Access ePerformance Use Google Chrome and Sign in with PHC username and password.

Set Up Your Profile Add your profile photo.

Review Your Manager’s Plan Review your manager’s operational goals and objectives.

Enter Your Plan Click the “Create a Plan” button to begin.

Strategic/Operational Priorities Enter your goals and objectives for your Strategic/Operational Priorities.

Personal Development plan Enter your goals and objectives for your Personal Development plan.

Align your GoalsAlign your Operational goals to the Providence Plan. Align your Personal Development plan to the LEADS Capability Framework.

ConversationsSchedule performance conversations with your leader. Update your plan with meeting date, notes and documents from each performance conversation.

Work Your Plan Navigate to your plan at any time by clicking the “Menu” button and the “My Plan” icon.

Goals CreatedThe total number of goals you have created in any status. Goal setting is complete when you have at least one Operational Goal and one Professional Development Goal.

Goals In Progress The total number of goals you have that are in progress.

Goals Achieved The total number of goals you achieved and marked complete.

Conversations The total number of Performance Conversations you have recorded.

Update Your Plan Anytime you make headway toward achieving your goals.

Share Your PlanSelect colleagues to share your plan with. They can only see your Strategic/Operational Priority goals, not your Professional Development goals.

View Other Plans View plans shared by others to see your manager’s plan or any plan a colleague has shared with you.
