The Oklahoma Association of Health Care Providers is pleased to present our 15th Annual Fall Fair. This
year’s meeting will be held at the Hard Rock Hotel & Casino Tulsa, 777 W. Cherokee Street, Catoosa, OK
74015 on Sept. 8-10, 2014. Our theme is “New Attitudes in Senior Care”. CARE kicks off our Fall Fair
with a golf tournament on Monday, Sept. 8th; our meetings begins on Tuesday, Sept. 9th promptly at
8:30 am and adjourn by 4:30 pm. The program will consist of educational classes for Administrators,
Nurses, & other staff members.
Golf: CARE is hosting the golf tournament Monday, Sept. 8th at 1:30 pm at the
Hard Rock Golf course. Sponsorships and Team Registration are available from CARE
at [email protected] or 405.524.8338. Team of 4 $550 or individual player
$150.
Registration: On-line registration with payment options is available at www.oahcp.org. Registration fee includes up to 12 hours of educational programs, trade show and all designated meal functions.
Registration MUST BE paid by September 2nd. On-line registration closes September 2nd.

Deadline: Deadline for class registration is Tuesday, Sept. 2nd. All registrations received after Sept. 2nd will be charged a $50 per registrant late fee. Cancellation fees will be based on our cancellation policy which is posted at www.oahcp-education. Continuing Education: This educational offering has been submitted but not yet approved by the Na-
tional Continuing Education Review Service (NCERS) of the National Association of Long Term Care Adminis-
trator Board (NAB) for 12.00 participant hours for NHA/RCAL license. This program has also been submitted
but not yet approved for 12 participant hours for Home Care Administrators.
Lodging: We have a link at www.oahcp.org for lodging at the Hard Rock Hotel & Casino Tulsa. Their
toll free number for reservations is 800.760.6700 or locally at 918.3384.7800. Our special room rate is
$110.00 per night; however, you must state you are attending the OAHCP Fall Fair. It is important that
you use our website link or call directly for reservation to make sure we get our block discount. Internet
in sleeping rooms is included for all attendees that book under our designated block. The cut off date for
lodging reservations is Monday, August 25th. Hard Rock’s Buffet breakfast is provided with
lodging; a breakfast buffet ticket will be provided at the hotel check-in.
Tradeshow: This is a one-day only trade show. We have over 60 vendors who will be displaying their
products and services relating to the long term care industry. Trade show is Tues. 7:30 am - 1:30 pm;
continental breakfast and lunch will be provided in the trade show area on Tuesday, Sept. 9th.

This is a keynote performance that brings art, music and the
spoken word together to inspire individuals to achieve fulfillment in

Fundraisers: Our fundraisers are to raise money for the Chartered Bus to Altoona, Iowa fund for the
NAHCA conference in June 2015. OAHCP’s goal is to charter two buses to transport Certified Nurse Aides to
Iowa to represent Oklahoma. There will be tickets sold for drawings for a gift card tree provided by our par-
ticipating vendors. So plan to bring some extra cash.

Tuesday Live Auction: It will be after the opening session; items for bid are gift certificates from the

Norman, a Limited Edition Kevin Durant MBA MVP Basketball, #786 of 2014 and the 6’x6’ original canvas
from our keynote speaker, Richard Hight. You won’t want to miss this opportunity to purchase original art
created by Richard Hight. How special to have an actual canvas hanging in your home or facility that was
created right before your eyes.
Wednesday Raffle: A Gift Card Tree raffle. We will sell raffle tickets for the gift card tree
on Tuesday and Wednesday then draw a winner at the closing session on Wednesday, Sept.
10th but you do NOT have to be present to win. The winner will receive all the gift cards
which were provided by our participating vendors!!
