We respectfully request an exception to Statewide Rule 701-A, as provided by Rule 7 01-D(1), to allow administrative approval of the proposed Artesia SWD Well No. 1 for use as a salt water disposal f a c i l i t y with disposal to be into the non-productive Lower San Andres and Glorieta formations. The proposed well i s to be d r i l l e d i n Unit 0, 330 ' FS and 2310 ' FE line s , Section 7, T-18-E, R-28-E, Eddy County, New Mexico. Details of the proposed application and f a c i l i t y are presented on the enclosed Form C-108 and i t s attachments.
Offset operators have been n o t i f i e d , by c e r t i f i e d mail, of this application as eviderced by Xerox copies of the mailing envelopes. The legal notice of this application i s to appear i n the Artesia Daily Press today with publication c e r t i f i c a t i o n to be furnished you as soon as i t i s available from the paper. Give the geologic name, and depth to bottom of a l l underground sources of d r i n k i n g water (a o u i f e r s containing waters w i t h t o t a l dissolved s o l i d s concentrations of 10,000 mg/l or less) o v e r l y i n g the proposed i n j e c t i o n zone as we l l as any such source known to be immediately underlying the i n j e c t i o n i n t e r v a l .

A. The f o l l o w i n q w e l l data must be submitted for each i n j e c t i o n well covered by t h i s a p p l i c a t i o n . The data must be both in tabular and schematic form and s h a l l include:
(1) Lease name; Well No.; lo c a t i o n by Section, Township, nnd Range; and footage l o c a t i o n w i t h i n the section.
(2) Each c;sing s t r i n g uoed with i t s s i z e , s e t t i n g depth, sacks of cement used, hoie s i z e , lop of cement, and how such top was determined.
(3) A d e s c i i p t i o n of the tubing to be used i n c l u d i n g i t s s i z e , l i n i n g m a t e r i a l , and set t i n c depth.
(4) The nane, model, and s e t t i n q depth of the packer used or a d e s c r i p t i o n of any other seal system or assembly used.
D i v i s i o n D i s t r i c t o f f i c e s have supplies of Well Data Sheets which may be used or which may be used as models for t h i s purpose. Applicants for several i d e n t i c a l wells may submit a " t y p i c a l data sheet" rather than submitting the aata for each w e l l .
8. The f o l l o w i n g must be submitted for each i n j e c t i o n w e l l covered by t h i s a p p l i c a t i o n . A l l items must be addressed for the i n i t i a l w e l l . Responses for a d d i t i o n a l wells need be shown only when d i f f e r e n t . Information shown on schematics need not be repeated.
(1) The name of the i n j e c t i o n formation and, i f a p p l i c a b l e , the f i e l d or pool name.
(2) The i n j e c t i o n i n t e r v a l and whether i t i s p e r f o r a t e d or open-hole.
(3) State i f the w e l l was d r i l l e d f o r i n j e c t i o n or, i f not, the o r i g i n a l purpose of the w e l l .
(4) Give ths depths of any other perfo r a t e d i n t e r v a l s and d e t s i l on the sacks of cement or bridge slugs used to seal o f f such p e r f o r a t i o n s .
(5) Give t h i depth to and name of the next higher and next lower o i l or gas zone in the area of the w e l l , i f any.

A l l a p plicants must f u r n i s h proof that a copy of the a p p l i c a t i o n has been furnished, by c e r t i f i e d or r e g i s t e r e d m a i l , to the owner of the surface of the land on which the well i s to be lo c i i t e d and to each leasehold operator w i t h i n one-half mile of the w e l l l o c a t i o n .

(1) The name, address, phone number, and contact party f o r the a p p l i c a n t ;
(2) the intended purpose of the i n j e c t i o n w e i i ; w i t h the exact l o c a t i o n of s i n g l e wells oi the s e c t i o n , township, and range l o c a t i o n of m u l t i p l e w e l l s :
(3) the f o r n a t i o n name and depth wit h expected maximum i n j e c t i o n rates and pressures; and

That property, as shown on the land map, should have i d e n t i f i e d Aminoil, Inc. as the owner. P h i l l i p s now i s the successor to Aminoil by acquisition.
