An Incentive Points Program allows individuals that participate in the health care plan to earn rewards for participating in a wide variety of health improvement initiatives.
What is the Incentive Points Program?
• 70 percent of diseases are preventable or reversible*. By helping people change unhealthy behaviors, we can improve overall health.
• Behavior change is most effectively created when internal, external and environmental influencers are working together. Incentives are a key external influencer.
• Incentives can provide effective motivation to drive participation in behavior change programs, potentially increasing participation rates by more than 100 percent.
• By participating, you can reduce future risk of health problems and maintain your current health.

Why is an Incentive Points Program Important?
• An Incentive Points Program allows employees to earn valuable rewards for taking steps to improve and/or maintain their health.
• An Incentive Points Program provides an easy-to-use online points tracking system.
Why is an Incentive Points Program Important?
In order to participate in the Incentive Points Program You Must be:
1. A City of Lawrence employee that is a member of the Health Care Plan.
2. Have a Cerner Health Account.

Incentive Points Program Tiers
The CHAMP Wellness Incentive Points Program is composed of 3 tiers. The activities you complete October 1, 2012 thru September 30, 2013 will award your Health Reimbursement Account (HRA) money in January
2014.
Tier 1 - Participation Requirements
All participants must complete Tier 1. No HRA Money will be awarded if Tier 1 is incomplete. Tier 1 includes your:
• Personal Health Assessment (PHA)
• Labs & Biometrics
• First Advisor Visit

Tier 2 - Non-Tobacco Declaration
Must certify that you have been tobacco free for the past 12 months.*
Completion of Tier 2 awards you $150 in HRA Money.

*Tobacco free would include the occasional celebratory tobacco product (i.e. wedding, birth of child, etc…) up to 4 times per year. Tobacco free would not include regular and ongoing (Daily, weekly, monthly) use of tobacco products.
Tier 3 – Wellness Activities
Completion of Tier 3 awards you $150 in HRA Money.
•Points are awarded based on your participation in various wellness activities. You can mix and match combinations of 5, 15, and 30 point activities.
•You will have several options to earn points. Several of the options made available to you will be based on the answers you provided when taking the Personal Health Assessment, so not everyone will have the same point options made available to them. For example, someone who answered on the PHA that they regularly take their medications will not be able to get points for regularly taking medications.
•A maximum of $150 can be earned in Tier 3. Earning < 150 points = $0 HRA money; earning ≥ 150 points = $150 HRA money.
•You do NOT have to complete Tier 2 in order to receive $150 for completion of Tier 3 (or vice versa).

Q: What do I do if my points aren’t adding correctly?
A: Contact the Human Resources division at (785) 832-3203.
Q: Can I participate in the incentive points program if I am a tobacco user?
A: Yes! If you’re a tobacco user, you can complete tier 1 and tier 3 of the program and earn $150 HRA money. You do not have to complete tier 2 (the non-tobacco declaration) to complete tier 3.
Q: Do my points rollover to the following year?
A: No, points expire at the end of the plan year.
Q: How do I prove my BMI, blood pressure, triglycerides, or bad cholesterol has improved in order to receive points?
A: Contact the LMH WellCare Clinic (785-505-3112) to set up an appointment with an advisor. Once your improvement has been verified by the advisor, the WellCare Clinic staff will import your points for you.

Q: What do I do if I forgot my Cerner Health username or password?
A: If you forgot your username, just log in using the email address you used to create the account. If you forgot your password, navigate to http://cernerhealth.com and click on the ’Forgot Password?’ link.
Q: How do I prove what I have reported is accurate?
A: Employees must complete and sign a participation form certifying the points recorded were earned by performing the activities associated with each point value.
Q: Do I have to pay to participate?
A: There is no cost payable to the City of Lawrence to participate in the program. However, some of the activities you can receive points for may have their own individual costs.
Q: Can my spouse and/or dependent participate in this program?
A: No.

Q: Can temporary employees, volunteers, or seasonal employees participate in this program?
A: No.
Q: If I am a new regular employee how do I sign up to participate in the CHAMP Wellness Incentive Points Program?
A: Please contact the City of Lawrence WellCare Clinic by calling 785-505-3112 to schedule an appointment.
Q: Will the information I report through the Incentive Points Program remain confidential?
A: Yes! The WellCare Clinic staff are the administrators of the incentive points program. All medical and wellness staff are employees of Lawrence Memorial Hospital. All medical records are obtained and maintained by Lawrence Memorial Hospital in compliance with HIPAA. Lawrence Memorial Hospital is a medical provider and operates under strict adherence to confidentiality and privacy laws.

