1 INTRODUCTION

The following strategy details the Traffic Management Plan (TMP) for the Lismore Base Hospital Stage 3B Main Works Redevelopment. The strategy highlights the planned access arrangements for construction vehicles and pedestrians. Ultimately the TMP has been designed to minimise the potential impact from increased construction vehicles on pedestrian and traffic on surrounding roads (namely Uralba Street, Little Uralba Street and Fermoy Avenue). To aid this, appropriate kerbside signage and provisions will be implemented as required to ensure ongoing operational safety and efficiency during the project.

JH notes that the TMP is a live document, and as required will be renewed and updated throughout the project.

1.1 Project Description

Health Infrastructure has appointed John Holland Pty Ltd (JH) under a GC21 Edition 2 Contract (Head Contract), to perform the design, construction, commissioning and completion of Lismore Base Hospital Stage 3B Redevelopment – Main Works.

For clarity, the scope of this plan includes the North Tower scope including the lower floors from Level 3 to Level 7 (Development Consent SSD 6848 dated 1st May 2015) and the North Tower Extension from Level 7 to Level 10 (Development Consent SSD 17_8963). The TMP for South Tower and Block A demolition is documented in the previous revision, namely LBH-PP-JH-Y-0007-03.

The major project stakeholders are:

- Health Infrastructure (HI) – Project Principal

- CBRE – The Principal’s Project Management company for the project, engaged by HI

- Lismore Base Hospital – Staff, users and visitors including the Local Health District Representatives (LHD)

- Local Indigenous Community

- Local and surrounding community and residents – Benefit through construction and at completion with the

new and refurbished Departments

- Local Authorities – Including Lismore City Council, Essential Energy, Rous Water, Telstra, NSW Fire &

Rescue, Northern Rivers Buslines, Elgas, etc.

- JH as Design Finalisation and Construction Contractor – Engaged under ECI Agreement and Early Works

GC21 Contract with HI

- Conrad Gargett – Engaged by JH as Architect

- All services and other building element design consultants – Engaged by JH for design of all services and

elements for the building

- Building Subcontractors – Engaged by JH to construct the works

- Local suppliers and companies – Direct benefit from the project

All contractors engaged will comply with JH systems and requirements for Safety (including Workplace Risk Assessment, Activity Method Statements, Task Risk Assessment and Start Cards as a minimum), Quality, Design and Environment.

1.2 Site

The Lismore Base Hospital is located at 60 Uralba Street, Lismore, NSW, 2480.

The site is approximately 5000 sqm with site access along Little Uralba Street which is a steep one-way residential street and adjacent to the new Ambulance entry bay. The site is heavily sloping and is constrained by adjacent hospital buildings to all building perimeters and private residencies along Little Uralba Street. Three properties along Little Uralba Street will be used as site amenities, deliveries and material laydown area for the construction works.

The site is part of Lismore Base Hospital precinct and the construction site is co‐located with the operational Public Hospital. The Contractors site will be surrounded by other existing operational hospital buildings.

1.3 Access

JH will put in place a number of key initiatives to control traffic flow onsite. These will include but are not limited to the

following:

- Use of the properties (numbers 9,11,15) on Little Uralba Street for site sheds, amenities, loading/unloading

zones and materials laydown zone to feed the project;

- Develop site layout plan to demarcate site access, sheds, worker pedestrian areas, material laydown areas to

assist with management of traffic within site compound;

- Regular consultation and coordination of works with LHD (Local Health District) representative, in particular

keeping them fully informed of layout and any changes to the site access and pedestrian pathways;

- Traffic Management controls within Little Uralba Street site compound (i.e. signage, 5km/h speed limit,

barriers between plant and people, etc.);

- Design and documentation of compliant traffic control plans (TCP’s) to define the control measures required to

be implemented to manage the construction traffic and pedestrians. A third-party specialist will be engaged to

develop the TCP’s.

