Mini Med School Verification Form - Online Participation

To receive an electronic Certificate of Participation for Mini Med School, please follow the steps below:

1. Visit Mini Med School online and select a year and series. For example, Mini Med School 2019: Beyond a Cure– Pediatric Cancer Research or Mini Med School 2018: Healthy Starts for A Healthy Life

2. Watch all the sessions in the series online. Please note each year and series has a different number of sessionvideos. For verification purposes, you will only be asked to answer the short question for any 4 of the sessions

3. Complete the form below

4. You and a parent/guardian must either print/sign/scan the form or electronically sign the form

5. Email the completed and signed form to reseduc@bcchr.ca
