2012 Accomplishments and HighlightsJohn C. Lincoln Health Network

The John C. Lincoln Health Network is an autonomous, not-

for-profit, Arizona-based health and human services system

with a long, rich tradition of caring for people since its

beginnings helping meet the needs of families living in the

tuberculosis health camps on the hills of

Central Phoenix in 1927.

Today, the Network is comprised of two

award-winning hospitals, a Physician

Network of primary care and specialty

practices, a spectrum of human services

programs through Desert Mission

ranging from a Community Health

Center to a Food Bank, and a foundation

that serves as the philanthropic arm of

John C. Lincoln’s efforts.

We continue the legacy of caring for our

community. In 2012, we had almost 750,000 patient visits,

and we continue to execute our vision to be the health

care provider of choice locally and recognized nationally for

quality, service and community programs.

As community needs increased, Desert Mission served

almost 40,000 clients through its Food Bank, Community

Health Center, Children’s Dental Clinic,

Marley House Behavioral Health Clinic,

Adult Day Health Care, Lincoln Learning

Center and Neighborhood Renewal.

The Network received several awards

showcasing its high quality of care, its

commitment to the community and its

fiscal responsibility. JCL Connect, our

electronic health record, was launched

at both hospitals and most physician

practices.

Desert Mission

Pathway to Healing at John C. Lincoln North Mountain Hospital

The transformation of the lobby and surrounding areas

opened at the end of November as a “Pathway to Healing”

for patients and families on a journey to wellness. David and

Joan Lincoln, and Lillian Lincoln Howell, donated $4 million

to the project. Additional major contributors included the

John C. Lincoln Health Foundation Guild, Lincoln GIVES, the

Health Network employee giving program, and the John C.

Lincoln North Mountain Auxiliary. Their generosity allowed

us to reinvent the patient experience and create a lobby

that reflects the quality of care received inside the hospital.

Every weekend, thousands of school-age

children have nutritious food because of

our Snack Pac Program. In 2012, 40,352

Snack Pacs were distributed, almost double

the number from 2011. A generous grant

of $50,000 from the Trends Charitable

Fund supported this important community

program. Thanks to The Walmart Foundation,

Desert Mission Food Bank received a gift

of $50,000 to help fund the 40,752

emergency boxes given to our families

in 2012.

Pediatric Dental and Health Care

In 2012, the Children’s Dental Clinic served

children ages 4 through 20, with 3,420 visits.

The Charles A. Becker Foundation provided

support with an $80,000 grant for dental care

and pediatric health care.

John C. Lincoln Adult Day Health Care welcomes

seniors in a warm, safe and pleasant setting for

activities, social interaction and field trips. The

Community Health Center provides a medical

home to low income children and their caregivers

and had 4,347 patient visits in 2012. Arizona

Republic Charities and 12 News Season for Sharing

provided two grants totaling $20,000 to support

the Community Health Center and recreational

activities for seniors at Adult Day Health Care.

Health Care and Senior Activities

Preventing Congestive Heart Failure Readmissions

Breast Health Support

John C. Lincoln’s Breast Health and Research Center

was the first in Arizona to offer 3D mammography. This

groundbreaking technology continues to save lives every

day. The Phoenix Affiliate of Susan G. Komen for the

Cure provided $275,573 in funding to ensure women

have access to diagnostic and treatment care as well as

for community education and outreach activities.

A $28,688 grant from the Cardinal Health Foundation

supported the John C. Lincoln North Mountain Hospital’s

Transition to Home: Congestive Heart Failure Readmission

Prevention Program to help patients stay healthy at home

and avoid unnecessary hospital readmissions. 5
