A dozen new front doors: Community Clinic at Walgreens
Community’s collaboration with Walgreens is growing again, and the result will be a dozen new front doors for conveniently accessing Community care. As of mid-2017, Community will exclusively own and operate healthcare clinics at 12 Walgreens retail locations in the Indianapolis area—each location will be branded Community Clinic at Walgreens.
For the past three years, Community has had a collaboration with several Healthcare Clinics at Walgreens in central Indiana. Under the new agreement, Community will assume operational responsibility to provide clinical services at 12 in-store clinic locations beginning in June, with opportunities for future expansion. Employees of these clinics will be Community employees, the clinics will adopt Community’s CareConnect Epic Systems electronic medical record technology with MyChart access, and the clinics will carry Community Clinic at Walgreens signage. These select Walgreens-based clinics will effectively become fully integrated Community primary-care locations.
Community Clinic at Walgreens patients will benefit from a streamlined continuum of care, with easier access to other Community facilities should they need any follow-up care or desire to establish a primary-care relationship. Meanwhile, existing Community patients will gain additional convenient and fully integrated care sites, with adjacent pharmacy services when they need them.
This latest announcement follows last month’s transition that brought Walgreens locations to Community’s East, North and South campuses, and made Walgreens the preferred pharmacy for those using Community medical insurance. For more on that transition, please see page 2.

ENGAGEMENT RESULTS, AND WHAT HAPPENS NEXT • • • Community
is committed to creating exceptional experiences not just for patients, but
for employees, too. How do we know what we’re doing right, and where we
have opportunities to improve? The annual employee engagement survey.
The results are in, and indicate that the network has made significant strides—
the goal for the engagement index was 77 percent, and we hit 84 percent. Not only that…participation increased from
8,970 responses in 2015 to 9,724 responses in the latest survey. All that is good news, but it’s still just one step in the
process. Now comes the work to maintain the positive things that we’ve achieved, while also improving aspects about
the employee experience that still need attention. Every leader who had at least five direct reports take the survey will be
getting individualized engagement survey results. As in past years, they’ll share those results with their teams, and then
work with their teams to create action plans addressing areas of opportunity. Congratulations to all on the progress we’ve
made…now comes the work to build upon that progress!
AN UPDATE ON THE PHARMACY BENEFITS TRANSITION • • • In December,
Walgreens acquired Community’s Wellspring Pharmacy operations. Walgreens
now operates retail locations on our East, North and South campuses, while the
two Wellspring Pharmacy locations at Community Health Pavilions are no longer
in operation. In addition, Walgreens’ 8,200+ pharmacy locations are now the
preferred, Tier 1 pharmacy provider for those using Community medical plans.
Since the transition, our employees have filled about 11,000 prescriptions at
Walgreens. For most people, the transition has been smooth, and while some
people have reported transition issues, Walgreens and Community have been
working to resolve them and ensure that all employees and their families have
easy, cost-effective access to the medications they need. If you have questions about

COMMUNITY EAST GAINS PERINATAL CERTIFICATION • • • Community East’s maternity unit has earned The Joint Commission’s perinatal care certification…just the second hospital in Indiana to do so, and there are only 30 others nationwide. The certification focuses on achieving integrated, coordinated, patient-centered care for pregnancies and births. The Joint Commission began accepting applications for the certification last year, establishing consensus-based standards, clinical practice guidelines and core performance measures. The aim is managing and monitoring aspects of perinatal care that are critical to improving and maintaining the health of newborns and their mothers. Goals of these high standards of care include lower infant mortality rates as well as maternal complications and mortality; less need to induce labor; fewer elective deliveries that lead to NICU admissions; fewer premature deliveries; a reduction in the complication of deliveries; and reduction of the hospital costs that go along with complications and lengthy stays. Amy Wire, who leads maternity services at Community East, offers praise for her interprofessional team and the whole staff, who double- and triple-checked charts, came in early, stayed late and adjusted schedules to meet with surveyors. The core team included Laura Sparkman, Laura Leaming, Rainey Martin and Claudia Quintero.
NOW OPEN! COMMUNITY HEALTH PAVILION ANDERSON • • • This month marks the grand opening of the brand-new Community Health Pavilion Anderson, a three-story, 68,000-square-foot facility packed with convenient Community services. Tenants are moving in, several of them relocating from other Anderson addresses to improve access and enhance services. A
Community MedCheck location and an infusion clinic are now open. Soon to follow will be Community Physician Network Family Medicine, Community Physical Therapy & Rehab, Community Behavioral Health, and laboratory services. Around the end of the month, a timeshare office and the community room will be ready for the public. The new facility is at 32nd Street and Scatterfield Road in Anderson.

