Welcome to the Oregon EMS Conference in sunny Bend, Oregon. We hope that everyone has had a wonderful year! We are excited about the dynamic speakers that are com-ing and we are honored that you have chosen to attend the Oregon EMS Conference.

We are welcoming back speakers Bob Page and Ritu Sahni and are happy to introduce Dan Limmer and Mike Shertz. If you have not heard them speak, prepare yourself for an educational experience that is both enriching and fun. These speakers as well as our other speakers this year are leaders in their field and are bringing wonderful educational content to this year’s conference.The cadaver lab continues to be a big success and will once again be held as one of our preconference courses. This course is an awesome opportunity to get hands on experience in human anatomy. Remember to sign up early since this course fills up fast!

We are also very excited to offer a new preconference course this year in Sim Technology! This course will be scenario based where students are immersed in real life scenarios with high-fidelity pediatric and adult manne-quins that provide feedback to the student. This course is designed for all levels of providers and will prove to be an enriching and educational experience for all.

The Post Conference BLS course will also be offered again this year. We had very positive feedback from last year’s post conference and are happy to offer this course again to our basic providers. The conference is at the Bend Riverhouse Conference Center in Bend, Oregon. They have been wonderful hosts to us in the past and we are looking forward to being there again this year. The committee would like to thank all of you for all the hard work that is done on a daily basis to make EMS great in Oregon. Thank you also for all of the support in making this conference great!

Attendees will have the opportunity for hands-on participation. Following each specific demonstration, further dissection will be conducted to support the theory of the subject technique. Controversies regarding these techniques will be discussed. The following emergency procedures will be included, but other procedures may be incorporated: Emergency airway (Cricothyrotomy and Intubation); Carotid massage; Cardiac massage/resuscitation; Scalene block for upper limb trauma; Supraclavicular tamponade to control upper limb bleeding; Thoracentesis; Pericardiocentesis; Emergency relief of tension pneumothorax; Paracentesis; Acute organ trauma; Peripheral venous access – neck (upper limb, lower limb); Arterial pulses. If anyone told you that you could take a 12 lead class and have fun, would you believe them? In this session, Bob Page will take you to the next level. How to squeeze every bit of diagnostic information from your monitor will be reviewed and examined in this 8-hour, highly motivating, non-stop interactive course on 12-Lead ECG. Bob will take the provider through the steps utilizing 12 Lead to recognize important acute care topics including proper lead placement, 12 lead validation, STEMI, 15 lead ECG, Axis and Hemiblocks, BBB, and Wide complex tachycardia. Also included is the use of a 15-lead ECG. Participants in the program will read approximately 200 12-lead ECGs, gaining both experience and confidence in their newly learned skill. There is also web site support of graduates of the program by continual competency and feedback from the instructor. During the past 24 months Dan has been working on a photo documentary of EMS. Throughout the country EMS agencies, providers and patients have allowed him to document their stories in a collection of images. In this photo-driven presentation he shares this experience with you providing both interpersonal and clinical insights that will both sharpen your skills and remind you of the EMS we passionately strive to provide.Daniel Limmer is an EMT-Paramedic who has been involved in EMS and Law Enforcement for more than 32 years and practices as a paramedic with Kennebunk Fire Rescue in Kennebunk, Maine. He is a co-author of Emergency Care, EMT Complete, Essentials of Emergency Care, Advanced Medical Life Support and First Responder - A Skills Approach. Dan has presented previously at the Oregon EMS Conference and his presentations are always well received and engaging. His ability to “speak to the BLS provider” is exceptional.

Pre-hospital Therapeutic HypothermiaPresenter: Dr. Jim Bryan0920 - 1030The 2010 ACLS guidelines recommend therapeutic hypothermia for comatose patients with Return Of Spontaneous Circulation (ROSC) after out-of-hospital VF cardiac arrest as well as arrest patients with an initial rhythm of PEA or asystole. The impact of the timing of initiating hypothermia is not completely understood, but studies have suggested the benefit may be lost when therapy is delayed. This presentation will review the available literature on the value of therapeutic hypothermia, current methods for inducing hypothermia and the benefits of initiating hypothermia in the prehospital setting.Jim Bryan, MD, PhD, is the medical director for Life Flight Network and a staff emergency physician at the VA Medical Center and Oregon Health & Science University in Portland. He previously served as medical director for the OHSU/OIT Paramedic Education Program for 14 years. Dr. Bryan has presented at multiple EMS conferences in Oregon, Washington, and Idaho on the topics of cardiac resuscitation and EMS pharmacology.

