SPECIFIC PAST PERFORMANCE IN HAITI

Following the 01-12-10 earthquake in Haiti, PHS Group, Inc. completed several important projects as part of the reconstruction effort.

1. USAID – Haiti Mission USAID Haiti – C&D Debris Site Management – Port Varreux Award: March 2010 In March 2010, PHS was contracted by USAID to establish the first and only Construction & Demolition (C&D) debris site management and debris reduction operations following EPA and US Army Corps of Engineers standards in Port Varreux, Haiti after the earthquake. PHS provided all management, security, labor and equipment to fully operate and maintain the site and fully operate the processing, sorting and reduction of rubble at the earthquake site. The site received all C&D debris hauled by USAID partners involved in demolition contracts and Cash for Work program. PHS was responsible for Site Design as well as all Site Development / Construction. PHS hired and trained more than 300 laborers with proper safety and protection gear using US standards (100% increase over 150 mandate of the contract). USAID Haiti - C&D Debris Site Management – Truitier Municipal Landfill – Award: June 2010 USAID executed a June 2010 contract with PHS to establish a Construction & Demolition (C&D) debris management site on a secure and controlled 7-acre (approximately 2.8 hectares) section near the entrance to the Truitier waste disposal site, an uncontrolled waste dumping site, particularly at night. PHS mobilized its team within 48 hours, implementing operational standards and safety measures comparable to those at the Port Varreux site. PHS was responsible for Site Design as well as all Site Development / Construction. USAID - Haiti DAI Subcontract – Cap-Haitian – Award: February 2011 In March 2011, PHS was retained to provide to DAI Construction Management services for a series of small building renovations projects started in 2010. PHS is responsible for making corrective action plan recommendation, as well as cost estimating and scheduling services for a series of schools, warehouses and other facilities in need of repairs and renovation.

2. Other US Federal Agencies in Haiti US Department of Defense (DoD) LOGCAP Contract - Emergency Response – Sub to KBR – Brown & Root in 1994-95 in Haiti during the US military intervention PHS (f/k/a C2M) was a subcontractor to Brown & Root under the DoD - LOGCAP Contract. PHS mobilized a team in less than 72 hours, transporting more than 125 longshoremen from Miami, FL to Port-au-Prince, Haiti. Emergency staff loaded and unloaded military ships at the dock in Port-au-Prince, Haiti. Sections of the port were converted to a military base, allowing employees to live on the site. PHS processed medical records, logistics, and provided document control for this operation.

3. Private Sector in Haiti

a. Sogebank Headquarters Building on Delmas – Port-au-Prince Haiti In June 2010, PHS was retained by Sogebank – the largest bank in Haiti – to consult on methodology and design structural repairs details for their headquarter building on Delmas in Port-au-Prince, Haiti which sustained significant damages during the 01-12-10 earthquake. In addition,

PHS provides threshold structural inspection services for the work performed by the GC. b. Demolition / Reconstruction Office in Petion-Ville

PHS is conducting the demolition and renovation of an office/residential structure in Petion-Ville, Haiti. The facilities serve as headquarters of the German NGO: Welthungerhilfe (Agro Action Allemande)

c. Demolition / Reconstruction Warehouses and Residential Units in Petit-Goave

PHS is conducting the demolition and reconstruction of several warehouse facilities and office/residential structures in Petit-Goave, Haiti. All Site Design and Site Development performed by PHS. The facilities are to be occupied by the NGO: Handicap International.

4. Current Clean Energy Projects

Jatropha Biodiesel Study in the Dominican Republic

PHS Group, Inc. is the Executing Agency for an Inter-American Development Bank / SECCI Fund Grant for a Biodiesel production study in the Dominican Republic using Jatropha Curcas as feedstock. PHS retained several leading bio-engineering firms from Europe and the United States to collaborate on the market study and methods of production. The financial model and feasibility study is to be completed by the international consulting firm Deloitte & Touche. PHS is working with focus groups, technicians, and energy experts in five Dominican provinces to determine the perceptions and potential for successful Jatropha-to-biodiesel production in the region.

SOCIAL RESPONSIBILITY IN HAITI As a Haitian-American owned firm with current operations in Haiti, PHS views its corporate responsibility as setting new standards for corporate philanthropy:

Donated Clinic in Port Jeremie in Cite Soleil: PHS using its own funds built a clinic to provide on-site medical care the surrounding neighborhood. The medical facility is currently managed by ―We Advance‖, an American-based NGO devoted to providing medical and economic assistance to Haitian women.

