Paint was sampled from the wooden lattice north of the fountain (Photograph No. A-10). 8. Paint was taken from the southeast corner of the garage (Photograph No. A-11) 9. Paint was sampled from two block wall locations, adjacent to the washroom, within the garage building (Photograph Nos. A-12, A-13). 10. EXP visited the site at random times between August 30, 2017 and March 5, 2018, as noted above, during material

sampling and borehole installations (this sampling and investigations performed by others) to be in attendance and

visually sample these material sampling and investigation proceedings in-progress, and the subsequent

repairs/reinstatements, where pertinent, performed as part of these investigations.

A.1 Material Sampling Locations

EXP observed the collection of material samples from various areas throughout the property. Materials were collected

(by others) from the following locations on August 31, 2017:

1. A shingle, found on the ground, adjacent to the small structure behind the pool house was collected

(Photograph No. A-1).

2. On the west elevation of the pool house, paint was collected from the wall, exterior soffit, the frame of the

door leading to the pool house basement and the metal eavestrough and downspout (Photograph Nos. A-2,

A-3, A-4, A-5).

3. Two samples were taken from the surface of the pool house roof material, both samples were approximately

1.5” x 6” in size.

4. Residue from the main entry threshold was taken from the east elevation of the pool house (Photograph No.

A-6, A-7).

5. Paint was sampled from the wall around the shower of the pool house’s west washroom (Photograph No. A-

8).

6. Some paint was collected from within the concrete fountain north of the swimming pool (Photograph No. A-9).

7. Paint was sampled from the wooden lattice north of the fountain (Photograph No. A-10).

8. Paint was taken from the southeast corner of the garage (Photograph No. A-11)

16. Paint was sampled from various locations along the main residence building’s veranda floor surface and at

several column bases.

17. A shingle was found on the ground north of the main residence building and collected.

18. Paint samples were taken from the main residence building’s furnace room door and door frame, and from

the corridor wall adjacent to the door.

19. Material from the pipe insulation in the main residence building’s furnace room was collected at a location

where the insulation was already damaged and exposed.

20. Paint was sampled from the north wall adjacent to the main residence building’s laundry room window

(Photograph Nos. A-20, A-21).

21. Paint was sampled from the northwest corner of the wall in the main residence building’s basement storage

room (Photograph No. A-22).

22. Three floor tile samples were taken from below the radiator in the main residence building’s back kitchen. The

samples were approximately 1”x 2” each (Photograph No. A-23, A-24).

23. Paint was sampled from the window sill in the in the main residence building’s back kitchen (Photograph No.

A-25).

24. Paint was sampled at a location where the paint finish had been damaged previously at the stairs leading to

the main residence building’s north exit door (Photograph No. A-26).

25. Paint samples were taken from the back corner of the wall and the radiator in the main residence building’s

back bedroom 3 (Photograph Nos. A-27, A-28).

26. Flooring samples were taken from below the radiator in the main residence building’s back bathroom. The two

samples were approximately 0.5” x 3” each (Photograph No. A-29).

27. Some flooring material was sampled from the main residence building’s hall bathroom from below the

carpeting at the threshold. The sample was approximately 4sq.in.

28. Paint samples were taken from the main residence building’s sun room ceiling and door.

29. Flooring samples were taken from under the radiator in the main residence building’s master bathroom.

Samples were both approximately 1”x 6” (Photograph No. A-30).

30. Paint was sampled from the baseboard in the main residence building’s yellow bedroom (Photograph Nos. A-

31, A-32).

31. Insulation from the main residence building’s attic was collected.

32. Paint was sampled from the windowsill in the main residence building’s bedroom 3 (Photograph No. A-33).

A.2 Finishes at Sampled Locations

All paint samples were small (consisted of a few flakes) and taken from areas where the paint finish was deteriorated

(peeling). Therefore, in EXP’s opinion the collection of paint samples created no appreciable difference in the finish at

all locations sampled. These locations have not been repainted. Floor tiles/material have not been reinstated at

locations where the floor tiles were removed. These locations are by the wall below radiators or under carpet, and

thus not easily visible. Roofing materials sampled were generally from shingles that were already off the roof. The

one location where material was sampled from a roof was some minor surface scraping (at the pool house roof) that

did not affect the integrity of the roofing system. Similarly, roof attic insulation samples taken from the garage and

main residence buildings, were small and their removal from the insulation system would also not affect the

performance of this system. Where insulation was taken elsewhere (around a pipe), it was at a pre-existing damaged

location. Insulation was not reinstated where collected nor was the pre-existing damaged pipe insulation location

repaired.

B.1 Borehole/Monitoring Wells and Core Locations

On August 31, 2017 and September 1, 2017 EXP observed the drilling of boreholes and monitoring wells on the

property. EXP was not present for the drilling of every borehole. EXP did observe the removal of the drilling

equipment from the property after the drilling. Figure 1, supplied by the borehole installer, shows the locations of

boreholes and monitoring wells.

1. On August 31 EXP observed the drilling of BH/MW3 on the driveway adjacent to the north side of the main

residential building (Photograph No. B-1).

