As you begin reading the Fallissue of CareLines, we arebeginning our recognition ofNational Hospice Month. Thisyear’s theme, Hospice: Care onYour Terms, puts into wordswhat our focus has been atCaldwell Hospice since 1982—patient-centered end-of-lifecare. The guiding principle ofDame Cicely Saunders, whofounded modern hospice care,was: “You matter to the lastmoment of your life, and wewill do all we can, not only tohelp you die peacefully, but tolive until you die.”

Our holistic approach to hospice care reflects DameSaunders’ strong belief inlistening to the patient. Ourinterdisciplinary team memberswork with patients to address all their concerns—physical,psychosocial, and spiritual—and to resolve them, whenpossible, but above all, to bethere as their “companions forthe journey.”

Part of listening to patients andoffering care on their termsinvolves recognizing the partic-ular needs of patients in varioussituations or circumstances.

Our cover story draws attentionto our Dementia Care Programand the opportunity to provide

“care on your terms” fordementia patients. The “sensorykits” are helping makemeaningful interaction possiblefor dementia patients, theirfamilies, and our team members.The training for Dementia CareProvider and Dementia CareSpecialist qualification preparescertified nursing assistants(CNAs), medical social workers,nurses, and volunteers to recog-nize dementia patients’ needsand address them compassion-ately. Our training with theVirtual Dementia Tour® gives ussome understanding of what dementia patientsexperience every day—and ouropportunity to facilitate thistraining will make it available toother healthcare providers,family members, groups, andindividuals in the community.

In November, in honor ofVeterans’ Day, we are proud toshare the stories of three localmen who were called to militaryservice—Mr. Bud Brown, Mr.Bobby Harmon, and Mr. MelvinGray. Each of them helps us ascivilians to understand what“care on your terms” means toveterans.

in the grocery store check-outline and watch their faces. Theirexpressions will tell you whetherthey’ve met the problem—yet. Ifyou continue talking, their storieswill be familiar: “Right in themiddle of our phone conversation,my aunt asked, ‘Now who am Italking to?’” “He’s been driving tothe post office daily for 30 years,at least, and today he called me,panic-stricken, because he didn’tknow where he was!”

In Caldwell County, Alzheimer’sdisease and dementia-relateddeaths in 2000 numbered 33;after more than doubling to 71 in 2010, there was a steadyincrease to 74 in 2011 and 86 in2012 (Source: Don Akin, Statistician,North Carolina State Center forStatistics, 07/05/2012, 10/07/2013).

During FY 2011-2012, about18.5% of patients admitted toCHPC hospice care had a primaryor underlying diagnosis ofdementia/Alzheimer’s disease. InFY 2012-2013, the percentagehad increased to 29.51%.

Caldwell Hospice is exploring anddeveloping resources to betterserve our patients, families andcommunity, struggling with or

who will soon enough strugglewith Alzheimer’s disease or anyform of dementia. Many CHPCstaff members and volunteershave completed Alzheimer’sFoundation of America (AFA)specialized training to becomequalified Dementia Care Providersand Dementia Care Specialists toequip them to meet the needs ofour patients and their families.

Additionally, Caldwell Hospicenow offers the Virtual DementiaTour® (VDT), a scientificallyproven experience designed tobuild sensitivity and awareness inindividuals caring for people withdementia. While we cannotunderstand fully what dementia“feels like,” the VDT allows ussome insight into the lonely,frightening world of dementia by“walking a mile in their shoes.”Through the AFA grant and CHFdonation, Caldwell Hospicepurchased VDT kits and trained agroup of facilitators to offer thisexperience to our staff andcommunity. P.K. Beville, award-winning geriatric specialist andfounder of Second WindDreams®, an international,nonprofit organization recognizedas the first in the nationcommitted to changing theperception of aging through thefulfillment of dreams for elders,created the VDT.

Through sharing the VDT, wehope to increase dementiaawareness among people whocare for and care about peoplewith dementia, thus creating abetter environment of care and amore meaningful interaction withpeople who have dementia. Caldwell Hospice isexploring and developingresources to better serve our patients, families andcommunity struggling withor who will soon enoughstruggle with Alzheimer’sdisease or any form ofdementia.

With an AFA grant and adonation from Caldwell HospiceFoundation (CHF), we createdDementia Resource Centers inboth Caldwell Hospice patientcare units and equippedDementia Sensory Kits for use byteam members with our patients.

