Annual Report from the General Secretary

Projects undertaken by MIOT during the last two years

I am pleased to present the 19th Annual Report of MIOT for the year 2008.

The central committee has met about 8 times since the last AGM. The venue for these meetings and general administration was provided free of charge by Rush Green Medical Centre, Romford.

In 2008 many projects are being supported financially by our organisation.

Primary Care Centre in Jaffna is up and running. Another centre is opened in Moolai Hospital and a medical officer is appointed to run this service. This project is being supported by Central Committee, North Regions & South regions as well.

Medical Students Scholarship fund is being given for Jaffna Medical Faculty every year from 2003 to help the medical students with need for financial support.

Training of Healthcare Assistants at Ariviyalnagar, Institute of Health Science project and continued support is being given by the Central Committee for this project.

Financial support given for the publication of monthly Medical Journal by Jaffna Medical Association until,April2008

In conjunction with Association of Anaesthetists journals are being provided for Jaffna,Batticaloa,Kalmunai,Kilinochi and Trincomalee hospitals by Northern MIOT

Financial support given to the Association of Social Revival & Eradication of Alcoholism in Gurunagar,Jaffna known as, ‘Puthiya Malarkal’ which helps men to overcome the use of Alcohol, drugs and smoking(treatment inclusive) March,2007.

Funds were given to Batticaloa Children Medical Emergency Fund(CEMF) .

Funds allocated by Central MIOT & MIOT Youth Forum to buy Anaesthetic equipments such as Pulse Oximeter and Capnograph to Jaffna Hospital

Funds were sent to Vavuniya Hospital to purchase a Defibrillator/External Pacing for transporting patients with heart block in 2007.

Money sent to purchase an Ultrasound scanner for Medical Unit, at Jaffna Hospital in 2007.

Funds were sent to Mankulam Boys Home for the children in need

Funds were sent to purchase a Paediatric Ventilator to Vavuniya Base Hospital to save the babies at paediatric intensive care unit.

The Central committee had organised one cultural event in September for fund raising which was a success. It was a classical Flute Music programme featured by renowned artists from South India. The main purpose of this cultural event was to raise funds to meet the medical needs of the displaced people in Vanni following the ongoing Civil War.

I would like to thank all the committee members who worked hard with me during this period.

Our aims can only be achieved with your help and continued support as members.

I therefore kindly appeal to all the Tamil Doctors living in UK who are not members to become one and help our charity to go forward and to make our organisation prouder.

Annual Report from the Regional Branches

North Thames Region (MIOT NTR)

Healthcare Projects MIOT NTR has been operating a primary care centre in Chavakachcheri which has been functioning since 2005. Despite the prevailing war situation in the area, the centre has been functioning without a break. The number of patients seen at this centre in the year ended June 2008 rose to 3862 representing an increase of 32% compared to the 2919 patients seen in 2007. The success of this project in meeting its objectives has been instrumental in replicating this primary care facility in Moolai and Jaffna. The Primary care centres in Jaffna and Moolai have been adopted by MIOT - South Thames region (MIOT STR) and MIOT Central. MIOT STR has allocated funding for the Jaffna project for a twelve month period. Medical Education MIOT NTR organised 6 Continuing medical events during the year ended 30 June 2008 amongst the topics covered were, Diabetes care in the community, Quality management of Heart Failure in General Practice, Echo Cardiogram Services in General Practice and Applying Secondary care Guidelines to the Primary care environment of CVD. Fund raising The annual cultural event "tear drops" was held at St Andrews school - Chadwell heath in December 2007. The event raised £955 after expenses. Another £1,700 was raised in donations for the primary care projects in NE Sri Lanka, through members' personal networking activities. Coming Together The North Thames MIOT and the Tamil Lawyers' Association held a combined event "The "Law-Medical cricket match and dinner" to raise funds for an angio-cath laboratory in Jaffna. The event, held in August 2008 raised £1410 from the Medics. MIOT'S share (50%) of the cost of this event (over £1700) was met by a MIOT member. Annual General Meeting 2007 The AGM of MIOT NTR was held on 8 December 2007 with a CME programme / Robin Memorial Lecture

