OUT OF SEASON CONDITIONING

• Each program has identified strength, speed, agility, conditioning, sport specific program.

REQUIRED FORMS

• PHYSICALS– Must be completed on IHSAA physical form before

athlete can practice (4 pages)– Must be done after April 1st to apply for following

year– Physicals will be given at LHS May 26th.

– Physicals must be signed by athlete and parent in two separate places. Doctor signature ONLY!

Athletic Fee

• A fee per athlete per season cost is used to cover athletic expenses, (Booster Club/Corporate sponsor assistance has reduced this fee) through D4D program. Cost of driver and $1.50 per mile, supervision, and athletic trainer, and supplies are deferred through this fee.

• Athletes must ride the school corporation transportation to and from athletic events.

• Travel release document (timeline).• Fees can be made by credit card at

preseason meeting

• Expenses- Officials, uniforms, warm ups, balls, transportation, security, contest workers,

awards, special recognition programs, medical supplies, safety equipment,

training equipment, variety of sports specific equipment, facility improvements

Important Date for Athletic Physical Night

Physicals will be held on Tuesday, May 27th starting at 4:45PM.– The athletic office of Lowell High School is accepting
