. Pleasanton/Unified School District * Amador Valley High School and Athlete: School Year 2008-2009 Welcome to the Athletic Department at Amador'Valley High School. Due to current legislation, it is necessary for you to complete the following steps in order to begin practice with your team: 1. READ the Student Handbook for Extra-Curricular Activities-- TURN IN a signed Athletic Agreement & Code of Conduct Form. 2. TURN IN a signed Athletic Insurance Information Card. 3. TURN IN a current Physical Examination Form signed by a doctor. An annual examination, or a statement by a medical doctor certifying that the student is physically fit to participate in athletics every year. Please note that chiropractic exams are no longer acceptable. TURN IN a completed & signed Emergency Card. TURN IN a completed Athletics Eligibility Screening Form. TURN IN a completed & signed NCS Non-Use of Steroid Agreement. Turn In a completed & signed NCS Ejection Policy. Turn In a signed Tri-Valley Orthopedic Certification Form. PAY the transportation fee-varies ($400.00 max/family/year). SHOW the Student Activities Secretary your grade card from the previous grading period which must reflect a 2.00 G.P.A. and no"F's". Grades will be monitored by the Athletic Director and your coach during the season. 11. PURCHASE a Student Body Sticker ($65.00). 12. Be clear of any indebtedness for lost or damaged books. This will be checked in the Student Activities Office. 13. Make sure you and your parents attend a pre-season mandatory parents' meeting. NOTE: Due to contracted services the sport cost will be based on the mode of transportation and number of away games. Fee will be collected as part of your sport clearance procedure. Once the team roster has been submitted to the Athletic Director the only APPROVED refund will be for injury/medical reason. If the player chooses not to participate in the sport there will be NO REFUND.     open starting August 4 from 7:30 a.m. to 3:30 p.m. NO ATHLETEWILL BE ALLOWED TO PRACTICE AND/OR PARTICIPATE IN ANY GAME, MEET,ETC., UNTIL ALL PERTINENT DOCUMENTS HAVE BEEN TURNED IN ~O THESTUDENT ACTIVITIES OFFICE. A CLEARANCE SLIP WILL BE ISSUEDVERIFYING THE COMPLETION OF THE PROCESS. Remember the Parent

    Information Meeting is mandatory and must be attended before theclearance procedure is complete.
