. Given the segmented nature of the hospital additions, the centralized street will provoke informal meetings between physicians, staff, and families alike. Different areas within the width of the street are provided for multiple rates of ambulation. There are also areas for lounging, research and retail. A gift shop, library, computer lab, and coffee shop are among the variety of amenities offered along the length of the street.

    Vision Statement: The overall development of a retreat campus thatcapitalizes on the potential for

    long-term patient recovery and research development initiatives.

    Mission Statement: The Carle Midwestern Regional PRC is a specialized medical center

    responding to the heightened aware-ness of victims suffering from multiple traumatic brain injuries. With respect to Carle Federation Hospital's 2011

    After designing the program, our team came up with the allowable perimetersfor our building if it were to become a 5 story, mid-rise rectangle

    The team seperated the inpatient aspects with the outpatient and cameup with the core space

    The overall concept incorporatesa large campus design. In order to maximizeviews for the occupants, the building is setback in a ying/yang principle

    In order to accomidate for the large exteriorpublic spaces, a small program is introduced to not only house bathrooms, bike shelters and cafes, but it also serves as a barrier between the services space to the east and the public campus to the West

    A balcony system stems throughout the buildingsfacade as a way of creating easy-to-reach exteriorspaces for occupants for optimal views to the events occuring on the boardwalk and campus

    Cut-outs and various gardens are introducedto the building for ease of access for inpatients, and also to give private spaces for clinicians, outpatients,students and families

    A MECCO shading system is placed on the southfacade to protect the occupants from the harsh southern exposure from the sun, to promote balconyexploration and interaction

    The building cover continues to wrap aroundto not only provide shading for the roof healinggardens, but also to cover the roof mechanical operations

    A 3 foot slot is designed within each building floor to not only maximize the amount of floor to floor height, but also to create a more quiet and adaptable hospi-tal. The slot runs all the plumbing, HVAC and electrical equipment to keep all floors free of wires and large un-movable objects. Carle Foundation Hospital also wanted to be sure the building was adaptable for fu-ture expansion and renovation, so the change of floor plans are easily done. Views were also an important part of our design, so the slot brings 15 foot height exterior walls for maximum views and optimal day-lighting. Because of the amount of material between floors, there is minimal noise from floor to floor, which is a huge healing coorelation to TBI patients.

    The design of the patient room was inspired through maximizing view points for both nurses and patients. This move was done due to our research of TBI victims. It was observed that patients with TBI had a faster and easier healing process when they were given the ability for larger exterior viewpoints to vegitation and the community. We prioritized the patients inability to see the clinitian over the patients safe transition to the bathroom. However, we designed a ceiling track system that would allow the patients to easily moveu-ver throughout the room. As a veteran, the patient usually has a tough minded opinion of themselves, and so they accept unwinnable challenges (like their physi-cal ability during intensive care). Thus, the track allows them to safely challenge themselves. Due to the urban fabric of the site, each patient room includes in-dividual garden spaces to provide horticultural therapy and views.

    Our proposal of the new polytrauma rehabilitation center chooses the Carle main campus as our site to utilize the existing resources from both Carle and the University. It also provides the opportunity to foster a connection amongst patient, the hospital, the university and the community. We share with Carle the dedication to provide high quality and assessable healthcare. The main objective of our proposal is to create an effe ctive healing environment for the patient and a less stressful working environment for the staff. To achieve that, we develop the idea of the integration of architecture and landscape. Once built it also provides the new bed tower a better natural environment outside of its patient rooms windows.