Award of more than 150 full scholarships covering tuition, books, meals, uniforms, transportation to elementary and secondary student near the Truitier area where PHS performed its USAD C&D debris site management contract.

Award of 20 educational scholarships for Local PHS employees to further their educational goals.

PHS sponsored general medical screenings, vaccinations, and health education services.

PHS underwrote the launching of a woman owned small business by donating its cafeteria and kitchen equipment to two female employees starting their own restaurant at the end of the USAID Varreux projects. The women received training on food safety and food handling by a Miami Chef as a consultant to PHS.

PHS partnered with the Banque de L’Union Haitienne to establish savings/checking accounts and pre-paid debit cards to those who lacked a banking relationship and proper documentation destroyed during the earthquake.

Memoranda of Understanding (MOU) with Universities PHS has established a relationship with institutions of higher education to collaborate on the development of knowledge initiatives and enhance student development programs. A Memorandum of Understanding (MOU) has been signed with Bowie State University and Morgan State University, historically Black universities in Maryland, and the Université Quisqueya in Port-au-Prince, Haiti to:

conduct research on technologies and processes in the areas of Clean / Renewable Energy and Health IT.

collaborate in presentation and/or publication of findings on issues of common interest and sponsor professional conferences to disseminate non-propriety information to experts.

provide internships and cooperative education opportunities to students in the areas of product development, engineering, computer security and telecommunications support.

RECENT DISASTER RELIEF and EMERGENCY RESPONSE PROJECTS

USAID IRAQ - Community Stabilization Program (Sub to: IRD 2007-2009) PHS served as subcontractor to International Relief and Development on a USAID Community Stabilization Program (CSP) project in Iraq. The CSP is designed to focus local energies toward productive economic and social opportunities. The initiative will provide the impetus for economic development and create jobs in critical fields. USAID Afghanistan - Power and Water Team (PWT) (Sub to: IRD – 2007) PHS is a subcontractor to International Relief and Development on a USAID project in Kabul, Afghanistan for Human Resources and Logistics Support (HRLS). Supporting the Power and Water Team (PWT) at the Ministry of Energy and Water, the Ministry of Agriculture and the Ministry of Mines and Industry. The PWT is part of the overall program in Afghanistan supporting development and capacity building. USACE - Task Force Guardian, New Orleans (2006) PHS completed a 2006 levee repair project for the USACE New Orleans District under the prime contractor, Stanley Consultants. PHS staff conducted the research for and compilation of test data from daily reports. For this assignment two PHS’ Quality Data Management Engineers received a Certificate of Appreciation signed by Colonel Lewis F. Settliff III, US Army Commander – Task Force Guardian. USACE - Quality Assurance Monitoring, New Orleans (2006) PHS completed a 2006 contract for Quality Assurance and Monitoring services for demolition and debris removal for the USACE in New Orleans. PHS staff was responsible for compliance with all

regulatory and safe execution requirements in debris removal. During this assignment PHS staff received commendations from the USACE Resident Engineer, Mike Bourgeois.

USACE - Task Force HOPE, New Orleans (2006) In September 2006, PHS joined the Joint Venture of Evans-Graves / HDR / PBS&J in working on the Program Management Team - HPO Support Group for Task Force HOPE. Miami International Airport – South Terminal Facility (2003-09) Construction Cost: $ 890 Million PHS was the prime consultant for Threshold-Structural Inspection Services for these 1.6 Million square feet of development and expansion project at Miami International Airport. PHS conducts the threshold inspections for the entire program including south terminal expansion, concourse J, and improvements of concourse H. The inspections are to ensure that the contractor’s structural work (Parsons and Odebrecht JV) complies with the permitted constructions documents. estate investment and development, construction management, and contract administration. Mr. Siclait has been actively involved in some key projects for PHS Engineering Corp. in contract

Dade DERM, and the U.S. Army Corps of Engineers for the Comprehensive Everglades Restoration

Project (CERP) where the firm is part of the Program Management Team for the Jacksonville District. In 1990, Mr. Siclait founded C2M Contractors, Inc., which managed from 1992 through 1995 in excess of $55 million in construction projects. A noted contract was the MCC-3 at Miami International Airport where C2M was part of the construction management team overseeing more than $30 Million of construction projects for Miami-Dade Aviation Department for their capital improvement program. In 1992, following Hurricane Andrew in Miami, Mr. Siclait supervised C2M’s design/built contract for the reconstruction of the Farmers Market packing facilities in Florida City for the company Centex Rooney.

