obligors for the reporting on their dying ormissing according to the provisions of theSensus-reglster Law shall take the formali-ties under the preceding paragraph within30 days.

Article 7. Matters to be reported according tothe provisions of Art. 6, Par. 3 of the Med-ical "Practitioners; Law (Law No. 201, 1948,hereinafter referred to as the Law) shall be'as follows :1. Full name, age,and gerider;2. Permanent address and present address;3. Number of registration-.on the medical

.practitioners* list and its date ;4: In case of those who are engaged in,

medical profession, ,the following matters,..tooV; ' :

Articled. ' Medical practitioners, In case 'they'have changed ,their addresses, shall reporttheir old and1 -newaddresses within' 10 days,to the prefetturai governor exercising juris-diction over their new addresses.

2 In case a prefectural governor receiving re-ports according' to the- preceding paragraph

, differs from the prefeotural.governor exer-cising jurisdiction >ver their former addres- "

-.ses, their new addresses shall be reportedto the prefectiiral governor of.exercisingjurisdiction over their former addresses bythe 'prefectural governor who has receivedthe said reports.

3 In case medical practitioners want to,re-move their addresses to places where' thisMinisterial Ordinance Is not in force, theyshall report it previously to the prefecturalgovernor exercising jurisdiction over theirdwelling places. ^.

Article 9..Those who have 'had their licencecancelled shall return their certificates of

. *- licence to the Minister of Welfare throughthe prefectural governor exercising jurisdic-tion over their dwelling places within 5days.

Article 10. Those who have had their medicalprofession suspended, shall submit theircertificates of licence to the prefectural gov-

ernors exercising jurisdiction over theirdwelling places within, 5 days.

! In case of preceding paragraph, the prefec-tural governor, writing the substance of thesuspension of medical profession on the re-verse side of the certificates of licence withseal, shall keep them to' return after^theexpiration, of the term.

Chapter 1L ExaminationArticle ll/ The Internship on medical treatment

in accordance with ,the provisions of Art. II ,of the; Law shall be carried out In the fol- 'lowing facilities :.

. - 1. Attached hospitals (including substitute•E , "attached hospitals) of the; -universities

mentioned in Item.l of Artll of the Law,. (including those deemed as universities

3o In case of those who come under Art.. 'll, Item 1 of the Law, certificates of

graduation and papers certifying the cora-•Epletion ^of Internship issued "by the heads

of facilities where Internship has.beencarried' out ;

4. In case' of those who come under Art.ll, Item 2 of the Law, copies of the cer-tificates of their passing the preliminaryexamination or papers certifying that theyhave passed the preliminary examination

Article 15 Those who want4 to take the preli-minary examination "shall submit to theMinister of Welfare written applications fortaking the preliminary examination (FormNd. 3) annexed with the following docu-

ments: ,1 Curriculum vitae (Form No. 4);2 * Documents specified in Items 2, 5 and 6' of Art. 13. However, as to documents,as

specified in Item 6, a mark of \vmj shall

be put instead of a mark [raj in the re-verse side of the papers.

Article 16\ Those who apply for taking thenational examination or preliminary exami-nation shall pay in 400 yen (in case of thosewho apply for either one of the 1st and the-2nd examination of the preliminary exami-nation, 300 yen) as the fee for it.

Article 17. Those* who have passed ,the* nation-al examination or the preliminary examina-tion shall be given certificates of eligibility*

Article 18. In case the original certificate ofeligibility has been stained, damaged or lost, •Eone may apply•E for <the issue- of a papercertifying ' eligibility.

2 One who applies for the issue of a papercertifying the eligibility according to thepreceding paragraph shall ,paf in 60 yen asthe' fee for it.. ,

•EArticle-,19. The fee shall be paid in by puttingon 'the written application a revenue stampcorresponding to the' amount of tlie fee.

