STEAM GENERATOR EDDY CURRENT

U3 R16 Refueling Outage

1.0 Summary

This report is intended to satisfy the requirements of PVNGS Technical Specifications 5.6.8 for thesubmittal of a Steam Generator Tube Inspection Report. The steam generator (SG) eddy currentexamination for the 16th refueling outage in Unit 3 (U3R16) was conducted during March andApril 2012. Mode 4 entry of Unit 3 Cycle 16 was entered on April 11, 2012. The initial examinationplan for both steam generators is listed in Table 1. This table summarizes the examinationsperformed for each of the various categories, examination types, extents, and the number of tubes ortube locations completed. This examination was a 100% full length tubing inspection.

The examinations resulted in a total of 6 tubes being plugged in SG 31, and 6 tubes being pluggedin SG 32. A description of the previous plugging history for these replacement steam generators iscontained in Appendix E.

2.0 Scope of Examinations Performed

The original examination plan was developed based on the "PVNGS U3R16 Steam Generator'Degradation Assessment" (Westinghouse document SG-SGMP-12-02, dated March 2012)developed per PVNGS Procedure 81DP-9RC01 as required by NEI 97-06. In addition, possibledamage mechanisms were reviewed along with the specific requirements set forth in Procedure73TI-9RC01 and the PVNGS Technical Specifications.

This original plan, along with the examinations performed as a result of bobbin indications noted, issummarized in Table l of this report.

3.0 Active Degradation Mechanisms

The only degradation noted during the examinations was determined to be wear, Section 8.0contains further discussions relating to this mechanism. Table 2 summarizes the results intocategories and sections B and C itemizes all indications reported.

The eddy current examinations were performed by Westinghouse Electric Company using the CoreStar OMNI 200 eddy current instrument. Westinghouse Anser software was utilized to acquire thedata along with the Pegasys robotic manipulator. This robot was configured with a dual guide tubein each of the hot and cold legs.

The tubing was examined with Zetec manufactured bobbin coil probes and Zetec rotating coil (RC)style probes. Probe diameters were 0.590" to 0.610". Plus Point RC probes were used for thecharacterization of non-quantifiable or distorted bobbin indications.

Fiber optic cable was used from containment to the data acquisition room located at the PVNGSNorth Annex. Primary and secondary analysis was all performed on site. The Primary andSecondary Resolution Analysts, Independent Review Analysts, and data management were alsolocated at PVNGS in the North Annex. Westinghouse provided the data acquisition and primarydata analysis. Anatec International, Inc. provided the secondary data analysis.

Each individual from Westinghouse and Anatec International, Inc. who performed data analysis wasrequired to complete and pass a PVNGS site specific Eddy Current Data Analysis Course as well asan associated performance and written examination. All individuals performing data analysis werealso required to have Qualified Data Analyst (QDA) certification. Per 73TI-9RCO 1, all certificationrecords are maintained in the Nuclear Information Records Management System.

5.0 Indication Summary

A detailed listing of the location and mheasured' sizes of all service induced indications recorded isincluded in Appendix B and C. A summary of these indication results is located in Table 2. Inaddition, Appendix A contains a reference drawing of steam generator support locations and reportlegend.

Appendix D contains a listing of the possible loose part (PLP) indications that were confirmed withrotating coil examinations. Note Section 8.0 for further discussion on the PLPs.

There were no indications that were identified as linear during this outage.

6.0 'Tubes Plugged

A summary of the tubes plugged is located in Table 2. Appendix E' contains a map that details theplugged tube location'along with the previously plugged tubes.

7.0 Plug History

A summary of the number and percentage of tubes plugged is also located in Table 2.

8.0 Condition Monitoring

Per the Steam Generator Program, as defined in PVNGS Procedure 81DP-9RCO1, a conditionmonitoring evaluation was conducted by PVNGS Engineering. The results of the eddy current.examinations are provided in Section 5.0. An engineering evaluation of the as-found condition ofinservice tubes did not reveal any degradation exceeding the threshold values for structural andleakage integrity. As such, all steam generator performance criteria were satisfied for Unit 3 Cycle.,16. No tube pulls or insitu pressure testing were required based on the results of the examinations.

Forei2n Object Search and Retrieval (FOSAR)

Tubesheet Annulus and Blowdown Lane FOSAR was conducted using a power cart mounted with aremotely operated camera and retrieval tooling. The applicable requirements of Revision 3 of theEPRI Steam Generator Integrity Assessment Guidelines Section 10.5, Secondary Side Visual'Inspections, were applied for the FOSAR inspections....

'The only loose parts observed in SG31 during U3R16 were sludge rocks. Four sludge rocks wereidentified in the cold leg during FOSAR. Two were in the blowdown lane adjacent to the staycylinder. An attempt was made to retrieve them, but .due to the nature of the RSG design wasunsuccessful. One sludge rock was found in bundle between tubes R163C78 and R164C77. It was'resting approximately 0.5" above the tube sheet wedged between the tubes. No attempt was made toremove it. The last sludge rock was observed between tubes R124C33 & R125C34 at the tube sheet:'An attempt was made to retrieve it, but was unsuccessful due to its. orientation.. No wear wasdetected in the vicinity of any of the sludge rocks.

The only loose parts observed in SG32 during U3R16 were sludge rocks. Two sludge rocks wereidentified in the cold leg blowdown lane during FOSAR. Due to the nature of the RSG design, noattempt was made to retrieve them. One sludge rock was found in the cold leg in bundle betweentubes R19C202 and R21C202. Itwas resting approximately 1.5" above the tube sheet wedged'

between the tubes. A sludge flake was observed in the hot leg between tubes R8C203 and R10C203at the tube sheet. It disintegrated when an attempt was made .to retrieve it. No wear was detected inthe vicinity of any of the sludge rocks.

Also included in the scope of the FOSAR effort was an inspection of the blowdown patch platewelds that were found to be cracked in Unit 3 during U3R1 5. The condition was first identified inUnit 2 during U2R15. The inspections confirmed that the weld material in the vicinity of thecracked weld on all 4 patch plates (2 per SG) is intact, and a loose parts concern is not beingcreated. A previous evaluation concluded that, with the presence of the cracked welds, the patchplates in the Unit 2 SGs will continue to perform their design function and that the probability ofloose parts being formed is remote. Thus, there is a very low risk that the cracked welds will affectthe structural or leakage integrity of tubes in these steam generators.

Potential Loose Parts (PLPs)

As noted in Appendix D, there were no new PLP locations identified in Steam Generator 31 or 32.PVNGS has historically taken the position that if a loose part is detected by ECT or FOSARwithout the presence of wear, it is reasonable to conclude that the required conditions to promotewear do not exist. Per the PVNGS SG Program, trending of these locations will continue in futureoutages.

Finally, PVNGS Procedure 81DP-9RCO1 requires, per the EPRI PWR Steam GeneratorExamination Guidelines, that a visual inspection of the previously installed steam generator plugsbe performed to assess plug integrity. Additionally, the Examination Guidelines Section 6.9.1 states- "Verify the location and presence of existing in-service plugs." The conduct of the plug locationand integrity verification was performed in U3R16 per procedure 81CP-9RC40. A review of theinspection results indicated that all plugs were accounted for and no evidence of potential plugleakage was identified.
