1 I would like to take this opportunity to welcome the returning students and the new students to our district. Our entire CCISD staff wants every student to be happy and successful. We be- lieve that parents and guardians play a key role. That role is to con- tinually show interest in, and support for your child. The child who knows that his/her family is interested in and supportive of school programs will almost always have fewer prob- lems in school and will enjoy greater success. Strong relationships are a key component of an effective learn- ing environment. I encourage all educators, students, and parents to build strong relationships with our staff. Community building is a strong, ongoing process in establish- ing a culture of excellence within our District, the schools and the class- room. We believe in fair and consistent discipline that encourages students to be responsible for themselves. At school, we foster a safe environment where courtesy and kindness prevail and where there is respect. Our students are our collective work and effort. District Committee Seeks Parent and Community Members

Crystal City ISD is seeking volunteers to serve on its District Site-Based Decision Making Committee for the 2016-2017 school year. The committee's main responsibilities are to advise the Superintendent in the annual planning, develop-ment, evaluation and revision of the District Improvement Plan and to review, recommend and approve district staff development for the year. The committee will meet at least two times yearly. Parents of students enrolled in the district, community mem-bers who are not parents of district students and business representatives are encouraged to contact Imelda Allen, Superintendent of Schools, or Adelicia Leeper, Federal Pro-grams Director, at 374-2367 to express their interest in serv-ing on the committee. The deadline for requesting a com-mittee position is Friday, September 11, 2016.

IMPACT Participants Needed!

Crystal City ISD holds monthly meetings with interagency and

business representatives within our community to seek solu-

tions to school challenges and local issues that impact the

students and families in our district and our town. Community

members are encouraged to participate in these meetings and

to collaborate with the school district to strengthen our commu-

nity efforts and partnerships. The first meeting of the new

school year will be held at 10:00, Tuesday, September 13,

2016, at Central Office.

District Parent Committee Seeks Parent Advisors

The District Parent Advisory Committee will hold its first meet-ing of the new year on Monday, September 26, 2016, at 6:00 P.M. at the Central Office Board Room. This meeting is open to all parents who would like to participate in monthly meetings designed to share information about the school programs and to seek parent input around various topics and initiatives con-cerning the district. Childcare is available upon request. An interpreter is also available should there be a need for transla-tion. 2016 –2017 IMMUNIZATION REQUIREMENTS

The minimum immunization requirements for the 2016-2017 school year are listed below. Immunization records showing the following immunizations must be submitted before the first day of the 2016-2017 school year. Students are not allowed to attend school without a current immunization record on file. Immunization Requirements for Pre-K

All students in Pre-K must have proof of: 4 doses of DTaP, DTP, DT vaccine 3 doses of Polio vaccine 1 dose of MMR vaccine administered on or after 1 year of age 1 dose of Varicella vaccine administered on or after 1 year of age 3 doses of Hepatitis B vaccine 1 dose of Hepatitis A vaccine administered on or after 1 year of age for students younger than 42 months of age, 2 doses are required for students

older than 42 months of age 3 doses Pneumococcal vaccine with at least one dose being administered after 1 year of age; if 3 doses were received prior to 1 year of age, an

additional dose is required (4 dose total). Only two doses are required if the first dose was on or after the first birthday. Only one dose is required if it was received on or after the age of 24 months.

2 doses Hib vaccine plus a booster dose on or after 12 months of age. If the 1st dose was received at 12-14 months of age, only 1 additional dose is required; or 1 single dose on or after 15 months of age.

Immunization Requirements for Kindergarten through 12th grades

All students in kindergarten through 12th grades must have proof of:

5 doses of DTP/DTaP/ DT unless the 4th dose was given on or after the 4th birthday, then only 4 doses are required. Three doses will meet the requirement for students in 7th grade and up if the last dose was given on or after the 4th birthday. A dose of DTP/DTaP/DT must have been

received on or after the 4th birthday for all students. Tdap booster for all students entering 7th grade if it has been five years or more since the last dose of tetanus containing vaccine. If 5 years have

not passed at 7th grade entry, then the dose is due as soon as the 5 year interval has passed. 1 dose of Td (tetanus booster) is required every 10 years after the last dose of DTP/DTaP/ DT; Tdap is recommended as a replacement for the 1st

Td booster. Students in grades 8 through 12 are required to have a booster dose of Tdap if it has been ten years since their previous dose of tetanus/diphtheria containing vaccine. 4 doses of Polio vaccine unless the 3rd dose was given on or after the 4th birthday; a dose must have been received on or after the 4th birthday for

all students. 2 doses MMR vaccine, the first dose must have been given on or after the first birthday 3 doses of Hepatitis B vaccine 2 doses of Varicella vaccine with the first dose received on or after the first birthday. For students who had chickenpox disease, an approximate

month and year of chickenpox disease must be documented on the immunization record or the parent must send in a signed statement stating the approximate date their child had chickenpox disease.

2 doses of Hepatitis A vaccine for all students entering K through 7th grades. The first dose must have been received on or after the first birthday. 1 dose of Meningococcal vaccine received on or after the 11th birthday for all students entering 7th through 12th grades.

Immunization Exemptions

Medical Exemptions The law allows physicians to write a statement if he believes a vaccine would be harmful to the health of a child. These statements must be dated and signed by the medical doctor and renewed annually unless the physician writes in the statement that it is a life long condition. For children needing medical exemptions, the physician’s statement must be submitted to the school by the parent prior to the first day of school.

