Introduction

Osteoarthritis is a common problem seen in all part of the world particularly in aged people.

The knee is most frequently involved joint site associated with disability in osteoarthritis in substantial

portion of the population over the age of forty and progress more rapidly in some individuals than in

others. In simple terms, it arises due to wear and tear of the knee joint with increase in age giving

tremendous pain and prevents the person from walking and doing regular activities. To get relief from

this, patient has to take either some pain killers or consult a Doctor. Total knee replacement is one

surgical method to get relief from this however everyone can't afford to go for it and what is the result

after replacement ? They can't sit cross leg, bend leg properly and even can't use Indian toilet which is

very essential for Indian conditions.TM

Protection, stabilization, pain relief & restoration of mobility by:-

1. Correction of the lateral or medial joint space by the 3 point principle even in advanced stage of Osteoarthritis.

1. Firstly let the patient sit on a stool/chair than make a cross over the patella as shown in figure 1.

2. With the help of measuring tape mark 6 inches above from the center of the cross. (Figure-2.)

3. Same as above mark the 6 inches below from that center. (figure-3)

4. Now measure the circumference of both the ends in inches only. (figure-4 & 5)

5. Measurement should not be made in more compressed way or in released manner.

6. If problem is in both leg than take separate measurement of each leg. This caliper is adjustable

up to ± 1 inches.

7. With this also take the age, weight & height of the patient.

1. Wear the “PARAGON™ Polycentric Caliper (Supra)” inside your clothes.

2. Put the bigger part of the “PARAGON™ Polycentric Caliper (Supra)” on front portion of thigh

and place the smaller part at the back of the calf.

3. Wear “PARAGON™ Polycentric Caliper (Supra)” as shown below:

·For varus deformity or when the knees are bent towards outside as shown in figure 'B' then keep

the joint of Caliper towards outside.

·For valgus deformity or when knees are bent towards inside as shown in figure 'C' then keep the

joint of the Caliper towards inside.

4. Joint of the “PARAGON™ Polycentric Caliper (Supra)” should be kept parallel to the centre of

knee.

5. The sequence to tie “PARAGON™ Polycentric Caliper (Supra)”

should be as per shown in the figure : -

6. Tighten the two angulation systems fitted in the “PARAGON™ Polycentric Caliper (Supra)”

with the help of the allenkey (given with the Caliper free of cost ) in anti clockwise direction.

Every week tighten the angulation system slightly.

7. If “PARAGON™ Polycentric Caliper (Supra)” slips down while walking, then tighten the

caliper again.

8. Use “PARAGON™ Polycentric Caliper (Supra)” for 2-3 hours in a day at the beginning and

increase the time gradually as per convenience.

9. For good results follow the exercise chart described on page no. 11.

1. Talcum powder can be used in case of itching problem.

2. Do not use “PARAGON™ Polycentric caliper (Supra)” while Sleeping.

3. If screws of “PARAGON™ Polycentric caliper (Supra)” get loose then tight it with the

allenkey (given with the caliper). Check the screws twice in a week compulsorily.

4. Loose “PARAGON™ Polycentric caliper (Supra)” after every 2-3 hours for 15-20 minutes.

5. Do not use “PARAGON™ Polycentric caliper (Supra)” in long Journey.

It gives initial result in 10 to 12 days and proper results occur in 1 to 1.5 months after proper TMuse of “PARAGON Polycentric Caliper (Supra)” as guided. It is compulsory to use it 4-6 hrs

in a day and it has to be used at least for 1 to 1 ½ year. After that gradually its use can be stop.

TM1. Firstly, wear cotton sock before wearing "PARAGON Polycentric Caliper (Basic)" which is

given free of cost with the caliper.2. Wear the “PARAGON™ Polycentric Caliper (Basic)” inside your clothes.

TM3. "PARAGON Polycentric Caliper (Basic)"should be folded at 45° and also fold your leg at 45°

and keep the padded side of caliper in front side of your leg. TM4. The bigger part of the "PARAGON Polycentric Caliper (Basic)" should be kept on the upper

TMside of knee while the smaller part of "PARAGON Polycentric Caliper (Basic)" on the lower side of knee.

TM5. The Joint of "PARAGON Polycentric Caliper (Basic)" should come parallel to outer side of the knee joint of your leg.

TM6. The "PARAGON Polycentric Caliper (Basic)"should be wrapped tightly on the back side of TMyour leg by strap. Firstly tie the upper end of the "PARAGON Polycentric Caliper (Basic)"

TMthen tie the lower end of the "PARAGON Polycentric Caliper (Basic)".

7. Thereafter, fold the leg at 90° and again tie up tightly. TM8. Stand up and again tight the strap of "PARAGON Polycentric Caliper (Basic)".

