Dear Principal, Athletic Director, and Counselor:

I am writing each of you to advise that Orange County Youth Sports Foundation ("OCYSF") is seeking qualified candidates for its Scholar/Athlete Awards for the 49th consecutive year. These $2,500 awards honor college-bound young men and women who will graduate from Orange County high schools in May/June, 2020. Thirteen (13) such scholarships were awarded in 2019. We ask that you select a minimum of one (1) young man and one (1) young woman from your school to submit an application for an OCYSF award.

The criteria for evaluation and selection are:

• Athletic participation, performance, leadership, and attitude • Academic achievement (minimum 3.5 GPA); • Campus leadership and citizenship; Community service; and • Financial need.

The candidates that you select should submit a written profile that addresses each of the criteria referenced above and anything else the applicant considers relevant, along with any available supporting statements from others (such as coaches, teachers, counselors) familiar with the applicant.

The submittal should include the following information about the applicant:

• Student’s name, parent’s name(s), home address, home telephone number, cell number and e-

mail address;

• Name of school, address, school contact and telephone number;

• Sport(s) played;

• Athletic record, including any awards/special recognition received;

• Scholastic honors, including any awards/special recognition received;

• Extra-curricular activities;

• Personality traits;

• Scholarships applied for, siblings in college, other anticipated sources of financial assistance, any part-time employment; and

• College plans -- in order of preference, schools to which applicant has applied and any

responses to date. The submittal should also include name and telephone numbers for applicant’s assigned counselor, applicant’s coach(es), school principal and other references. If all of the requested information is not provided, the application may not be considered. The application materials, along with an original transcript, should be received no later than March 25, 2020. Please mail to:

OCYSF will make its selections in time to present the awards to the recipients at school awards ceremonies in May/ June. You will be notified by mail or phone of our selection(s).
