Unique Workflow

The Perennity AccessBox is connected to your local network. Simply push DICOM im-ages from your modalities (using standard DICOM Store). Once stored, the studies are immediately available to your diagnos-tic work-station(s) (using DICOM Q/R or Move). Studies may automatically be repli-cated to any secure local storage pool (e.g. RAID based NAS, Silent Cube, ...), or re-mote Data Centre using a secure encrypted Internet connection. Remote Diagnostic centres get access to your studies through these same Data Centres. You will have full control and grant access to authorized us-ers only. This may include doctors, referring physicians and, optionally, patients, allow-ing them to review the studies using a Web Portal.

Diagnostic reports

The Diagnostic Centre or the Radiologists working at a local or remote location, can easily publish their diagnostic report back to the Data Centre using our unique Per-ennity DICOM Report Printer. Type or dic-tate the report using your existing applica-tion (e.g. RIS, the Diagnostic Workstation, ...) and print the report to our Virtual Re-port Printer. Then select the study/patient whose report belongs to (from a Worklist or the Perennity AccessBox) and a DICOM Re-port will be sent to the Perennity AccessBox and made accessible for the doctors using the Web Portal.

Unlimited Storage Capacity

The Perennity AccessBox offers an internal local storage capacity (500GB) for more than 100.000 average studies (based on 50MB, i.e. 5 DR/CR images, per study). This represents a production of more than 50 studies per day during 5 years. When the Perennity AccessBox reaches its inter-nal local capacity, the oldest studies will automatically be deleted from the local storage but will remain accessible from the designated repository (local and/or global). Only a few moments will be required to ac-cess those old studies.

Secure Archiving and Communication

Perennity can use state-of-the-art, on-site long-term archiving technologies, such as Content Addressed Storage (e.g. Silent Cube), to guarantee that all your studies will always remain securely accessible.

You may also decide to keep a copy of your studies in the Data Centre. Studies are encrypted in containers (AES 256bit) and securely transferred from the Perennity Ac-cessBox to/from the Data Centre (HTTPS). No need for complex, expensive and slow VPN connections. A regular fast Internet connection is the only requirement. 3G/4G/UMTS connection will of course work, but the transfer may be slower.

The Perennity AccessBox is also capable of for-warding any received DICOM study to an existing PACS using secure storage (e.g. SAN, CAS, ...).

Cloud technologies and Data Centres

Replicating your valuable and sensitive data, including patient images and diag-nostic reports, outside of your facilities may worry you (just as the checked-in lug-gage): who is taking care of my data? Can I fully trust this Data Centre and where is it located? What support services are they offering? We are partnering with Amazon Web Services (AWS). Today, AWS provides a highly reliable, scalable, low-cost infra-structure platform in the cloud that pow-ers hundreds of thousands of businesses in 190 countries around the world with secure Data Centre locations in the U.S., Europe, Brazil, Singapore, and Japan serving cus-tomers across all industries. AWS is a se-cure, durable technology platform with in-dustry-recognized certifications and audits like ISO 27001, HIPAA, ...

Business Continuity

The automated replication of the studies stored in the Perennity AccessBox to an AWS Centre or a local secured storage sys-tem insures “24/7” accessibility, even in the event of an unforeseen failure.

The AccessBox as a DICOM repository

Let’s start with your local environment. First of all, the Perennity AccessBox is a genuine DICOM Server. Modalities can send DICOM studies to the AccessBox. From there, the Perennity AccessBox can automatically for-ward received images to your local diagnos-tic workstation(s). You can Query/Retrieve studies stored in the Perennity AccessBox from those workstations as well. As result, the Perennity AccessBox acts as a real local archive for your valuable DICOM images.

The AccessBox as a transition to PACS

The Perennity AccessBox is a cost effective solution for clinics that have invested in new digital DICOM-based modalities, replacing film-based equipment, and are planning to invest in a complete PACS solution at a future date. The Perennity AccessBox is an ideal transition solution allowing to start archiving immediately and then investing later in a PACS when new budgets become available. The Perennity AccessBox can be perceived as a “pre-PACS” solution.

Patient CD output & off-line archiving

The Perennity AccessBox includes Perennity DICOM, our proven and world leading Disc Autoloader plug-in, that can be used to out-put Patient Discs. The same robot can also be use for backup and/or Off-Line archiving (e.g. DVD or BluRay) of the content of the AccessBox. Perennity supports Rimage, EP-SON and Primera discs autoloaders.

Exchanging DICOM studies between sites

Some medical institutions may have radi-ology acquisition equipment but are using remote diagnostic Centres or are working with radiologists working from outside loca-tions (e.g. Home, etc). Also, clinics or pri-vate radiology Centres may be working as subcontractors for larger hospitals.