2. On August 31 EXP observed the drilling of BH/MW4 and BH/MW5 in the basement of the main residential

building. BH/MW4 was in the storage room east of the stairs to the north exit door. BH/MW5 was in the

furnace room (Photograph Nos. B-2, B-3).

3. On August 31 EXP observed the drilling of BH/MW2 at the northwest corner of the property between the

garage and the storage building (Photograph No. B-4).

4. BH/MW1 was drilled at the south west corner of the pool house. EXP did not observe the drilling and the

condition of this area was reviewed the following day.

5. On September 1 EXP observed the drilling of BH7 east of the main residential building.

6. On September 1 EXP observed the drilling of BH6 east of the garage (Photograph No. B-5). BH8 and BH9

drilling was not observed however the condition of these areas (BH8 – south of the driveway in the NE corner

of the property and BH9 – south east corner of property) was reviewed after the drilling was completed.

On October 10, 2017 nine additional ‘Delineation Boreholes’ were drilled. Refer to figure 2, supplied by the borehole

installer, below for the locations of these additional boreholes. EXP observed the drilling of BH15 at the north door to

the main residential building (Photograph No. B-6) and BH10 north of the garage.

On October 23, 2017 EXP observed coring of the foundation walls in the main residential building. EXP observed one

core being taken from the south wall of the basement corridor that services the laundry room, storage rooms and

stairs that lead to the north exit door. Two cores had been completed prior to EXP arriving on site. One was taken

from the west wall of the laundry room (Photograph No. B-7). The other was taken from the east laundry room wall,

however two attempts were made as the first location reportedly did not provide enough stability for the drill

(Photograph No. B-8). Two additional cores were taken from the basement storage room wall (at the east end of the

basement). EXP was not present when these cores were taken. It was reported to EXP that the homeowner gave

input on the locations for the cores. Furniture and other items were removed from the area while the cores were

taken.

B.2 Reinstatements at Borehole/Monitoring Well and Core Locations

Interior boreholes have been filled with a cementitious material (Photograph No. B-9). Exterior boreholes and

monitoring wells were also decommissioned and the holes have been filled. Boreholes on the lawn were filled with soil

or granular material (Photograph No. B-10, B-11). BH8 is on the lawn, south of the driveway, and appears to have

been filled with a cementitious material. The borehole taken from the driveway appears to have been filled with

cementitious material (Photograph No. B-12). Cores taken from the main residence building’s foundation wall were

removed and the holes were filled with a cementitious material. The contractor drilling the cores informed EXP that

they intended to use Set 45 Mix Hydraulic concrete to fill the holes. However, EXP did not observe these core holes

being filled. Metal anchors that were drilled into the stone foundation walls were not removed.

C.1 Site Finish Deficiencies and Damage from the Investigations

On March 5, 2018 EXP reviewed all the borehole/monitoring well and core locations and performed a cursory review

of the property for evidence of damage to site finishes and buildings that could have been caused as a result of the

borehole/monitoring well drilling, decommissioning, and core extraction. Except for the two items below, reported by

the Owner, EXP did not note any evident damage to the site finishes and buildings that could have been caused by

the borehole/monitoring well drilling/decommissioning, and core extraction operations.

The following damage to the site were reported to EXP by the Owner:

1. The stone path south west of the main residential building was damaged by the drilling machine when it drove

over the path. Some stones were cracked (Photograph Nos. C-1 and C-2). There were approximately 10

stones affected.

2. Stones along the north edge of the driveway north of the house were damaged/chipped. Two areas were

affected (Photograph Nos. C-3 and C-4).

The above locations have not been repaired. The damage at the above two locations was not witnessed by EXP and

cannot be confirmed by EXP.

The following additional observations were noted by EXP:

1. The surface of the cementitious patch material used to fill the main residence building’s boreholes (in the

storage room and furnace room) is higher (i.e. not flush) with the surface of the surrounding floor (Photograph

No. C-5).

2. Metal anchors for the coring drill have not been removed from the foundation walls at each wall core location.

However, there was no evidence of water leakage at these locations at the time of EXP’s review (Photograph

No. C-6).

3. Soil and sod was not reinstated at the exterior borehole locations.

4. The cementitious material used to fill BH3 (north of the main residential building on the driveway) appears to

be chipped/cracked (Photograph No. C-7).

5. BH8 appears to have been filled with cementitious material unlike other boreholes on the lawn (Photograph

No. C-8).

6. The tread marks from the borehole drilling machine used on August 31 and September 1, 2017 were still

evident on the lawn to the east of the house (Photograph No. C-9). The damage at this location was not

witnessed by EXP and cannot be confirmed by EXP.

The reinstatements at the borehole/monitoring wells and core locations appear to be structurally adequate and, with

respect to the foundation wall core locations, do not appear to affect the water tightness of the foundation wall at

those locations. However, the finishes noted above have not been reinstated, or removed in the case of the metal wall

anchors, to match the existing pre-investigation finishes, and as such would be considered deficient if the aim is to

reinstate the finishes to match the existing pre-investigation finishes.