Exemption for Reasons of Conscience Exemptions for reasons of conscience, including religious beliefs are allowable. To claim this exemption from immunization for reasons of conscience, the parent or legal guardian must submit a written request to obtain the Affidavit of Exemption from Immunizations for Reasons of Conscience to the Department of State Health Services Immunization Branch, 1100 West 49th Street, Austin, Texas 78756. Fax: 512-458-7544. Affidavit request forms are usually processed by the DSHS and mailed within 2 weeks from receipt of the request. Once this form is received by the parent or legal guardian, they must sign and have the form notarized and submit the original form to the school. Copies are void and will not be accepted. Since proof of immunization status is required before the first day of school, to claim this exemption, this form must be submitted prior to the first day of school. A new original copy of this form is required to be resubmitted every 2 years.

In the event of a disease outbreak, the Texas Department of State Health Services may mandate that students having exemptions from immunizations be kept out of school until the outbreak is over as determined by the Texas Department of State Health Services.

Exemptions Texas law allows (a) physicians to write medical exemption statements that the vaccine(s) required would be medically harmful or injurious to the health and well-being of the child or household member, and (b) parents/guardians to choose an exemption from immunization require-ments for reasons of conscience, includ-ing a religious belief. The law does not allow parents/guardians to elect an ex-emption simply because of inconvenience (for example, a record is lost or incom-plete and it is too much trouble to go to a physician or clinic to correct the problem). Schools and child-care facilities should maintain an up-to-date list of students with exemptions, so they may be excluded in times of emergency or epidemic declared by the commissioner of public health. Instructions for requesting the official exemption affidavit that must be signed by parents/guardians choosing the exemption for reasons of conscience, including a religious b e l i e f , c a n b e f o u n d a t www.ImmunizationTexas.com under “School & Child-Care.”Original Exemp-tion Affidavit must be completed and submitted to the school or child-care facility. For children claiming medical exemptions, a written statement by the physician must be submitted to the school or child-care facility. Unless it is written in the statement that a lifelong condition exists, the exemp-tion statement is valid for only one year from the date signed by the physician.

Provisional Enrollment All immunizations should be completed by the first date of attendance. The law re-quires that students by fully vaccinated against the specified diseases. A student may be enrolled provisionally if the stu-dent has an immunization record that indi-cates the student has received at least one dose of each specified age-appropriate vaccine required by this rule. To remain enrolled, the student must com-plete the required subsequent doses in each vaccine series on schedule and as rapidly as is medically feasible and pro-vide acceptable evidence of vaccination to the school. A school nurse or school administrator shall review the immuniza-tion status of a provisionally enrolled stu-dent every 30 days to ensure continued compliance in completing the required doses of vaccination. If, at the end of the 30-day period, a student has not received a subsequent dose of vaccine, the student is not in compliance and the school shall exclude the student from school attend-ance until the required dose is adminis-tered. Required Immunizations for the 2016-2017 School Year for Students in Texas Schools, Kindergarten through 12th Grade.

Texas public or private school to another, students who are dependents of active duty military, and students who are home-less can be found in the TAC, Title 25 Health Services, Sections 97.66 and 97.69. Documentation Since many types of personal immuniza-tion records are in use, any document will be acceptable provided a physician or public health personnel has validated it. The month, day, and year that the vac-cination was received must be recorded on all school immunization records creat-ed or updated after September 1, 1991.

Crystal City High School

Crystal City High School will continue to observe a closed campus during lunch and students will not be allowed to leave campus.

ABSENCES/ATTENDANCE Regular school attendance is essential for a student to make the most of his or her education─to benefit from teacher-led and school activities, to build each day’s learning on the previous day’s, and to grow as an individual. Absences from class may result in serious dis-ruption of a student’s mastery of the instructional materials; therefore, the student and parent should make every effort to avoid unnecessary absences. The state laws -one deal-ing with compulsory attendance, the other with attendance for course credit─are of special interest to students and parents.

CLASS SCHEDULES All students are expected to attend school for the entire school day and maintain a class/course schedule to fulfill each period of the day. Exceptions may be made occasion-ally by the campus principal for students in grades 9-12 who meet specific criteria and receive parental consent to enroll in less than a full-day’s schedule.

ACADEMIC PROGRAMS The school counselor provides students and parents information regarding academic programs to

TRANSPORTATION RELATED SAFETY PROCEDURES

Students who need to cross the road to wait for the bus should do so before the bus arrives. Statewide experience shows this is the safest procedure. When the bus brings student home, the bus will wait for the student to cross the road before leaving. The cross-ing must be in front of the bus. Students have been instructed to follow this crossing procedure when they get off the bus: Take ten (10) GIANT steps in front of the bus (out far

enough for the bus driver to see their feet). The driver will establish eye contact with the student,

hold up one finger, point finger to either left or right to start student crossing.

Students will stop at the traffic side of the bus and will look left-right-left and then cross the road.

Students will always cross in front of the bus. Crossing behind the bus will result in a disciplinary action.

Students will not stop or return if any item is dropped. STOP when the bus horn is blown. (the horn will always

mean STOP AT ONCE). This procedure applies to all students both elemen-tary and secondary. Failure to observe safe crossing procedure is subject to disciplinary action, including loss of riding privileges.