In both cases, DICOM studies must be transferred from the acquisition Centre to the place where the diagnostics will be per-formed. Once the diagnostic report is done, a copy should be sent back to the acquisi-tion Centre to be shared with the referring physician and/or the patient. The challenge here is that radiologists may be using differ-ent tools to write or dictate reports. It can be integrated in their RIS, DICOM Worksta-tion or be a simple text editor. Last but not least, the radiologist may have annotated images (e.g. measurements, ROI, ...).

The Perennity AccessBox can address both challenges: exchanging DICOM studies be-tween several sites and sharing the diag-nostic reports.

Transferring DICOM studies

The Perennity AccessBox is not only a DI-COM storage repository but also an “out-of-the-box” DICOM router appliance. You will need a Perennity AccessBox at both loca-tions: one at the acquisition location and one at the remote location. You may have sev-eral acquisition centres using one or several central diagnostic centres depending on the type of examination. In any case, you will need a Perennity AccessBox at every site. The workflow is straightforward. The des-tination site is defined by a DICOM tag like the referral physician, institution name or even patient ID/name.

Once the study is completed, the local Ac-cessBox will compress it without any loss of the image quality. Compressing the im-ages will dramatically reduce the time to transfer the image from one AccessBox to the remote one. For security reasons, the study will first be uploaded to the Amazon server using highly encrypted connections. The remote AccessBox will watch its “inbox” and download all the studies assigned to it, decompress the study and make it available to the remote diagnostic workstation using automated image forwarding or query/re-trieve. Then the downloaded study will auto-matically be removed on the Amazon server.

Archiving your DICOM studies to the Cloud

The Amazon server can also be used as an archive repository for your valuable DICOM studies. With the Perennity AccessBox pro-gram, you can also purchase our Replica-tion/Archiving Services (RAS), a long-term storage pool provided by Amazon. The price depends on the storage capacity. You can define the maximum capacity of this storage pool starting from 250GB. The AccessBox will automatically send compressed copies of the studies stored in the AccessBox to the RAS. When the RAS is running out of storage capacity, two options are offered to you: you may increase the total capacity of the RAS storage pool or decide to delete the older studies.

If for any reason, your AccessBox or the lo-cal storage system used to extend its ca-pacity is not accessible, all your studies can be recovered from the RAS storage pool.

Make studies available to Doctors & Patients

Film, CD, paper or ... Web? The Perennity Web Sharing Service (WSS) is a Web Portal hosted by Amazon allowing referring phy-sicians, doctors and, optionally, patients to review their studies including the diagnostic report. This Web Portal is only intended for review, not for diagnostic.

Perennity WSS includes a full featured Win-dows 2008 Virtual Server, thereby saving the high costs of acquiring your own Web Server!

Access rights are automatically assigned based on the value of one or more DICOM tags (e.g. the referring physician). Option-ally, the radiologist can also provide access to studies to other doctors manually.

The Perennity WSS is extremely fast in dis-playing studies, even those containing large images. Showing a 4 image mammography study is a matter of only a few seconds! Doctors can easily zoom in/out, adjust the windowing, rotate and move the images.

There are two ways to access and review the studies. First, the doctor can simply log on the WSS Web Portal providing his user name and password. The user can search and filter studies based on criteria such as a range of dates, the patient name, ID or birth date, ... The WSS will display of stud-ies the selected criteria and the study can immediately be opened by clicking it. Ad-ditionally, an email can be sent to the doc-tor to notify him that a new study is avail-able to him. The email contains a link to the Web Portal to directly access the study. Of course, for security reason, the user will have first to log on to open the study.

The WSS Web Portal works with any In-ternet browser and does not require any add-on like Active-X, Java, Silver-light, ... WSS works on any computer: Win-dows, Mac and Linux!

Our WSS also has a WADO (Web Access for DICOM Objects interface) that allow the use of any WADO compatible applications (e.g. Osirix PRO on iPAD).

Local Sales, Training and Support

Your Perennity AccessBox is covered by a local Perennity Partner in your country. He will assist you during the installation and provide you with deep training, hot-line and remote technical support. If your Perennity AccessBox fails for any reason, it will be simply exchanged. The configuration of the Perennity AccessBox and a backup of its local database is stored in a USB stick. Remove it from the failed unit and plug it in the new Perennity Ac-cessBox and your original configuration will be automatically recovered. Your last ar-chived studies will then be retrieved from the Amazon Data Centre and automatically restored to your AccessBox Your business is restored in just a few minutes!