Our Dementia Resource Centersstore five kits designed toenhance the quality of life forpatients with dementia. Thekits—including art, sports, music,laundry, and dolls/ stuffedanimals—offer an array ofoptions for connecting with apatient’s life experiences.

Caldwell Hospice staff and volunteers using the resourceshave reported many positiveresponses from patients, aswhen a patient named her babydoll and asked to have a picturetaken of the two of them. Hernurse, CNA, volunteer, andprimary caregiver expressedamazement at her enthusiasm.

Scouting Projects Benefit Hospice

Patty Williamson, a 25-yearveteran of Caldwell Hospicevolunteer service, accepted theJudith Lund Person HospiceVolunteer Award from Judith LundPerson at The Carolinas Center for Hospice and End of Life Care(TCC) conference in Asheville,recently.

“For me, becoming a hospicevolunteer meant opening a doorto friendships,” Patty wrote inCaldwell Hospice’s newslettersome years ago. “Through volunteer encounters, I think the people in Caldwell County—both giver-receivers and receiver-givers—are knitted togetherpermanently. Lucky us.”

“When I started, if Patty herselfwasn’t available for an assign-ment, veteran social workerswould call to ask for ‘someonelike Patty Williamson’ for high-riskassignments,” says Brittany Bonn,Caldwell Hospice’s volunteercoordinator. “She approachesevery situation with confidenceand a calm demeanor.”

Patty hasfulfilled justabout everytype ofCaldwellHospicevolunteerassignment:sitting withpatients andfamilies as an 11th-hour volunteer; documenting files inpatients’ charts for decades andtraining newer volunteers for thisassignment, recently; staffing thedisplay board and greeting seniorcitizens at the annual Celebrationof Life luncheon; feeding, pickingup meals for, and sitting withrestless patients in the patientcare units; transporting patientsto doctors’ appointments.

Caldwell Hospice is proud toannounce the recognition thatPatty has gained for her “inspira-tional, insightful, and involved”service to the organization and its patients.

Grief in Schools BreakfastThe bereavement staff welcomed Caldwell County Schools supportstaff members—school nurses, psychologists, counselors, and socialworkers—to its annual Grief in Schools Breakfast in September. After a hearty breakfast, Bereavement Support Specialist SandyWhite explained the grief and loss services available for students and staff members. She and Bereavement Coordinator Steve Butler,D.Min., announced winners for the morning’s door prizes, whichincluded two backpacks filled with school supplies. Ashewood Grief and Counseling Services has hosted the combined social andinformational breakfast for approximately 10 years.

According to the NationalHospice and Palliative CareOrganization, in 2011...

• The median length of servicefor hospice patients is 19.1days, yet more than 35% ofpatients die or are dischargedin seven or fewer days—tooshort a time to benefit fromthe full range of services ahospice can offer.

• Cancer accounts for under38% of hospice patients—surprising to many people whomistakenly think hospice onlyserves cancer patients. Thefive other leading diagnosesare heart disease, debility,dementia (includingAlzheimer’s disease), lungdisease, and stroke.

• Research shows that eight of10 Americans would want tostay in their homes, surroundedby family and loved ones, ifthey were faced with a life-limiting illness. Hospice caremakes this happen.

Answering the Call to Service“Duty” is a complex word—is it a“burden,” an “obligation,” our“responsibility,” a “job” or a“calling”? For the three meninterviewed in recognition of USmilitary service veterans—onewho is currently a CaldwellHospice patient and two whoserve now as Caldwell Hospicevolunteers—“calling” is the rightdefinition, maybe the only definition. Each man carried his obligation to country; hecompleted his job. And each manrecognized a richer purpose,deeper meanings.

Melvin Gray:Called to CompassionMelvin Gray welcomes you intohis house with a handshake.When you prepare to leave, heasks whether he can say aprayer, and then he sends youinto the world with a hug. I hadonly just met him, and thisgentle man made me feel aswelcomed as his medical socialworker, who visits him regularly.

Melvin Gray was comfortable withthe sea, having grown up atCarolina Beach, so his choosingthe US Navy was no surprise, buthis parents had to sign for himbecause he was only 17. Hewanted to serve on the sameship with his brother Don, but thegovernment wouldn’t allow it.

Melvin’s job was security on theRobert L. Wilson, the destroyerto which he was assigned. “Icarried a .45 and was supposedto keep everything straight.”

For Melvin Gray, the slogan “Jointhe Navy and see the world”included short trips—“maybe aweekend”—to such places asCuba, Puerto Rico, Spain, Italy,France, Greece, Turkey, Gibralter,and Egypt. “We wouldn’t staylong anywhere.”