- Implementation of third-party designed and certified traffic control plans (TCP) for all external road

entrances/exits and the pedestrian pathways around the site (see Appendix 2 for the proposed TCP’s for the

project);

- Engage accredited, qualified and competent traffic controllers to manage traffic and pedestrians as required

by approved TCP’s to ensure safety of staff, the public and motorists;

- Engagement and consultation with Lismore City Council (LCC) and relevant neighbours on the proposed

traffic control plans (TCP);

- Construction car parking: It is acknowledged that no construction parking on or within one block of the hospital

campus is allowed;

- Encourage car‐pooling/sharing among site workers where possible;

- No right turn for normal vehicles (Ambulances are excluded) off Uralba Street into Little Uralba Street when

approaching from Ballina (i.e. no right turn into site from the eastern approach);

- Planning of large material deliveries to minimise impact;

- Utilising school holidays for large deliveries where possible;

- Permits for oversized vehicles and times of operation on the external road network as required by regulations

and legislation;

- Stabilisation of site access routes to prevent dust creation;

- Controlled and monitored entrances to the Work Site to prevent unauthorised and public vehicle entry;

- Ongoing monitoring and review in conjunction with various stakeholders and LCC on the traffic controls and

measures implemented throughout the project.

2 OBJECTIVES

2.1 Objectives Overview

The TMP has been designed to effectively and proactively manage traffic conditions coming on to and moving within

the Lismore Base Hospital Stage 3B project site. This includes management of onsite vehicles and deliveries,

construction parking and the associated impacts on surrounding roads and pedestrians.

Specific objectives include:

- To ensure all pedestrian access is maintained at all times. In event works impede or impact on pedestrian

walkways, develop a management plan including, as required, signs, personal, barriers etc. to redirect

pedestrians. Any changes to pedestrian paths along Uralba Street or into the hospital departments require

consultation and approval by LHD;

- Develop and implement traffic control plans (TCP’s) that detail control measures to demarcate the public,

pedestrians and construction traffic;

- To recognise the critical importance of hospital operations and develop TCP’s to mitigate any impact on the

Hospital Main Entrance and Emergency Department along Uralba Street. A priority is to ensure that the

Ambulance Bay drop-off is not impeded by construction vehicles and traffic;

- Any modification, if required to public roads, entrances, kerbs and associated drainage within road reserves is

to be consulted and approved by Lismore City Council (LCC) and other relevant authorities;

- Access to properties, vehicular driveways, carparks and adjacent businesses are to be maintained where

possible and any temporary closure requires consultation with the relevant parties including the LCC

- Qualified traffic controllers are to manage traffic into and out of site as required by the implemented TCP’s

- Reporting of any traffic or pedestrian incidents immediately to the Principal (verbally) and in writing within 24

hours

- To prevent incident or injury due to traffic movements/conditions;

- To minimise traffic delays and adverse impacts on the streets surrounding the site, particularly on Uralba

Street, Little Uralba Street, and Fermoy Avenue;

- To provide a suitable parking solution for workers/visitors;

- To have a clear traffic procedure for all site deliveries to improve efficiency and safety.

2.2 Key Performance Indicators

Outlined below are the Project’s overriding Traffic Management key performance indicators;

- JH target to have no more than 5 traffic complaints per month;

- JH to target zero Loss Time Injuries (LTIs) due to a traffic related incident;

- JH to target zero traffic incidents both within the site compound and on Uralba Street, Little Uralba Street and

Fermoy Avenue during designated deliveries;

- JH to advise Principal’s Authorised Person of traffic complaints within 24 hours from time of complaint;

- JH to maintain a maximum 24‐hour response to community traffic complaints/queries;

- JH to include traffic complaints/issues close out actions to the Principal’s Authorised Person via Monthly

Report and formally register within Project Pack (JH internal system) complaints section.

3 VEHICULAR TRAFFIC IMPACTS

3.1 General Site Considerations

JH understand and acknowledge the importance to ensure Hospital operations and business continuity is maintained. The objective of the traffic assessment and proposed delivery strategies have been developed to maintain and where possible eliminate disruption to the Hospital’s Main Entry and Emergency Department (including the ambulance bay) that are located along Uralba Street, in particular the intersection of Uralba and Little Uralba Street (location of Ambulance Bay entry). As required, any impacts or disruption to vehicular and pedestrian access to these areas will require detailed planning and consultation with the LHD representative and the Principal.

It is recognised that Uralba Street is a major thoroughfare for heavy vehicles, passenger vehicles, bike riders and pedestrians. Because of this, it is mandatory that specific Traffic Control Plans (TCP) be developed and implemented to safely control and segregate general users of the surrounding roads from construction traffic. The project specific Traffic Control Plans (TCP) are included in Appendix 2 with below paragraphs explaining the development and need for those TCP’s.

The Stage 3B2 site is heavily constrained and restricts all project deliveries to be via Little Uralba Street. John Holland’s proposal for deliveries management during North Tower construction took into account the probable impact on surrounding properties and roadways, the safety of public motorists and pedestrians, the safety of site personnel, and the ability of different sized trucks/vehicles to navigate the intersections at either end of Little Uralba Street.