Eastern Region (MIOT ER) The focus of this group is to develop Medical education. Since the successful inaugural meeting on 27th Jan 2007, we had a well attended charity dinner on the 27th October 2007 and another successful study day at Lister hospital on the 27th June 2008. This year the study day was dedicated to ‘Junior doctors Training issues’ and over 100 doctors/medical students attended. Speakers were of exceptional standards and their outstanding presentations were well received by the audience. We thank and congratulate them for their excellent contributions. We also organized and sponsored two final year medical students from homeland to do one month ‘Elective clinical work’ at Lister hospital. We hope to continue this every year and especially in 2009 we are planning to award scholarships to five first year medical students from underprivileged families from homeland.

Chirumiyar Illam (MIOT Home for Girls) Last year was extremely difficult for the Children and Staff due to the political problems in Eastern Region. Colmed 1978 doctors group and 3 other well wishers continued to support financially to maintain this home. The home has been refurbished to an excellent standard with improved facilities such as new toilets, shower rooms, new well and a good playing area. There are regular English, computer & Yoga lessons. The children were taken on a weekend holiday trip in February 07 to Kandy. The home is staffed by a Matron, a Cook and a Watchman. The total running cost of this home is around SLRs.50,000/= per month.

Wales (MIOT WALES) The Committee met twice this year, first on 26th April 2008 at Dr Navaratnams' residence and on 9th August 2008 at Dr Vipulendrans' residence. MIOT-Wales sponsor medical students of Jaffna university, who are identified to be in need of financial assistance, due to their family circumstances in consultation with the Dean of the Medical faculty. £2000 has been ring fenced for this purpose. In order to raise fund for the above sponsorship, we organised a charity walk on the 09th of August 08, at Pembrookshire and collected in total, of £ 1432.50. This event was supported by members of other regions and central committee. Eighteen members from other regions participated in this event. We sponsored Dr Sakunthala Ambikaipakan, at the request and recommendation from Dr Sivapalan, the Dean of Jaffna Medical faculty. We have donated £500 to her to complete her PhD study in India. For next year we plan to have a number of clinical meetings and organise a social evening in Summer 2009. We also hope to repeat the annual get together of all MIOT regions in Wales in Autumn 2009 The next committee meeting is scheduled for 17th January 2009.

South Thames Region (MIOT STR) Healthcare Projects MIOT STR donated £2800 to the Jaffna Hospital development fund in the year ended June 2008. We also adopted the Jaffna Primary Care centre and ear marked £3,300 to meet the staff costs for a twelve month period. We have been paying for the staff at this centre every month since August 2008. The centre offers free medical consultations and patients are given prescriptions to purchase their medication. Charitable Projects We donated £6240 in total to various charitable organisations. Hari Children home in Batticaloa, Thilagavathiyar Home in Vaharai, ORHAN rehabilitation of handicapped in

Vavuniya, Human development centre in Jaffna and Annai Saratha Girls home in Trincomalee were the beneficiaries of these donations. Medical Education MIOT STR have been conducting Continuing medical events every month and are proud to say that we held our 150th event in September 2008. These meetings help medical practitioners to keep abreast of current medical developments as well as fulfilling their evidence requirements for their personal development goals. We also funded scholarships (£720) for medical students from the Eastern and Jaffna Universities. Fund raising / cultural event - muththamil maalai The annual cultural event of MIOT STR - Muththamil Maalai was held on 10th May 2008. The event was filled with classical eastern dance, music and drama as well as popular music and dance programmes. A musical drama "Varalaru Vahutha Vanniyan", in the direction of Dr Mathi Chandrakumar, received a well deserved reception from the audience. The cast consisted of many senior members of STR who together produced a memorable drama, despite being first timers on stage.

