June 2017 WELCOME TO OUR PRACTICE The practice as a team strives to maintain a high quality of service to our patients and we at all times welcome and encourage comments. Our surgery building is welcoming and appropriate to the needs of all users including the disabled. Our practice area extends from Llanon to the South, Tre’rddol to the North and Ponterwyd to the East. Our team includes GPs, two Practice Nurses, a Health Care Assistant, two District Nurses, a Health Visitor as well as our Practice Manager, reception and administration staff.

We offer a full general practice service and we run nurse led specialist clinics in Asthma, Diabetes and Coronary Heart Disease. We aim to treat all our patients promptly, courteously and in complete confidence. The receptionists and other practice staff are here to help you and anything you tell them is treated in absolute confidence. We try to have a Welsh speaking member of staff on duty at all times. If you have any doubts please speak to a receptionist. Simply call in to the surgery to fill out a registration form and a new patient questionnaire if you wish to register as a patient.

If you would like the Practice Leaflet in large print please ask at Reception. PERSONAL INFORMATION Please notify the surgery of changes to name, address or telephone number. If you move outside the Practice boundary area, you should register with a more local doctor. OPENING HOURS Padarn Surgery is open from 8.30 to 6.30pm weekdays. For patients wishing to make appointments, collect prescriptions etc, the receptionists are available

between the hours of 8.30am and 6pm. ACCESSING HEALTH CARE APPOINTMENTS Appointments can be made either by calling at the reception desk or by telephoning the surgery between 8.00am and 6.30pm Monday to Friday. Appointments are available Monday to Friday 8.30 am – 6.30 pm. The morning surgery commences at 8.30am or 9am and the afternoon surgery is from 1.30pm or 2pm with an evening surgery from 4pm. If you wish to see a particular doctor please inform the receptionist when booking the appointment but you

may have to wait longer for your consultation than if you are happy to see any GP. Please book a separate appointment for each member of the family that is coming. If you need an urgent appointment, please make this clear to the receptionist and she will ensure that you are seen with a minimum of delay. Appointments are for ten minutes, however, if you feel you need to discuss something at greater length please let the receptionist know and she will arrange a longer appointment. Please cancel your appointment if you are unable to attend.

We offer: Same day/emergency appointments These can be booked from 8am and should be booked if you need to see a GP that day. Although there is a choice of doctors we cannot always guarantee the doctor requested. Pre-booked appointments You can book an appointment with a doctor of choice up to 3 weeks in advance. However it is important to note that the GPs are sometimes away, which will affect the number of appointments available.

TELEPHONE AVAILABILITY/ADVICE A time has been set aside for patients to speak to Doctors/Nurses after morning surgery. Please request this service before 11am if possible. Requests for telephone advice will be triaged by the on call duty doctor. HOME VISITS Home visits during surgery hours are only available to the housebound and terminally ill and carried out at the discretion of the doctor on call when clinically appropriate.

REPEAT PRESCRIPTIONS If you take medication regularly, repeat prescriptions, which are subject to appropriate review may be obtained by either by leaving your request at reception, posting or faxing it or alternatively you can use our new online prescription ordering service. Please ask for information and a registration form at Reception. We are unable to accept telephone requests. In all cases please allow 48 hours for preparation and signing of your prescription before collecting. TEST RESULTS Please telephone between 2 - 4pm for results of x-rays, laboratory tests or smear tests. Please allow at least a week before enquiring

about a test result. Remember that the results of tests can only be given to you as the patient, unless we have your prior permission.
SERVICES AVAILABLE
NEW PATIENT HEALTH CHECK All new patients may be asked to see a member of the practice team when registering for a simple health check. This enables us to obtain details of your past medical problems and to provide better care. CHILD IMMUNISATIONS A baby clinic is held every week at the surgery where the Health Visitor

does routine child immunisations as well as development assessments. TRAVEL CLINICS These are given as and when indicated. A fee may be payable. You will be informed of this when you see the nurse. CERVICAL SMEARS We follow a policy of recommending a repeat smear test every three years until the age of 65 unless it has been recommended otherwise. Usually the practice nurse will do this test at a Well Woman Clinic.