The keys points to developing the TCP’s for the use of semi-trailers and rigid vehicles are:

- Key plant and deliveries for North Tower that necessitate the need for semi-trailers, these include: Piling Rig,

pile cages, structural steel, reinforcement deliveries, material hoist components, tower crane components,

satellite pump components, formwork, fit out materials, etc.

- JH developed and implemented a TCP for semi-trailers for the removal of the Maternity pod sections in early

2017 along Little Uralba Street. The methodology developed and implemented was the approach of the semi-

trailer from Hunter Street to Little Uralba Street and then exiting via a right turn onto Uralba Street. During the

maternity pod removal, it was learned that semi-trailers were able to successfully navigate Weaver Street and

Fermoy Avenue to access Little Uralba Street without damaging kerbs. A detailed TCP was developed that

included the closure of Little Uralba Street to general traffic that would detour via Fermoy Avenue.

- Closure of Little Uralba Street enables the movement of rigid trucks (flatbeds, concrete, etc.) within the site

compound and exit back onto Uralba Street therefore eliminating the exit route of all rigid vehicles through the

residential streets of Fermoy Avenue and beyond.

3.2 Construction Materials Delivery and Removal

All construction deliveries to site will be planned and pre‐arranged by JH through the subcontractors at the daily coordination meeting (held in the afternoon and coordinated by the JH Site Manager and Supervisors).

The major deliveries and vehicle movements for the construction of the Lismore Base Hospital Stage 3B works are:

- Delivery vehicles – heavy rigid, articulated, utes, etc.

- Possible precast concrete elements

- Formwork

- Steel reinforcement

- Concrete/steel/façade panel deliveries, glazing, roof sheeting

- Ceilings and partitions material: stud and plasterboard, internal glazing, door frames, doors, etc.

- Services: cable trays, cabling, pipework, etc.

- Finishing materials and FFE: vinyl, ceiling tiles, paint, waterproofing, FFE, handrails, door hardware, etc.

- Earthworks/pavement materials deliveries;

- Excavated earthworks materials transported from / off site;

- Tank and pump deliveries (potable water tanks, sprinkler pumps, potable water pumps, etc.)

- Tower crane

- Materials hoist

- Satellite concrete pump

- Plant visits (excavators, tipper trucks, Manitou, forklifts, mobile cranes, mobile concrete pumps, concrete

trucks, hired equipment, scissor lifts, EWP’s, excavators, etc);

- Delivery of new plant including but not limited to: main switchboard (MSB), generator, chillers, condenser

pumps, Air Handling Unit’s (AHU), electrical boards, hot water units, medical gas pumps, etc.

- Miscellaneous large MME and FFE equipment deliveries (i.e. imaging equipment, pendants etc.).

The proposed traffic control plan for North Tower plant, deliveries and materials handling is:

- Closure of Little Uralba Street from Ambulance Entry to the northern edge of the JH site compound on Little

Uralba Street with designated signage. This will eliminate the interface of public vehicles with construction

traffic within the construction deliveries and materials handling site compound.

- To eliminate pedestrians interface with the high volume of construction deliveries on Little Uralba Street, JH

will be installing a chain-wire fence with gates adjacent to the HI office driveway (i.e. still provide access to HI /

CBRE / LHD to the house and office) and to the northern edge of the JH site compound.

- The chain-wire gates will eliminate both public vehicles and pedestrians from interacting with construction

plant and deliveries.

- The gates will be closed when the TCP are implemented. JH proposal is for the TCP to be implemented

during construction hours and then opened to allow public to use outside of construction hours. JH understand

that through consultation with stakeholders (HI, CBRE, LHD, and LCC) that the TCP will be implemented

permanently during construction/working hours of the construction phase of North Tower.

- These gates will allow residents unimpeded access to the houses located north of the site compound, while

keeping the construction area clear of public vehicles and pedestrians.

- With the proposed gate location, all loading/unloading activities would be isolated from any public vehicles and

pedestrians, including the residents on Little Uralba Street who would retain access to their properties from the

northern end of Little Uralba street.

- As per the developed TCP, the public can use the detour from Uralba Street, to Bent Street, to Fermoy

Avenue to access residencies, etc.