TM9. If "PARAGON Polycentric Caliper (Basic)" slips down then again tight the straps of caliper.

TM10. Use "PARAGON Polycentric Caliper (Basic)" for 2-3 hours in a day initially & increase the

Exercises for Patients with knee osteoarthritis

Rest is an important part of the treatment for osteoarthritis, but it must be balanced by regular exercise. Exercise is essential for two reasons: It keeps your joints from becoming stiffer, and it strengthens the muscles surrounding the joints. Strong muscles provide needed support, making movement easier and reducing pain.

To get the most out of these exercises, you need to gradually increase the number of repetitions and sets over the course of 7 weeks.

1. All the exercises is to be done without caliper.2. Exercise should be according to body capacity.3. Repeat every exercise 5-10 times.4. Do every exercise up to 2 to 3 times everyday.5. Rest for a minute between each sets of exercise.* Exercise & effect of exercise may differ from person to person; take appropriate guidance from your doctor / health consultant before starting these exercises. If you are having any problem during the exercise stop the exercise immediately & consult your doctor/health consultant.

Figure 5: Unloading the knee: Put a light weight (0.5-2 Kg) on your ankle and sit in a position that allows the leg to dangle (eg, on a bench, tall stool, edge of bed). Hold for 5 to 10 seconds.

Figure 3: Knee extension: Sit on a chair and rest your foot on another chair across from you so that the knee is slightly raised. Gently push the raised knee toward the floor using only leg muscles. Hold the stretch for 5 to 10 seconds, and then rest for a minute.

Figure 6: Knee flexion While sitting : loop a long towel underneath your foot. Gently pull on the towel with both hands to bend the knee, raising your foot 4 or 5 inches off the floor. Hold for 5 to 10 seconds.

Figure 1 : Quadriceps exercise: Lie on a flat, firm surface with legs extended. Then raise one leg 1 foot off the surface and use it to trace a "T" pattern in the air.

Figure 7: Resisted knee extension: Use a piece of rubber tubing such as Thera Band. Loop the tubing around the bottom of your foot, hold onto the ends while in a sitting position, and bend your knee about 30° (maximum). Straighten your knee against the resistance of the tubing, pushing as hard as is reasonably comfortable. Hold for 5 to 10 seconds.

Figure 2: Knee extension exercise: Sit on the bed with your legs extended, and place a rolled-up towel underneath your Achilles tendon(Just below the ankle). Gently push your raised knee toward the floor. Hold the stretch for 5 to 10 seconds.

Repeat all the exercises with other leg also.

Spondylosis is a condition in which there are degenerative changes in the spine that affects the

Bend knees, lie on back, take a deep breath, places your hands on your thighs and relax.

Tighten your abdomen and buttocks. Press your lower back onto the floor.Action-stretches and strengthens stomach and back muscles.

Turn both knees to one side while rotating your head to the opposite side. Action- stretches lower back, mid back muscles and joints.

Pull both knees to your chest.Action- stretches lower back, buttocks and abdominal muscles.

Slowly raise hips upward, keep a straight line from the knees to the shoulders. Do not arch your back.Action- stretches buttocks and stretches upper leg muscles.

Cross your arms, tuck your chin in, tighten abdomen, and curl halfway up. Use hands behind head for support only. (Do not pull).Action- strengthens abdominal muscles.

(A) Lying

Lie on your back with one leg bent and the foot flat on the floor. Extend the opposite leg straight out. Relax and allow your back to feel the floor.

With knee bent, pull it to your chest, keeping the leg straight, press your knee and lower back to the floor, Action- stretches buttocks muscles, back muscles and hip.

Press your lower back against the floor, raise the straight leg until it is level with the bent knee.Action - strengthens and stretches quadriceps muscles, hamstring muscles and stretches hip joints.

Lie on your stomach; raise one leg off the floor, while keeping the knee straight, Action- strengthens lower back, abdominal and leg muscles, stretches hamstrings and quadriceps.

Keep your neck in a normal position. Push yourself up on your forearms. Keep hips and abdomen against floor. Action-strengthens posterior back muscle attains normal low back curve.

(C) Hands & Knees

While on your hands and knees, keep your knees directly under your hips, your hands under your shoulders. Keep abdominal muscles firm, keep your neck relaxed and in its normal position that is, with your ears in line with your shoulders.

Relax your stomach and buttocks muscles and allow your back to sag.Do not sit back on your hips.Action- to stretch back and abdominal muscles and help maintain lower back curve.

Stretch one arm straight out in front of you while maintaining your back and head position while keeping support arm straight.Action- strengthens and stretches your shoulder, upper back muscles and joints.

Extend your straight leg behind you while holding it parallel to the floor. Maintain your back and neck in normal position.Action- strengthens buttocks, abdomen and leg muscles.
