Favorite Memory of Residency: One fall BMTU day, treating a very, very sick
patient in the Unit with Destry as my co-senior.
I recall having to urgently place a dialysis line at
approximately 615 am shortly after sign out.
Both Destry and I were in the room working
together to place the line quickly. Destry
attempted the line without success and I
aggressively told him to step aside to "expedite
the process" and I promptly accessed the
carotid. Destry politely looked on as I held
pressure for 45 minutes. During this moment,
we were both terrified, but looking back, it was
hilarious and a quintessential Laya-Destry ICU-
esque moment.

Favorite Memory of Residency: MICU nights as second year. Nick was intern. An 85 year old with foot
osteomyelitis crashed on the floor. Ended up with MAP in the 30s for a
while, I put in emergent central line while he was obtunded. Nick got ready
to set up for an A line when his mental status suddenly recovered (but
delirious). He started trying to hit Nick yelling, "I don't want any needles you
SOB!" Each time Nick would try to hold his arm he would get cussed out
and grabbed. Nick was at a loss. I was bringing him supplies and while seeing
that I lost it and started rolling around on the floor outside his room in the
SICU. The nurses thought I was having a seizure and checked to see if I was
ok. Then we finally got a hold of his only family (elder brother "Ted") to see
if he consented but Ted was pretty senile. Every time we explained about his
BP he was like, "whaaaaaaaaa? His blood pressure is high? Well get it down
then!". In the background we could hear his wife screaming, "noooooo Ted
they are saying he has LOW blood pressure!". He kept on going on tangents
so we ended up having to do the lines emergently anyway.

There are too many to name! I would say
intern year when I went trick or treating
throughout the hospital on Halloween
during Loyola MICU rotation while my
seniors, Mark Tancredi and Matt Sugimoto,
ran the MICU. They didn't seem to notice I
had disappeared for some time.

Favorite Memory of Residency: Patient care. At Loyola, we are so fortunate to
have the opportunity to care for a variety of
complex, high-acuity patients of diverse
backgrounds. The most memorable cases are
those which present challenging diagnostic and
management dilemmas. Even more
memorable though, were those times when
I was able to connect with my patients on a
personal level and to share in their
happiness when we achieved favorable
outcomes. I will also cherish all the
friendships I have made over the past three
years among my co-residents, medical
students, and attendings.

Favorite Memory of Residency: I had this patient on gen med, old guy who was in a slow process of dying.
Headed towards sub- acute rehab type of thing, all acute things resolved
pretty quickly. We'd talk to his kids from time to time, and they'd
alternate visiting/staying with him at his home (needed 24 hour care), but
he also still had his wife who was always with him in the hospital. She was
a quirky woman who was a Round Killing Talker, often with some repeats
of the stories she already told. So I would tend to cut her off in the
morning and tell her I'd come back on the afternoon to chat.
So I'm back in the afternoon and she pulls me into the hallway (I thought
pretty unnecessary as the patient wasn't processing much) and starts
strolling the halls with me. We walk to the main area on 3 then turn
around and go back to the end of the hall. She's been talking the whole
time, and then she gets to why she's walking with me. She wants to tell me
one of her favorite poems, and so she did:
I walked a mile with pleasure. We chatted all the way. But never did I learn a thing for all she had to say.
I walked a mile with sorrow. Never a word said she. But oh the things I learned from her that mile she walked with me.
I found that poem pertinent and powerful. I wrote it down right then while
walking with her. It applies so deeply for hospitalized people, who often
sit in silence due to illness, weakness, fear. And their family is often right
beside them, joining them in that silence, but still sharing the experience
and learning about their loved one. I've tried to keep it in mind for my
own experiences. It was a poem I thought about a lot while hanging in the
hospital with my dad, and I read it aloud at his funeral.
We are fortunate to work in a field that has impact on others and where life
changing events occur. They're not always the acute traumas, they can be
occur over a period of time. On reflection, even those illnesses whose
course takes years to play out, feel like a singular event, an era of one's life
that changes you.