- Outside of working hours, the gates would be opened to allow residences to use Little Uralba Street whilst

also maintaining the 24-hour access to mortuary driveway

- A JH traffic controller will manage all incoming and exiting deliveries and eliminate any parking or blockage to

Ambulance Entry Bay

- JH will not accept any delivery vehicles parking along Uralba Street outside the hospital precinct.

The construction plant and delivery movements proposal for North Tower is:

- For heavy rigid, flatbed, concrete trucks, incoming route will be heading east along Uralba Street with a left

turn into Little Uralba Street through the chain wire gates and unloaded within the site compound area. The

exit route would be out the southern gates on Little Uralba Street and a left turn onto Uralba Street. These

type of delivery vehicles can manoeuvre the turnaround within the site compound area.

- For articulated (semi-trailer) vehicles, it is proposed the incoming route would be from the northern end of

Little Uralba Street (i.e. Uralba Street to Hunter Street, with right turn to Orion Street, right turn to Weaver

Street, left turn onto Fermoy Avenue and a right hand turn into Little Uralba Street). The semi-trailer would

reverse into the south-western corner of the site compound, utilizing the flat area at the southern end of the

site compound to load/unload. The exit route would be heading south from Little Uralba Street and right turn

onto Uralba Street with traffic controllers. Noted that semi-trailers could not reverse from Uralba Street to Little

Uralba Street from a traffic movement perspective or from a safety perspective (reversing of vehicles from

Uralba Street down Little Uralba Street would also increase impact to Ambulance Bay Entry).

The above proposed semi-trailer movement is very similar to the movements implemented for the removal of the Maternity Pod segments in early 2017.

3.3 Site Access

The site layout will include a loading/unloading area on Little Uralba Street (property lots for numbers 9, 11 and 15) which will also incorporate a turning area for rigid vehicles. Appendix 3 contains a plan showing the site layout and turning area on Little Uralba Street. As described above, access to the loading/unloading area will be from either the southern (Uralba Street) end of Little Uralba Street for rigid/flatbed/smaller vehicles, or the northern (Fermoy Avenue) end of Little Uralba Street for semi-trailers.

A luffing tower crane located on the western side of Little Uralba Street will service the loading/unloading zone, with an overhead-protected walkway providing pedestrian access between the construction area and the loading/unloading and amenities areas.

A concrete pump located directly adjacent the vehicle turning area on Little Uralba Street will pump concrete across (via an

underground pump line installed under the road) to the construction area where it will be placed via a satellite concrete pump.

Concrete trucks will access via the southern end of Little Uralba Street, enter the truck turning area to feed the concrete pump, and

exit via the southern end of Little Uralba Street.

3.4 General Construction Deliveries

As shown in Appendix 3, all construction deliveries will enter/exit the site via Little Uralba Street. The project will separate worker access routes from vehicle routes using either water-filled barriers or concrete barriers and applicable signage.

All visitors must report to JH site office at 73 Uralba Street prior to being permitted to enter the construction site. Deliveries for large scale items and oversized sections will be coordinated and planned to mitigate the disruption to external roads and pedestrians as well as the normal onsite traffic and pedestrian access.

JH will implement a brief Delivery Driver Induction process which will educate drivers on key site risks and requirements (i.e. sloping site, restricted space, use of wheel chocks etc.), mandatory PPE, designated loading/unloading zones and emergency response information.

All drivers will adhere to the below Code of Conduct, which aims to minimise the impacts of earthworks and construction on the local and regional road network, minimise conflicts with other road users, minimise road traffic noise, and ensure truck drivers use specified routes.

Driver Code of Conduct

- All drivers will adhere to the LBH 3B Traffic Control Plans (for both north-bound and south-bound traffic on

Little Uralba Street). This includes all vehicles (with the exception of semi-trailers) turning left onto Uralba

Street when exiting Little Uralba Street from the southern end, to prevent the need to close both lanes of

Uralba Street. Semi-trailers are to turn right onto Uralba Street when exiting Little Uralba Street from the

southern end, and this must only be done as per the TCP (both lanes of Uralba Street temporary closed).

- Drivers of semi-trailers must not travel east on Fermoy Avenue past the Little Uralba Street intersection, nor

must they enter Bent Street from Uralba Street.

- Delivery vehicles must not stop on Uralba Street or hinder Hospital parking, loading or unloading areas,

including the Uralba Street Ambulance Bay.

- Deliveries must only be performed during approved construction hours.

- All delivery drivers entering the site will undergo a Delivery Driver Induction outlining the site rules and TCPs.