Chapter III aPracticeArticle 20. A medical practitioner shall describe-

; oa a medical certificate or post-mortem.cer- %tificate which he delivers the following mat-ters, putting his name and seal'or sign: ^1. Full name, age and,sex of the dead;2. The date of being taken ill; " -\3. The date 'and hour of death;4. The place of death- (including the name

of a hospital, cliftic or midwifery, home,In case of death in an'y one of them);

5.^ Kind of death1;,6. Cause of death and the term of its con-\ tlnuation ;k Any" annexed disease or other conditions

x ' of body and^the term of itscontinuation;*8 Principal opinion concerning an'.opera-

tion and the date of an operation;> 9. Principal opinion concerning dissection ;

made out the concerned document and thefact that he is a medical practitioner.

Article 21. Medical practitioners shall describe,on the prescription delivered to the patient,the full name and age of the patient, name

of medicines, quantity, directions for use,dose, date of the prescription, term of usageand name and address of the hospital orclinic or address of the medical practitionerand put his name and seal or sign.

Article 22 Medical practitioners shall describeclearly on the receptacles or covers of themedicines delivered to the patients, direc-tions* for,use, dose, date of delivery, fullvname of the patient and name and addressof the hospital or clinic, or address andname of the medical practitioner.

Article 23. Matters to be entered in the patientrecord-book shall be as follows :1. Address, full name, sex and age of one

treated ;2. Name of disease and the mainoonditions

of the illness;3. Method of treatment (prescription and

treatment) ;4. Date of treatment.

Supplementary Provisions :Article 24. This Ministerial Ordinance shall be

put into force as from th,e day of the en-forcement of the Law.

Article 25. Regulations for* the Enforcement ofthe National Medical Treatment Law (Min-istry of Welfare Ordinance No. 48, 1942),Ordinance concerning Enforcement of Im-perial Ordinance No. 42, 1946 (Ministry ofWelfare Ordinance.No. 6, 1946) and Ordi-nance concerning matters to be entered indeath-certificates and post-mortem certificates(Ministry for Home Affairs Ordinance No.41, 1900) shall be abolished.

Article* 26. In. case medical practitioners whohave been given licence without taking na-tional examination in accordance with theformer provisions, having taken and passednational examination, want to have it reg-istered on the medical practitioners list, theymay apply for correction of medical practi-

tioners list to the Minister of Welfarethrough the prefectural governor of theirdwelling places annexed with a copy of thepaper certifying the eligibility and the certi- ,ficate of licence.

2 In the case of the preceding paragraph,the certificate of licence shall be rewrittenand delivered.

Article 27. Formalities of application, mattersto be registered on the medical practitionerslist and procedure of taking examinationand other matters concerning examinationof those who come under Art. 36, Par. 3 ofthe Law shall still be in accordance withthe former examples. ;

Article 28. Procedure of an application for li-cence of those who come under Art. 41 ofthe Law shall still beb in accordance withthe former examples.

Chapter I. LicenceArticle 1. Those who want to obtain the licence

of dentists shall submit to the Minister ofWelfare through prefectural governors exer-cising jurisdiction over their dwelling placeswritten applications (Form No. 1) annexedwith the following documents :1. Copies of their certificates on their pas-

sing the National Dentists' Examination.(hereinafter referred to as the NationalExamination) ;

2. Copies of their sensus registers or ab-stracts of them ;

Article 6. Dentists, in case they want to applyfor an erasure of tjjeir registration on the-dentists' , list, shall _return their certificatesof licence, stating the reasons, to the Min-ister of Welfare, through the prefecturalgovernors exercising jurisdiction over theirdwelling places.

2 In case dentists have been adjudged asmissing or having died, the obligors for thereports on their -dying or missing accordingto the provisions of the Sensus-register Lawshall take formalities under the precedingparagraph within 30 days.

Article 7. Matters to be reported according tothe provisions of Art. 6, Par. 3 of the Den-tists Law (Law No. 202, 1948, hereinafter

referred to as the Law) shall be as follows:1. Full name, age and sex;

list and its date;4. As to those who are engaged in dental

profession, the following matters in ad-dition : ^(a) Address of the hospital or the clinic

profession is carried ony its name andits establisher ;

Article 9. Those who have' had their licencecancelled, shall return their certificates of

Articles 10. Those swho ^have ,had their dentalprofession suspended shall return their certi-ficates of licence to the prefectural governorshaving jurisdiction over their dwelling placeswithin 5" days. -