Mr. Siclait was also responsible for C2M’s hurricane debris removal and recycling project for FEMA and the US Army Corps of Engineers. In 1994, Mr. Siclait was the principal in charge for the rapid response project for DoD LOGCAP – Brown & Root logistical and facilities management contract at the port of Port-au-Prince during the military intervention in Haiti. In terms of residential projects, C2M redeveloped the Arthur Mays Village II devastated by Hurricane

Andrew. Other development projects included Pinecrest luxury homes, and the Wildflower

Development of luxury town homes in Coconut Grove. From 1987 to 1990, Mr. Siclait was the vice-president of W.M. Associates, Inc. a real estate

investment limited partnership with holdings in Queens, New York. The firm served as general partner for co-op conversions for large multi-unit residential complexes for both Governor Associates and Bel-

Senior Civil Engineer BRIDGEFARMER & ASSOCIATES, INC., Dallas, Texas 2003-2006 Managed a team of four engineers to perform quality control/quality assurance in support of Design-Build effort for 90 miles of State Highway 130 in Central Texas. Coordinated the design review, comment resolution and certification for 154 bridges of pre-stressed concrete and/or steel plate girder composition. Corp. Dr. Salaheldin’s expertise includes the design of force mains for sewage systems; waterlines

billion that placed the CERP as the world’s largest water restoration plan. The CERP consists of 68 major elements grouped into a 50-project framework designed to restore, protect, and preserve the water resources of central and southern Florida. The project area covers 16 counties over an 18,000-

square-mile area. The CERP will result in sustainable restoration of the ecosystem and will ensure clean and reliable water supplies and improved flood protection. Following tasks are a few of Mr.

Haque’s notable achievements as a project manager:

• Master Implementation of Sequencing Plan (MISP). Developed and maintain MISP that

provides the sequence and schedule of all projects to maximize achievement of CERP’s goals

in the shortest time frame and most cost-effective manner. • Developed 5-year Congressional Report intended to provide a status report on the progress of

the CERP. This report, submitted jointly by the Secretary of the Army and the Secretary of the Interior, summarizes the progress made to date and accomplishments expected over the next five years. The report provided to-date expenditures along with five-year funding forecasts.

• Produced Annual Report to Office of Management and Budget (OMB). Implementation of the

CERP involves a long-term program with a significant annual commitment of resources from

the Federal government, State of Florida, South Florida Water Management District, and local

governments. The report provides estimates of the total cost of the CERP.

• To support the funding for CERP program, 11 schedules had been developed. Maintain those

schedules in P3e and into P2 system. Update monthly to reflect the progress. • Provided consultation to deployment team for System Integration of P3e and P2 using Kapow

Robot.

• Produced the South Florida restoration Program Status Report on a monthly basis. The report

is the snap shot of time for CERP and Central and South Florida project progress. The report

summarized Fiscal Year (FY) expenditures-to-date versus total budgeted costs for the FY

using data from FY05 Office Budget Worksheet and Corps of Engineering Financial

Management System (CEFMS).

Project Control Engineer for CERP, 2002-2004. Scheduled numerous CERP projects using

Primavera Enterprise (P3e). Managed multiple base lines for projects and status every month to reflect

project progress. Created various management reports that help executives to keep track of current

project performance and predict future course. Analyzed Corps of Engineering Financial management

System (CEFMS) database structure to merge cost information in p3e database.

WATER RESOURCE ENGINEERING

Lake Griffin Flow Way Phase 2; St. Johns River Water Management District, St. Johns County,

FL. Designed flow way and delineated wetland to provide construction drawings for this project that

consists of a 540 acre wetland site, which will treat water from Lake Griffin. The site has two multi- culvert inlets and a pump station that will drive constant flow through the site and thereby provide

treatment. The flow-way is divided into three inter-connected cells with two cells for treatment and one cell for conveyance. Hydraulic analyses in the treatment cells are modeled using two dimensional

finite element techniques (Surface water Modeling System - SMS) to determine flow distribution, short circuiting, and/or dead zones.

TRANSPORTATION ENGINEERING

Drainage Work of Section 6-5 for SH 130, Texas DOT, TX. Performed HEC RAS open channel

hydraulic model to simulate flood stage elevations at four bridges crossing of the State Highway 130.

The streams at these locations did not have detailed FEMA studies and HEC RAS modeling was

performed to determine the bridge length, structure sizing and the 50 year and 100 year WSEL.
