MT SEYMOUR SEASON PASS WAIVER INSTRUCTIONS

THIS FORM MUST BE PRINTED IN COLOUR Our policy holds that all persons must sign a Release of Liability prior to obtaining their season or spring pass, and that parents or legal guardians must sign on behalf of minors under the age of 19. Please note the following conditions with regards to signing the waiver. On one line:

Name: The name of the pass holder.

Date of Birth: The date of birth of the pass holder.

Signature: The signature of the pass holder, or if under the age of 19 the parent or legal guardian.

Current Date: The date the waiver has been signed.

Witness: The signature of a non-related individual over the age of 19 who witnessed the signature of the pass holder or parent or legal guardian.

The pass holder or parent or legal guardian must also initial the box at the top left corner of the waiver, corresponding to the number of the line that you have completed.

Family members with the same surname can be included on the same waiver (please use line 2, 3 etc.). If you have different surnames please use a separate form.