3.5 Concrete Deliveries

Concrete deliveries, predominately for North Tower structure, are considered to be a special case of deliveries, given that once a concrete pour has commenced it is essential that concrete continues to be delivered without delay until the pour is complete. The strategy will be to commence larger pours first start in the morning and finish by mid‐ afternoon.

During peak stages of construction, concrete deliveries would involve up to a maximum of 14 trucks per hour (including allowance for a member of ancillary trucks for large scale steel/façade deliveries that might be coordinated concurrently). Due to the nature of concrete pours it is necessary to ensure continuous deliveries; hence the concrete deliveries will extend through the morning peak hour. It is noted that large concrete pours may require concrete deliveries to extend into the afternoon, however they are not expected to extend into the peak afternoon / PM traffic period.

3.6 Traffic Management Implementation

3.6

Please refer to Appendix 2 which includes the project specific TCP’s for the project. The plans nominate traffic control

measures, signage and site arrangements to be implemented for the project. An accredited traffic control designer will

be engaged by JH to develop, consult with Lismore City Council and obtain approval for the relevant TCP’s that will be

implemented during the various phases of the project including the use of accredited traffic controllers, signage and

equipment (witches hats, barricades etc.) to control the traffic and pedestrians through the project.

As required during the project, additional TCP’s maybe required to be designed, approved & implemented due to

specific requirements.

Implementation of the Traffic Management Plan will be monitored via a daily signage register and inspection

completed by the site traffic controller. In line with the LBH 3B/3C HSE Risk 3 Month Look-Ahead Tool, John Holland

will also conduct Traffic Management inspections using form 3B2-FRM-012 Traffic Management Inspection.

3.7 Uralba Street Closure for Generator Install to L7 South Tower

North Tower works will include the installation of a new generator to the level 7 western balcony of South Tower (E

Block). This will involve the set-up and operation of a mobile crane on Uralba Street, along with delivery vehicle

access adjacent to the crane, which was performed on the previous stage of works to lift plant to the same balcony

area.

These works will require the closure of the west-bound lane of Uralba Street. Vehicles travelling east to west along

Uralba Street will be detoured via Dibbs Street, McKenzie Street and Hunter Street. Traffic flow travelling west to east

on Uralba Street will be maintained at a reduced speed and be diverted into the east-bound lane by traffic cones and

signage (due to closure of west-bound lane). The use of the drop-off / pick-up lane will also be maintained during the

works and will be managed by traffic control and spotters to prevent any suspended loads travelling over vehicles or

pedestrians. The LBH Ambulance Bay will remain open at all times and maintain priority. These controls will be

included in a specific Traffic Control Plan requiring approval by Lismore City Council prior to works. This TCP will use

the same set-up and controls as outlined in the TCP (see Appendix 7) utilised on the previous stage for similar works.

Along with the generator itself, the mobile crane will also be required to lift the associated load banks and the supporting steel frame components to the balcony. It is predicted that there will be three non-consecutive instances (lasting a maximum of one day each) where the TCP will need to be implemented, being;

- One day for lifting the supporting steel frame components

- One day for lifting the generator and load banks

- One day for lifting down the load banks.

The finalisation and approval of the TCP and the execution of the works is planned to occur in late 2018 or early 2019.

4 INTERNAL TRAFFIC

4.1 Construction Personnel

An average of 80 construction personnel are expected to work on the site per day with peak of up to 200 for the major

stages of construction. It is expected that a majority of personnel will arrive by car. A car occupancy rate of 1.2

persons per vehicle has been assumed, which is consistent with industry standard trends for travel for work purposes.

On average, i.e. during non‐peak construction periods it is estimated that 72 cars will arrive during the morning and

depart during the evening. During the peak construction periods it is estimated that over 150 cars will be travelling to

and from the site each day. The provides average estimates only noting that the workforce will increase from small

numbers at the project commencement and will correspondingly decrease post the peak workforce towards the end of

the project.

The peak hour for construction personnel arriving on site will typically be between 6.00am and 6.45am. The morning

peak hour for general traffic in the area (i.e. on Uralba Street) occurs between 7.30am and 8.30am. Therefore, the

construction traffic is not expected to increase the volume of traffic or degree of saturation of the road network at this

existing peak time (i.e. minimal impact to peak Uralba Street traffic volumes).

Construction personnel will typically leave the site between the hours of 3.30pm and 4.30pm. Hence the majority of the

construction personnel traffic will exit prior to the typical peak times on the external road network which is between

4.30pm and 5.30pm. Due to no schools being located adjacent to the hospital, the construction traffic is not expected

to impact on any school traffic peak times. Based on this the construction traffic will have minimal impact on the

volume of traffic or degree of saturation on the external road network at the existing peak time.

