Counseling and Preference form filling Procedure Information and Instructions for Candidates

Introduction The Government of Maharashtra has decided to recruit competent and efficient staff

nurses on the basis of merit through competitive written examination. This competitive

written examination was meant for recruitment of competent candidates for post of staff

nurses and to appoint selected G.N.M./B.Sc.(Nursing trained) nurses in the hospitals under

establishment of Directorate of Medical Education & Research, Mumbai. The examination

was held on 22/09/2013 and final result was declared on 25/09/2013.

Recruitment of the Staff Nurses for institutions / hospitals under the establishment

of Directorate of Medical Education & Research, Mumbai will be done by Counseling and

filling of the "preference Form" at four Regional Centers based on the State Merit List

declared on 25/09/2013. The preferences given by the candidates at the regional centers

will be centrally processed at DMER office and list of selected candidates will be declared on

web site. The post of reserved category candidates (Constitutional & Parallel) will be filled as

per the present backlog as shown on page no. 27of broacher meant for MHN‐CWT‐2013

(Annexure‐ R).

There are 1489 clear vacancies in different institutions / hospitals under

establishment of Directorate of Medical Education & Research, Mumbai and Director of

Ayurved, Mumbai. More vacancies are also anticipated due to retirement/other reasons. All

eligible candidates from respective category will be called for the filling of preference form.

The clear vacancies as specified earlier will be filled through this process. The posts failing

vacant subsequently due to any reason whatsoever will be filled later on by candidates from

respective categories merit wise.

The selected candidates will have to pay Rs. 1500/‐ (Rs. One thousand five hundred

only) as administrative charges incurred for post examination process through Demand

Draft in the favour of "DMER, Mumbai" payable at Mumbai. The Demand Draft should be

submitted at the office of Dean/Head of the institution at the time of joining as a staff nurse

in the selected Hospital/College.

The four centers for filling the preference form are Sir J.J. Group of Hospitals,

Mumbai, Sassoon General Hospital, Pune, Govt. Medical College & Hospital, Aurangabad

and Govt. Medical College & Hospital, Nagpur. The Candidate should report to the centre

given in Page No. 02 Table No. 01 as per their State Merit List (SML) No. as per Schedule, for

the counseling and filling up of the "Preference Form".

brochure of counseling/ preference system. The candidates are requested to go through

this before filling preference form.

The preference form duly filled by the candidate shall be scanned and the data thus

generated shall be given to the candidate for verification of the choices of place of

recruitment exercised by him/her. After verification the candidate will report discrepancy if

any in the data and it will be duly corrected according to his/her preference form. If the

candidate does not report at the verification counter, it will be presumed that data entered

against his/her name is correct and subsequently, corrections will not be made at later date

in any case.

The choices of place of recruitment, once entered in the record/data, after

verification by the candidates, shall be final and irrevocable. This final data shall be used

in entire selection process for the recruitment of the Staff Nurses in the

GENERAL INSTRUCTIONS TO CANDIDATES FOR FILLING PREFERENCE FORM

1) Please check the schedule for filling the preference form. Report to the office of

Regional Centers according to the State Merit List (SML) No, on the scheduled date

and time along with relevant original documents and a set of attested photocopies

of each document.

2) Collect scrutiny form and fill all the details as per the instructions. Submit scrutiny

form along with all the original documents and one set of attested photo copies to

the scrutiny officer. Scrutiny officer will verify all the documents and ascertain the

eligibility of the candidate. The scrutiny officer will also verify the claims of

constitutional and specified reservation. The scrutiny officer shall endorse the

eligibility on the scrutiny form.

3) All the eligible candidates will collect the Original preference form from the window

meant for this purpose. If the candidate is not eligible for the recruitment according

to the rules stated in the information brochure, the Original preference form shall

not be issued. The Original preference form will be pre‐printed along with the

relevant information of the candidate, which he/she has filled in his/her original

application form along with his/her Roll number and his/her State Merit list Number.

A copy of working preference form is enclosed in this booklet (Page No.13) the

candidate should first fill up working Preference Form with Pencil. No duplicate /

second Original preference form shall be issued.

4) Read carefully all the instructions to fill the form before you start filling the Original

preference form. First, Candidate must fill the working preference form by using

pencil (Page No. 13). Please refer the code number of Hospital / Institution given on

the page number‐10 for filling the working preference form.

5) Candidate must fill following information in the original preference form.

a) Year of passing of G.N.M./B.Sc. Nursing final examination.


b) If candidate is Project Affected person he/she should have appropriate certificate

in the prescribed format and should darken 'Yes' bubble at item No. 08

c) Write institution code according to the most preferred place of recruitment at

preference No. 1 followed by the next choice, at preference No. 2 and so on.

[Once the candidate is satisfied with his choices filled, on the working preference

form, he will fill the same using black ballpoint pen. Then he should start filling it

on the original preference form, using Black ball point pen. Candidate must be

very careful while filling up the Original preference form. Under any

circumstances he/she will not get another Original preference form. Candidate

should fill all the preferences. If a candidate does not wish to fill all the

preferences then he has to sign an undertaking stating the following.

"I understand that I am required to fill all the '38' preferences and that the

allotment is being done merit wise within the respective categories. I also

understand that if I do not give all the preferences, I may not get the

posting and I owe the responsibility for the same."

6) Fill the Original preference form as per instructions and submit it at the specified

counter. Candidate shall inform regarding the name correction if any at the counter.

7) Collect your data sheet, verify the entries, and in case you find any discrepancy

please report immediately to the verification counter. If the candidate does not

report at the verification counter, it will be presumed that the data entered against

his/her name is correct and subsequently corrections will not be made at later date.

The choices of places, once entered in the record/data, after verification by the

candidate, shall be final and irrevocable. This final data shall be used in entire

selection process for the recruitment.

8) A reserved category candidate should refer annexure‐E given in the MHN‐CWT‐2013 brochure He/She should submit. a. Caste Certificate & Caste validity certificate.(CVC)

b. All the reserved category candidate (except‐S.T.) should submit as per the G.R.

Dated 12th December,2011 caste validity certificate within 6 months from the

date of selection.

c. Candidates belonging to S.T. Category should submit Caste Validity Certificate

invariably during this process for Selection.

d. Non creamy Layer certificate valid upto 31/03/2014 issued after 31/03/2013 in the

name of parent of the candidate wherever required (necessary for V.J., NT(1),

NT(2), NT(3), OBC & SBC)


9) All the documents shown & submitted by the candidate during this preference form

filling process will be considered for selection of the candidate. Any document

shown after this process will not be accepted. All the candidates have to bring all

the documents mentioned above during the process. Non Creamy layer certificate is

required & if failing to produce, the said candidate will be included in the Open

category.

10) This directorate has conducted the MHN‐CWT‐2013 and declared the result on

25/09/2013. All the candidates have been issued the mark sheets on which state

merit list number is mentioned. Candidates will have to refer rule no. 5 given in the

information brochure.

11) In the mark list of reserved category candidate along with state merit list number

category merit list number is also mentioned. All the candidates should note, that

they should not attend this process as per the reserved category merit list no.

12) Candidate should refer Rule No.4.8 given in the MHN‐CWT‐2013 information

brochure for the eligibility related to age. Those who will be fulfilling these

conditions will be considered for the process.

13) If a candidate remains absent for this Counseling/ preference form filling process

then he/she will not be considered for the appointment. No application/request will

be entertained afterwards.
