Using this diary may make it easier for you to manage your type 2 diabetes
Your Ozempic® diary offers suggestions for diet and exercise that you may use along with your therapy.
Helping you manage your type 2 diabetesSpeak with your healthcare professional to help you determine the day of the week that works best for you to take your dose of Ozempic®
Just as a reminder, Ozempic® should be taken once a week on the same day each week, and does not need to be taken with meals.
Here are a few ways to help you remember to take your dose:
It is recommended to note the chosen weekday (e.g., Wednesday) on your Ozempic® carton. You can also write the date on the carton every time you have injected Ozempic®.
Helpful tips for remembering to take your medication
Mark it on your calendar
Set up weekly electronic reminders, such as text or email alerts
Pair it with an activity you enjoy each week
Get help with reminders from a family member or friend
Please see the Patient Information Leaflet for more information or contact your healthcare professional with any questions you may have about type 2 diabetes or your Ozempic® treatment.

Eat smaller, more frequent meals
Drink plenty of water (stay hydrated)
Eat slowly
Avoid fatty food
Like all medicines, Ozempic® may cause side effects
The most common side effects are nausea and diarrhea, but these usually diminish over time. Another very common side effect is low blood sugar when Ozempic® is used with a sulfonylurea or insulin.
If you experience nausea or diarrhea, here are some things that may help:1–3
For the full list of possible side effects, please see the Patient Information Leaflet. If you experience any side effects, speak with your healthcare professional.
If you feel comfortable using a diary format, you may wish to write down information such as your weekly Ozempic® dose, your injection day, your fasting blood glucose as well as your weight and lifestyle goals. Although this is optional, some people find that recording their information in a diary like this helps them and their healthcare professional to see how they are responding to treatment.
Your doctor will start you on a dose of 0.25 mg over the next 4 weeks.

Month 2
You just finished month 2 That went by fast! Have you noticed any changes? Looking ahead to the next 4 weeks, your healthcare professional will decide if you need to increase your dose of Ozempic®. Depending on your needs, your doctor may decide to keep you on the 0.5 mg dose, or move you up to the 1 mg dose if your blood sugar is not controlled enough with the 0.5 mg dose.
Do not change your dose unless your doctor has told you to.

Month 3
Keep it up!
Now is a good time to look back at where you started and reflect on your journey so far. How are your goals coming along? Try to keep things interesting by adding new vegetables into your meals or a new type of exercise. Looking back may inspire you to set new goals as a way to help challenge yourself.
If you would like to make notes, there is space on page 13 of this diary.
