“Woking & Sam Beare Hospices is a patient-led charity that delivers specialist palliative care to people who have advance life-limiting illnesses, as well as supporting their carers and families.It is my privilege to lead such a wonderful team of healthcare professionals who make up our Community Teams based in Weybridge and Woking. We work closely with GPs and District Nursing Teams from across Spelthorne, West Elmbridge, Runnymede, Surrey Heath and Woking. We provide a co-ordinated approach and are committed to providing care that is tailored to the individual, at the time they need it, but also in the place of their choosing. We know from experience that the majority of our patients want to stay in their own home. Our focus is therefore to work with our community healthcare colleagues to enable patients to stay in their own homes and avoid unnecessary hospital admissions.Please do not hesitate to get in touch should you require our support services for one of your patients and their family.

“CoSI

Coordinated, Safe and Integrated Hospice Care at Home [CoSI] is an award winning palliative care service designed to enable patients across North West Surrey to remain in their preferred place of care, typically in the last 6-8 weeks of their life.Funded by the North West Surrey Clinical Commissioning Group [CCG], CoSI is delivered by Woking & Sam Beare Hospices, with the support of Marie Curie, and the Central Surrey Health (CSH) District Nurses, to provide an additional tier of support, day and night, seven days per week. CoSI also care for patients who do not have specialist palliative care needs.

Referrals to Woking & Sam Beare Hospices

We accept referrals from healthcare professionals who have patients with advanced progressive life-limiting illnesses. All referrals, whether to our community teams or to our in-patient ward, must be made with the relevant medical information and forwarded to Woking & Sam Beare Hospices. Urgent referrals will be triaged and contacted within 24 hours.While the work of Woking & Sam Beare Hospices is not exclusive to North West Surrey, generally we will accept referrals from GPs and other healthcare professionals from across Spelthorne, Runnymede, West Elmbridge and Woking.