Northern Region (MIOT NORTHERN) It has been another productive year from the North, and we are very grateful to its members and well wishers for their continued support. Our activities could be divided into Orphanage support, Medical student scholarships Hospital facilities development and Rural Hospital support. Orphanage support We support the running of the MIOT Thilakavadiyar Mahalir Illam in Vaharai and the MIOT Home for girls in Thambuluvil. The Thilakavadiyar Home has been handed back to us after the cessation of hostilities. The roof had to be repaired and the orphanage had to be re-fenced. Doors and windows had to be fitted and desks and chairs made for the children – by courtesy of MIOT/STR. The orphanage badly needs an electric generator and a building large enough to house 50 girls in the future. We maintain 5 orphaned boys in the Hari Illam in Batticaloa. There was a one off donation to the Boys Home in Mankulam. Hospital facilities development Anaesthetic Dept - Jaffna Hospital We have provided a telephone and internet facilities to the Dept. of Anaesthesia and the trainees are using these facilities to the full by browsing the net. We have set up an e-mail facility for them and they can correspond with the outside world. Renal Dept - Jaffna Hospital We have funded the dialysis programme for patients in chronic renal failure

Medical Student scholarships – Jaffna We support 8 students reading medicine in the University of Jaffna. They are in various stages of their medical course in Jaffna. We handle this project through the Dean Faculty of Medicine, Jaffna Nainamadu Hospital We continue to fund the day to day running of this rural hospital in Vanni. Puttur School medical facilities We have provided first aid and other ancillary medical facilities in this village school.

Annual Report from the Youth Forum (MIOTYF)

The year 2008 has been a busy one for MIOTYF. The year began with the election of a new team, with our new and enthusiastic president, Sujie Pararajasingam who succeeded Thomas Sivapatham. She had the daunting task of ensuring that all our events and projects ran smoothly, as well as working around the busy schedules of many of the committee members.

The first event of the year was the annual Medical Symposium. The symposium involved a weekend of revision lectures aimed at finalists from medical schools throughout London. We are very grateful to those who gave up their weekends to lecture and provide invaluable advice to the students. Special thanks to Miss Shah, Dr Nitkunan, Dr Sritharan, Mr Sooriakumaran and Dr Francis for their continued support. We raised over £1200 and received very positive feedback from our finalists. Generally, students were pleased to have been lectured on topics that are not usually covered well in textbooks and came away feeling that the revision course was very good value for money.

Twilight 2007 aimed to place MIOTYF into the social calendars of students and young professionals throughout the UK. The success of achieving this aim was amply evidenced, when all the tickets for the Twilight 2008 were sold out prior to the event. This year we had the added contribution of our DJ Aravinthan Vetharajah who took an active part in promoting the event too. The event was a huge success as we set sail down the Thames on the “Golden Jubilee” with London’s finest young professionals coming away after having had a fantastic night of fun, laughter and dancing. We raised over £1000 after expenses, which we will be using to fund our forthcoming projects.

For those who wanted to join us on that day but could not do so due to limitations on numbers, we wish to say "join in early so that we can plan big and achieve more".

This year we donated £600 to the Jaffna General Hospital Development Association, which allowed for the purchase of a pulse oximeter to be used in operating theatres and during the transfer of patients between different departments.

We sponsored Imperial College Tamil Society's annual show, Mega Maalai, which involved regular liaisons between the two committees. This has greatly strengthened our links with the Tamil student body in London.

- Developing new ideas for fundraising - Further developing the MIOTYF social network, both through MIOT and future

Sri Lanka - Providing an opportunity for young medical professionals to present their latest

research ventures

Annual Report from Dental Focus Group

I wish to draw your kind attention to the previous years report which has clearly shown what this group has done since 2003. However, we regret, that we are in no position to show any new progress for the year2008. A year with many sad events and stories that has unfolded in our homeland and within our membership. Colleagues have spent their time and energy to find ways and means to help alleviate the suffering of our kith and kin back home and was not prepared to spend time on developing the Dental focus group during this year. We hope you would remember the International centre in Calais France was inaugurated just the day after our last AGM. We had plans to start the Dental practise in April2008. However, due to the delay on the building works and the necessary completion on rewiring and modernising the electrical system, has hampered our plans to start the dental practise on time. New dental equipment was necessary to replace the old ones.there is some delay due to shortage of funds. We hope with the dawn of 2009,we could accelerate our plans to start the Dental practise and the resource centre. We are hoping for new enthusiasm among our membership, and to have a drive to recruit more willing members from the first and second generation of Tamils. These are a few agend on our the card for next year.