Tourniquets Really? How advancements in the application and use of these devices have proven to save thousands of lives.Presenter: Dr. Michael D. Shertz1435 - 1545

To date, more than 2000 lives have been saved with the liberal use of tourniquets by US forces in Iraq and Afghanistan. During this session we will discuss issues relating to selection, correct application and use of commercially available tourniquets, as well as the military’s experience using them on casualties over the last ten years of war. Michael D. Shertz, MD FACEP - Dr Shertz is a board certified Emergency Physician practicing at St Vincent Medical Center in Portland, Oregon.

He is the physician advisor for Hillsboro Fire Department, Washington County Fire District #2 and Gaston Fire Department. In Mike’s presentation, the role of the Emergency Medical Responder and EMT will be examined in the relation to working with the agency’s transport provider in assisting for a smooth informative transfer of care; working through the many challenges, ranging from emotional disconnect, where is your equipment going, and what might be expected from you riding along to assist in patient care. EMR and EMT scope of practice will be reviewed, and “how much” a provider may do under the direction of a higher level provider will be discussed. Time will be allowed for interactive

The concepts behind the Department of Defense’s Tactical Combat Casualty Care guidelines will be presented. These guidelines are credited with decreasing the killed in action rate in modern combat for the first time in over three hundred years of warfare. Currently only one in every ten wounded service members dies from injuries sustained in combat. Traditionally that number has been one in four. Additionally we will discuss how these military guidelines can be applied to civilian EMS / first responders through the TCCC civilian counterpart the Tactical Emergency Casualty Care guidelines.See bio on page 11.

Shock has several causes. It can be hemorrhagic, septic, or cardiogenic. For EMS the treatment and recognition of shock is central to understanding how to adequately treat patients. This lecture discusses the cause, recognition, and treatment of shock. The physiology of shock as well as treatment will be discussed. Dr. Eschelbach will bring firsthand experiences of dealing with the “shock” patient in the rural setting. This track is appropriate for all levels of providers from EMR to Paramedic. Dr. Matthew Eschelbach has been involved in EMS for greater than 20 years. He is the medical director for many central Oregon agencies as well as being involved in EMS education. He has served as the medical

director of the EMS conference and is dedicated to the advancement of EMS in the State of Oregon. Matt was the recipient of the 2001” Facility Medical Director of the Year” awarded by Northwest Emergency Physicians and in 2003 he was named the” Oregon EMS Medical Director of the Year”. What is Critical Care? What is Critical Care Transport and is it any different from other ALS patients? What training currently exists and does it make any difference? Many athletes try to play through a concussion or return too soon before being fully recovered. We need to recognize and treat concussions much better than a sprained ankle. Sports Concussions are more than a Sprained Brain! Concussions seem to be occurring more frequently and causing more serious side effects than ever before. But not everyone knows how to recognize or optimally treat a concussion, not even much of the medical community. Most people will recognize a sprained ankle and know to rest the sprained ankle long enough for pain and swelling to improve and then slowly transition back to play to allow for improved function and to prevent risk of re-injury. Dr. Chesnutt will share with the pre-hospital provider a solid understanding of the range of severity in the injured athlete. Dr. Jim Chesnutt , is the medical director of the OHSU Sports Medicine Program and Co-Director of the Oregon Concussion Awareness

and Management Program. OHSU Sports Medicine has the most comprehensive concussion management and rehabilitation program in the state, serving local schools and universities and the community through direct patient care, research, athletic training programs in schools and medical professional and community education. Conference Scholarships Available!

The ODOT, Transportation Safety Division is providing a grant offering a limited number of scholarships to cover registration for the 2012 EMS Conference! The scholarship amounts will be the $249 tuition for the two day conference. Scholarships will be given to support those who may otherwise not be able to attend.

Scholarships will be awarded to 10 Oregon residents responsible for EMS in rural/non-urban Oregon who respond to motor vehicle crashes and/or provide prehospital and/or hospital care to motor vehicle crash victims, especially pediatric patients.

As the Transportation Safety funds are targeted for motor vehicle and highway safety, we recommend that scholars primarily attend sessions that relate to motor vehicle crashes, such as From Battle Field to Hay Field; Lessons Learned in the Care and Treatment of the Traumatic Patient, Shock, and classes to meet CEU requirements.
