End-stage renal disease (ESRD) is almost unique in that patients may choose (and can later change)
their treatment modality, if there are no contraindications.

Proven benefits of Renal replacement therapy option education include: • reduced urgent dialysis starts, reduced time spent in hospital and
improved resource utilization [1]; • earlier placement of permanent vascular access or peritoneal
catheter [2]; • a greater likelihood of choosing a self-care modality [3]; • extended time to requiring dialysis [4]; • improved adherence [5]; • reduced anxiety and fear [6]; and • reduced mortality [7].
These benefits lead to considerable cost savings [1].

Maintaining lifestyle: • The medical outcomes of treatment were considered less important
than the effect of the treatment on the patient’s lifestyle—that is, patients were less concerned about their longevity than they were about their quality of life.
• Treatment choices were based on minimizing disruption to usual activities, upholding responsibilities, and maintaining personal interests.
• Examples of this included the ability to continue working, maintain a social life, or care for grandchildren.

Peer influence: • Peer influence was a powerful and persuasive method for patients
to gain knowledge of their treatment options. • Meeting other patients and listening to their experiences helped
patients and their carers to conceptualise the reality of dialysis and transplantation.
• Peers may have been more influential than clinicians in decision making.

Timing of information: • Ten studies reported the importance of the timing of information
on treatment options. • Patients recounted being too unwell to take in the information
presented or too rushed into making a decision without having time to discuss the options with their families.
• Information about kidney transplantation was commonly introduced to patients after dialysis had been established.
• For some patients information about treatment options came after undergoing surgery for vascular access.

Il progetto PIRP non è uno studio di popolazione, nè un protocollo di studio, ma un progetto con una forte valenza clinico-assistenziale, e si avvale della stretta collaborazione con i MMG e altri specialisti. E’ basato sul concetto del quality improvement. Il progetto è rivolto alle persone a cui viene riscontrata una MRC o che sono a rischio di svilupparla, affinché siano precocemente identificate e indirizzate ai nefrologi. Le 13 UO di Nefrologia della Regione hanno organizzato degli ambulatori espressamente dedicati alla diagnosi e cura della MRC. Per I MMG sono stati fatti specifici corsi di formazione. I dati anagrafici,clinici,biochimici dei pazienti con MRC accertata (da > 3 mesi) vengono inseriti in un Registro informatizzato, che viene aggiornato ad ogni visita ed è alimentato da tutte le 13 UO che aderiscono al progetto.

• Some patients might not be receiving all the information they need about renal replacement therapy options for ESKD and the limited likelihood that dialysis has for restoring health or prolonging life.
• We found that many patients did not have much understanding of
dialysis prior to their decision even in the absence of urgency.
• Interventions to promote effective communication to help patients
understand that the decision to initiate dialysis is complex and requires significant trade-offs are solely needed.