During peak phases of construction, additional construction personnel is not expected to cause an increase in traffic

volumes on the surrounding access roads that would exceed normal peak hour traffic levels.

4.2 Construction Parking

There is shortage of street parking directly around the Hospital to cater for the operational hospital’s number of staff

and visitors. As per the head contract requirements, there will be no construction parking in the hospital campus or

within one block of the hospital campus.

Appendix 5 shows the area prohibited for construction parking, and the JH project induction will include this diagram.

These provisions will also be included in subcontractor agreements.

JH acknowledge the sensitivities of the lack of car parking in and around the hospital and will work with the Principal

and LHD to achieve a mutual outcome.

4.3 Internal Traffic Movements

Internal traffic movements have been designed to minimise reversing activities in line with John Holland GMR 1 –

Traffic, Plant and People. As per the devised Traffic Control Plans in Appendix 2, rigid/flat-bed trucks will enter site via

the southern end of Little Uralba Street, have sufficient room to turn around within the Site and park on level ground for

loading/unloading, and then exit site again from the southern end of Little Uralba Street onto Uralba Street.

Semi-trailers will enter from the northern end of Little Uralba Street, reverse onto the level loading/unloading area

under direction of a spotter and exit via the southern end of Little Uralba Street under traffic control.

Within the site itself, pedestrian walkways will be separated from the vehicle access routes by use of waterfilled

barriers or concrete barriers.The proposed plan of isolating the loading/unloading areas from public pedestrians and

vehicles by use of gates on Little Uralba Street further minimises the risk of a traffic incident.

5 OPERATIONAL MATTERS

5.1 Approvals

Prior to implementation, JH understands that there is a formal approval process before the implementation of

proposed traffic control plans and any proposed alterations to Council and State infrastructure.

The traffic management strategy for heavy rigid and for semi-trailer vehicles accessing from the south and north ends

of Little Uralba Street respectively has been submitted and consulted with LCC and the Principal. JH understand that

the proposed strategy has been endorsed by LCC (see Appendix 6).

Further, this plan provides the Principal with detailed proposal of the traffic management strategies, objectives and the

developed TCP’s for the project.

5.2 Notices

Where applicable, JH will notify surrounding residences and businesses of significant traffic condition changes and

heavy vehicle movements via a letterbox drop process. This will be at the discretion of JH in consultation with the

Principal. If the traffic conditions are deemed to be highly significant, it may be that a public notice is placed in the local

newspaper.

5.3 Revisions and Future Consultation

The overarching TMP for the Lismore Base Hospital is a live document. As such, JH will review and update the

document if required. JH will also continue to consult LCC of any changes to site access/traffic issues as the project

develops.

6 COMPLAINTS AND REPORTING

6.1 Reporting Protocol

All traffic complaints will be directed to a phone line within the JH Project Site Office. In the event of an urgent enquiry

(i.e. medical/emergency), the contact details for the Site Manager and Project Manager are provided on the Project

sign located at site entry and outlined at the site induction.

Complaints will be classified and issued to the Project Manager daily. The Project Manager will keep the Principal

informed of any complaint and the response proposed. A summary of complaints and resolutions will be provided

fortnightly at the Site Coordination Meeting and in the monthly report to the Principal’s Authorised Person. Complaints

will also be logged on a complaint form and lodged into JH’s internal reporting process, Project Pack. Refer to

Appendix 4 for sample of the complaint form.

Receipt of a call

Details of the call are recorded including time, issue or enquiry, name and details if provided,

answer provided, and action taken. These details are also to be entered on to the complaint

form in the Project Pack (JH internal system).

Unanswered calls

Outside of business hours callers can leave a message which will be recorded as above. If

further detail is required or a request for a call‐back is received, staff will contact the caller

(where contact details have been provided) and request further information. This call‐back will

take place within 24 hours of receiving the message.

If no contact details are provided, comments will be entered into the reporting template under

‘anonymous’.

Response time

All queries will be responded to within 24 hours and where possible within a shorter timeframe,

Monday to Friday (during business hours). Any queries received on the weekend/public holiday

will be responded to on the next working day.

Register

All complaints will be registered into JH’s internal reporting mechanism, Project Pack (refer

Appendix 4) and included in JH Monthly Report issued to the Principal’s Authorised Person.