Report from the Family Medicine Focus Group

The Chavakachcheri primary care centre has been in operation now for three years. Despite the prevailing situation in Sri Lanka it has continued to provide uninterrupted service to the local community. The centre saw just under 4,000 patients in the year ended June 2008. The centre is supported by MIOT NTR.

The group has seen through opening of two more primary care centres in Moolai and Jaffna. Moolai centre is supported by the central MIOT and Jaffna by MIOT STR.

Institute of Family Medicine (IOFM) has been registered as a general practice in Jaffna.

We are in the process of obtaining WONCA (World Organization of National Colleges, Academies and Academic Associations of General Practitioners/Family Physicians) registration for IOFM as an academic institution. Wonca is made up of national colleges, academies or organizations concerned with the academic aspects of general family practice. Beginning with 18 members in 1972, there are now 97 member organizations in 79 countries. This includes eight organizations in collaborative relations with Wonca. In all, the total membership of the member organizations of Wonca is over 200,000 general practitioners/family physicians.

Dr S Jothimurugan & Dr S Poologanathan presented a paper on "Developing Family Medicine in Developing Nations" at the WONCA European convention held in Istanbul in September 2008. Dr Rajan Kailainathan presented a paper on diabetic screening programme at the WONCA Asia Pacific regional conference in October 2008.

IOFM intends to support presentation of research papers which are of academic excellence.

I am presenting the MIOT Annual Financial Report for the Fifth consecutive year.

All income and expenditures are shown in the Auditor’s Report. As of the 30th June 2008, MIOT had three bank accounts:

Halifax Fixed Account - £11,000.00

Money present in the Halifax Fixed Account is from the subscription fees of MIOT Life Members. The interest from this, £621.50 for year 2007/08, will be utilised to provide scholarships for medical students at Jaffna University.

I thank all the members of the regions and the central committee who had worked hard with our Auditor Mr Lingham in getting the financial report just in time to go to print.

Out of the 393 members, only 189 (48%) are paying the correct subscription. There are 40 life members from whom no subscription is due. 28 members have not paid the 2008 subscription. Some members are still paying at the outdated rates ranging from £15.00 to £35.00. Request letters are posted to remind these members. All efforts have been made to get correct subscriptions without much success. This constitutes a loss of £3855 through subscription

There have been considerable efforts made by only a few members to bring new members, especially the young members, into MIOT without much success. If recruiting continues as it is, we will have a severe membership crisis in few years time.

We have collected £3786.54 from the Inland Revenue, by Tax Relief, for the year of 2006-07. Some of the members still have not returned Gift Aid forms. We will be posting new Gift Aid forms to those members, who have not yet returned one. Please be kind enough to fill them in and return them.

During this financial year, we have not sold any copies of the “Alumni of Peradeniya Medical School”; there are still copies available, for those of you interested in purchasing a copy.

This year membership stands at 393. Today there are more than 300 Medical and Dental Sri Lankan British students studying in UK universities. In the MIOT Youth Forum not more than 25 members enrolled. UK is the second largest Tamil doctor population in the world though we have such a low membership with very few new young doctors. As you all can see in 10 years time this organisation will struggle to exist once we all retire from service unless we actively recruit new younger members and involve them in to our central and regional committees. I hope to see more new younger members in the next AGM.

Bringing new members into MIOT and the Youth Forum have a two-fold benefit.

Firstly, more members will lead to a stronger organisation, leading to more money for all of the causes in Sri Lankan for years to come. Secondly, this is a perfect way for our sons & daughters to meet others to exchange views and ideas.