CONTRACEPTION All the doctors provide advice on all forms of contraception and family planning. Special clinic times are available for the fitting of coils. Oral and emergency contraception is available from all doctors. Contraceptive injections are also given by arrangement. SMOKING CESSATION Run by our Practice Nurse, this clinic gives advice and support to patients trying to give up smoking. NON NHS EXAMINATIONS Please note that some medical examinations are not included on the NHS; e.g. Insurance, HGV and

sports medicals. Please ask to arrange an appointment. A fee as recommended by the BMA may be payable for these and for any letters or reports not covered under the NHS and these fees are displayed in our reception area. SPECIALIST CLINICS There are nurse led clinics for asthma, chronic heart disease and diabetes. We encourage all patients to attend these clinics on a regular basis. IN ADDITION There is a practice based counsellor and team of community nurses who all work closely with the doctors.

OTHER INFORMATION
TRAINING PRACTICE As a training practice we have doctors who join us for a six month placement. From time to time a medical student from the University of Wales Cardiff may accompany one of the doctors. Your help in allowing them to be present in a consultation would be much appreciated. You will always be asked if this is acceptable to you. COMMENTS, SUGGESTIONS AND COMPLAINTS The practice has an in-house complaints procedure. If you have a complaint or concern please contact the practice manager. We will try our best to resolve matters

as quickly as possible. You have a right to expect a high standard of medical care and we will try at all times to provide this within the resources available. CONFIDENTIALITY We respect your right to privacy and your personal information is treated in the strictest confidence. It is important that the NHS keeps accurate and up to date records about your health and treatment so that those treating you can give you the best possible advice and care. This information is only available to those involved in your care. Only the minimum amount of information needed will be passed on. Confidential patient data may be shared within the practice health care team and with other health

care professionals to whom you are referred for care. It may also be required for the broader purposes of public health, audit and research. Data disclosed is anonymised before disclosure. You have the right to know what information we hold about you. ACCESSING MEDICAL RECORDS The practice manager will assist any patient wishing to have access to their own medical record subject to the limitations of the law.

VIOLENT PATIENTS – ZERO TOLERANCE We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. We take seriously any threatening, abusive or violent behaviour. If a patient is violent or abusive, they will be warned to stop their behaviour. If they persist, or an incident is serious, we may exercise our right to take action to have them removed, immediately if necessary from our list of patients. OUT OF HOURS The Local Health Board is responsible for commissioning this service. When the surgery is closed a doctor from the Out of Hours

service is always available (including nights, weekends and Bank Holidays) for visits/advice and emergency care. This service can be contacted via the following number:

HOW YOU CAN HELP US We ask you to use the service we provide in a responsible way. In particular we would ask that:
If you have an appointment, please keep it or give adequate notice if you wish to cancel.
Doctors’ and nurses’ time is limited and they have many patients to see. Please do not

delay us without good reason and be aware that there are other patients who need to consult as well. There are however, occasions when you or another patient will need a longer consultation. Sometimes this will delay your consultation. Please take this into account when you are waiting, because you may need a longer consultation at some time.
We can see more patients in surgery than when home visiting. Please come to the surgery whenever possible rather than requesting a visit. You may of course request a visit if serious illness or infirmity prevents you from attending the surgery. We ask our staff to treat you with courtesy and

friendliness, and would ask you to treat our staff in the same way. It is not their fault if doctors are delayed.
ACCESS FOR THE DISABLED There are full facilities within the surgery including a specially equipped toilet and a small waiting area for wheelchair bound patients. The reception area has an induction loop system for patients with hearing aids. Please let the receptionist know if you wish to use this service. There are designated disabled parking spaces on site, which are clearly marked.

CARERS Do you look after an ill or disabled relative, friend or neighbour at home? Please tell any member of staff that you are a carer as we may be able to offer extra help and advice. Young carers are children and young people up to the age of 18, who look after someone in their family who has an illness, a disability, or is affected by mental ill-health or substance misuse. Anyone who provides care and is aged between 18 -25 is considered to be a young adult carer.

FREEDOM OF INFORMATION ACT From 1 January 2005 you can ask for access to general information held by the practice. Your personal health information will continue to come under the Data Protection Act 1998. This Act aims to promote greater openness and accontability across the public sector, including the NHS and independent contractors i.e. General Practitioners. GPs are legally obliged to adopt and maintain a publication scheme. Please ask the Practice Manager for Padarn Surgery’s publication scheme, which is based on the model suggested by the Welsh Assembly Government.