Decades later, Melvin Gray’scompassion remains evident.Portsmouth, England, he recalls,was “the poorest place we went.They were still on ration booksafter World War II. Somecountries were so poor, I don’tknow how they survived,” hecontinued. “We threw orangesand apples to people as we cameinto port. They would begrabbing food.”

Melvin Gray’s worst memory fromthe Korean War was “seeing howpoor people were, and I couldn’tdo anything about it.”

When he returned home, MelvinGray worked for Blowing RockChair Company; lived inWashington, DC, for “a longtime”; came back to Lenoir andworked in the furniture industryuntil retirement.

Married for over 50 years, MelvinGray has three children, and four

grandchildren. His living roomshelves are weighed down withframed family photos and withmementoes from his militaryservice. The essence of this kindman can be captured in his lovefor his God, his family, and hiscountry.

Bobby Harmon:Called to DevotionWhen Bobby Harmon was stillliving at home, he was helpingwith his father’s five chickenfarms. At one time, he wasgoing to school, working atSpainhour Furniture, serving inthe National Guard, and helpingon the farm. He claims thatworking on the chicken farm“made me go into the Army!”While the motive might not havebeen pure at the time, thedevotion to his duties hasbecome a lifelong commitment.

In addition to his National Guard service, Bobby Harmon’sactive-duty Army career lasted23 years. He completed twotours of duty in Vietnam, and he was stationed at the DMZ(demilitarized zone) in Korea,and in Germany, Belgium, andHolland.

Bobby Harmon remembers theenjoyment of working with manyyoung soldiers and says hemisses that. However, his retire-ment reflects a natural and

compassionate extension of hisservice. He has served aschaplain for Tri-County ChapterSix DAV (Disabled AmericanVeterans) and the Lee PowellPost 5381 VFW (Veterans ofForeign Wars). Currently, he isthe Chapter Six DAV chaplainand vice-commander for theChapter Six DAV Honor Guard,which participates in funeralservices for veterans.

Veterans need people who “know where they’ve been andwhat they’ve been through,”Bobby Harmon says. “Combatveterans react totally different to life situations because theirexperiences are unlike those ofcivilians.” He feels honored toserve as a chaplain to them,listening and understanding.

Bobby Harmon’s devotion to the needs of others came withhim to Caldwell Hospice, wherehe and his wife of 49 yearsvolunteer together.

Whatever the need is, look forBobby Harmon to fulfill it or findan individual or organizationwhich can.

Bud Brown:Called to CommitmentDependable. Affable. Busy. Threeof the many words that describeveteran and Caldwell Hospicevolunteer Bud Brown. Whencalled upon, he’s always ready—to accept a new hospice patient-family assignment, document aVeterans Honoring Veteranspresentation, help with church,community, or American Legionprojects. At the age of 85, Bud is always involved and alwaysinterested.

Bud Brown entered active dutywith the US Army Air Corps in1946, and for 20 years, hezigzagged from foreign todomestic assignments. He retiredfrom the Army as First SergeantE-8 Active and Captain, InfantryReserves, in 1966.

Moving to Lenoir, Bud worked at Bernhardt Furniture untilretirement 40 years later. Hechaired the SesquicentennialVeterans Committee; he chairedthe Caldwell Veterans HistoryCommittee, maintained member-ship in the American Legion, andhis commitment to the EmployerSupport of the Guard and Reserve(ESGR) has earned him NorthCarolina chair emeritus status.

When Caldwell Hospice’s volunteer program initiated the Veterans Honoring Veteransprogram, Bud began takingphotos during ceremonies asCaldwell Hospice volunteers whoare military veterans presentcertificates, American flags, and pins to hospice patients who are veterans.

It’s hard for soldiers to talk with civilians about their experiences,Bud says, but when they are withother veterans or families ofveterans, they have a connection.And Bud Brown is committed toserving them, caring about them,understanding them: “You needto know who they are and whatthey’ve done. Pass your love tothem.”

Each of these men was called toservice in the United Statesmilitary, in domestic and foreignassignments. Each man answereda deeper calling: Melvin Gray’scompassion for people whosesuffering he couldn’t “fix.” BobbyHarmon’s devotion to supportingveterans in whatever way theyneed. Bud Brown’s commitmentto veterans and to his commu-nity. Caldwell Hospice offers ahumble thank-you to these menand to all the veterans in ourarea who answered the call toservice.