I am requesting all MIOT members who have children, to encourage their children to get involved in this organisation which we all are very proud of.

Special Resolution to Amend the Constitution

Dear Colleagues, I wish to introduce this resolution on behalf of my working party colleagues who were entrusted the job of establishing such an Institute of Family medicine under the umbrella of MIOT in 2003. The tremendous work had been carried out by this working party in a short period. This would be illustrated today. As to consolidate these efforts and to achieve our goal of building a resource centre named IoFM. We present to this house assembled today at this AGM to adopt this resolution for it to be included in our statue book , that is on to our MIOT constitution.

With the changing need, circumstances and the demand for funding to cover all our educational projects and commitments we have already made to support our community in our homeland, we are also made to look elsewhere, other than from our own pockets. Few entrepreneurs among us have also been supporting these projects financially. Having taken all accounts on a global dimension we are forced to look around for funding. we have good opportunity to get funds from WONKA and other institutions which are prepared to fund projects based on the Family practise that includes rural medical and urban medical set ups. we could only get help from these institutions if we name our medical teaching and research resource centre as the Institute of Family Medicine.

The following amendment to the constitution of MIOT is proposed by a special resolution at the AGM on 15 November 2008.

To amend clause 4.1 of the constitution of the Medical Institute of Tamils as follows.

New Clauses

4.1 Regional and Functional Divisions

4.1.1 There shall be a Central Office, with a number of regional Offices as deemed necessary. Each regional Office shall have an executive standing committee. The regional offices have to be approved by the Management Committee

4.1.2 There shall be an educational wing of the Institute named the "Institute of Family Medicine" hereinafter referred to as IoFM. All educational activities of the institute shall be conducted under the banner "Institute of Family Medicine".

The above clauses replace the following.

The trustees present their report along with the financial statements of the charity for the year ended 30 June 2008. The financial statements have been prepared in accordance with the accounting policies set out in the notes to the Financial Statements and comply with the applicable law.

Aims & Objectives

Medical Institute of Tamils Charity is a registered charity. Its objectives are,

To relieve poverty, sickness, and preservation and protection of health of the Tamil community in the North & East of Sri Lanka (Homelands), in the UK and elsewhere.

To advance the education and training in the Tamil community in the science and art of medicine. To provide facilities in the interest of social welfare for recreational and other leisure-time

occupation, primarily for persons within the Tamil community with the object of improving their quality of life.

The charity aims to harness the extensive network of Tamil medical professionals working across the whole of the United Kingdom through its regional branches Northern, Wales, Eastern, North Thames and South East Thames regions and also by collaborating with like organisations operating in the rest of the world to achieve its objectives.

Financial Review

Income

Total income generated for the year ended 30 June 2008 was £78 186 (2007 - £84 776). The northern region, which has been at the forefront in major rehabilitation projects, has continued with them and secured further sums this year too. Each region has contributed in different ways in generating funds. MIOT youth forum, consisting of younger generation across all regions, continue to grow. The income is shown under "Central" which is the administration cost centre encompassing all regions.

The above amounts include funds generated in prior years but utilised in this financial year. The charity is managed entirely by volunteers. The trustees provide their services voluntarily and do not derive any financial benefit.

Risk management & reserve policy

The North & East of Sri Lanka, the area in which this charity's beneficiaries live has become inaccessible due to the escalation of the conflict. The volunteers who are working on the ground have not been able to carry out their tasks and the distribution of aid has become hazardous since December 2005. The statuses of the various projects are being monitored remotely with e-mail correspondence.

Trustees will ensure that both central committee and regional committees raise additional funds through fund raising activities such as cultural programmes and charity dinners to avoid incurring financial deficits.

The trustees review the major risk which the charity faces on a regular basis and believe, in maintaining reserves at current levels with constant review of controls over financial operations, will provide sufficient resources in the event of adverse conditions. The following resources are available to meet future requirements.