Q: Do I have to see a certain medical provider to earn points?
A: Unless specified otherwise, you can earn points for seeing both network and non-network providers. However, we recommend you visit network providers whenever possible as network providers typically cost less. One exception is that to receive points for a second follow-up visit with an advisor, you must see a LMH WellCare clinic advisor.
Q: What is the time frame?
A: The activities you complete October 1, 2012 thru September 30, 2013 will award you HRA money in January 2014.
Q: Do I need to submit proof every time I complete a wellness activity?
A: To receive points for a wellness activity, you need to log that activity on your Incentive Points Program account, (accessed by clicking the WellCare Clinic logo at lmh.org).

Q: My “Wellness Evaluation” activities are only worth one point. Does that mean they earn me $1?
A: No - “wellness evaluation” activities are a requirement for participation in the incentive points program. You will not receive any HRA money if all you do is complete these. Any points that appear to be associated with these program requirements simply serve to visually illustrate on your dashboard when you have completed the requirements.
Q: I already have 150 points in Tier 3, can I continue to log my activities?
A: Yes and we encourage you to do so. However, the money awarded in the incentive points program is the same. Once you have obtained 150 points, the program’s Tier 3 award amount is $150 regardless of amount of points in excess of 150 you log.
Q: I have completed my PHA and biometrics, why doesn’t that progress show on my “Wellness Evaluation” tracking bar on my dashboard?
A: Your “Wellness Evaluation” tracking bar will only be full once you have completed all three participation requirements: PHA, labs and biometrics, and your first advisor visit.

Q: I have special health circumstances that prevent me from completing wellness activities as provided to earn 150 points. How can I participate in the program?
A: We have included a variety of wellness activities such that individuals of all ages, regardless of their health situation, can easily earn 150 points. However, the WellCare Clinic staff can at any time arbitrarily award points for healthy activities based on their discretion. Please contact the WellCare Clinic staff at (785) 505-3112 to arrange for an alternate medically appropriate activity.
Q: Do I need to log my activities as I complete them, or can I just log them all at the same time?
A: We recommend you log your activities as you complete them to increase accuracy of the records you are submitting. Activities need to be completed and logged by September 30, 2013.
Q: How do I know if my fitness program, event, webinar, etc. qualifies to earn points?
A: Please use your best judgment. If you have any specific questions or concerns about an activity’s validity, please contact Human Resources at (785) 832-3203.

on the LMH logo on the right hand side of the screen.

Enter your email or username and password if you already have an account in the section titled “I Already Have an Account.” If you do not have an account enter your email in the “I Need an Account” section to sign
up to participate.

If you already have a Cerner Health Account the following screen will appear. Under the field titled Actions there is a link called SELF-REPORT. Click on the link to self-report
points in each area you wish to enter points in.
Please keep in mind that fraudulent reporting of incentive points constitutes insurance fraud. The City of Lawrence, Kansas would aggressively pursue any fraud under this program as we would with fraudulent filing of health care claims.

Each area that you choose to Self-Report points in will ask you to confirm the date in which you completed the action or
activities listed in that area. Below is what the self-report points screen looks like for the Non-Tobacco Declaration. On
this screen you would enter the date and then click the Report button.

Please make sure to click on and review all of the various areas that you may accumulate points in. Each area is
different, and provides you with various options for reporting activities. For example, when you click on Self Report located
by Preventative Wellness Activities you can select various event types and specific events to report.

In this example Complete Wellness Classes was selected as the event type and the options for various events that can be selected are
show under the Select an Event drop down box.

In the example below the event Attend CPR Certification class was selected. You can see that this event is worth 5 points. You should enter the Date of
the Activity and then click “Report” to report the points.

In order to be eligible for the Wellness Activities incentive you only need to accumulate 150 Wellness Activity points, but you
may self-report in as many areas as you like. Once you complete the 150 points you will see the green bar in Tier 3
filled in all the way.

Tracking your weight or steps can also earn you points. To track your steps, select the “PARTICIPATE” link to the right of
“Start Tracking Your Steps.”