CommunityALMOST TIME FOR MANDATORY TRAINING • • • Ongoing training is important to the critical work we do each day in caring for our patients as well as for workplace safety. The next round of mandatory training will soon be available for all team members to complete (please note that there’s a different process and training schedule for Community Anderson employees). The appropriate eLearning courses will be assigned to your “To Do” list in MySuccess Center/MyLearning, based on your role at Community. You’ll receive an email with more details when the training is available.
STAFF PERFORMANCE APPRAISAL FORMS ARE NOW AVAILABLE
The 2016 performance appraisal forms for staff are now available for all employees who did not maintain 2016 goals in the Leadership Evaluation Manager. This is an opportunity for employees to share their accomplishments related to PRIIDE values and the essential functions of their role. The due date for self-assessments is January 18. Those hired on or after September 26 are not required to participate in the 2016 performance appraisal process and will not receive a performance appraisal form. Leader performance appraisal

IT’S A NEW YEAR…CHECK YOUR BENEFITS • • • The first paycheck of 2017 was issued Friday, January 6. Now’s a perfect time to verify that everything is in order with your Community employee benefits. You already should have received a benefits confirmation statement at your home address, and if you haven’t already done so, please review the confirmation statement for accuracy. Payroll deductions for this year’s benefits began with that first 2017 paycheck, and it’s a good idea to review that paystub to ensure that your deductions match your benefits confirmation statement. If you have questions or spot a discrepancy, please contact your human resources specialist.

NOMINATE A COMMUNITY NURSE FOR SALUTE TO
NURSES • • • It’s time once again to put the spotlight on
exceptional Community nursing. The annual Salute to Nurses
program is just around the corner, and StarMedia is accepting
nominations through January 31. It’s the program’s 15th year of recognizing nurses who go above and beyond the call of duty.
You can nominate a Community nurse in one of six categories: Advancement in Nursing, Extraordinary Caregiver, Community
Outreach, Inspiration, Lifetime of Compassion and Nurse Educator. The selection committee reviews every nomination

GRANT WILL HELP COMMUNITY SERVE MORE STUDENTS • • • Community Health
Network Foundation is the proud recipient of a grant totaling $130,300 from The Glick Fund, a
fund of Central Indiana Community Foundation, to support a comprehensive student health
clinic at Phalen Leadership Academy @ 103 on the east side of Indianapolis. Medical and
behavioral health services provided by Community professionals at the clinic—much like
Community’s clinical partnership with a number of local school districts—will be offered at no
cost to students and their families. Thanks to this grant, Community can reach more underserved youth in our communities,
partner with their families to better manage health concerns and address students’ health needs, and enable more children
to spend more time learning in the classroom.
HOLIDAY HELP PROGRAM A SUCCESS • • • The holiday season has passed,
and the generosity of the Community team helped make it a merrier time for
more than 1,100 children of Community employees. More than 300 families
requested assistance through the annual Holiday Help program, and employees
from across the network stepped up to purchase gifts for the kids in those
families. Thanks to all for being part of Community’s culture of caring and strong
relationships!

HOWARD AND EPIC: BY THE NUMBERS • • • The Howard region will soon begin using our CareConnect Epic Systems
applications for their electronic medical records. During the past several months a team of people at Community Howard
and across the network have been working with knowledge management to make this a reality on February 1. Let’s look
at the numbers:
• 270,000 patient records will be added to Community’s Epic system from the
Howard region.
• Those records include information about 3.4 million patient encounters.
• 350 providers from the Howard region will be better able to deliver seamless
patient care across the network.
• 400 training classes will have been held by go-live day to train the providers
and operations staff on using the Epic applications.
• 75 nurses will head to Kokomo on February 1 to help users when Epic goes live.
WELLNESS PAYS OFF • • • The first fitness challenge of the 2017 wellness program
is wrapping up this month. If you haven’t yet started collecting wellness points, it’s a
good idea to check your options and get started, because we’re now in the program’s
fourth month. To earn the full $750 reward—to be deposited into your 2018 health
reimbursement account or health savings account to help cover your medical
expenses in 2018—you’ll need to collect 1,000 points in the 2017 program, and the
deadline is the end of September. The key to success with this program is making the
effort by taking part in wellness and preventive screenings, joining fitness challenges

CONNECTING AND NAVIGATING THE PATIENT EXPERIENCE • • • Team connections over the course of a patient journey
are critical for ensuring that patients and families have a positive experience. Navigating through the plan of care can be
complicated and confusing, especially for those patients who are going through serious and possibly life-changing events.
This is particularly true for a newly diagnosed oncology patient and his or her family. Here at Community, we ease the stress
associated with a new cancer diagnosis by partnering patients with a navigator. These navigators help explain complicated
terms and make themselves available for questions after a physician visit. The navigators also help coordinate the patient’s
care by ensuring appropriate timing of appointments. Navigators connect not only with patients but also with multiple
departments along the patient journey. Keeping lines of communication open is essential for a smooth and steady care
experience. That might include updating the radiology technician or informing the emergency room nurse about a crucial
piece of information.
Our 25-person oncology navigation team
includes site-specific RNs who specialize in breast,
gastrointestinal/colorectal, head/neck and lung,
gynecological, genitourinary and cutaneous cancers.
Community has general oncology navigators
to follow other cancer types. While navigation
at Community started for the oncology patient
population, it is spreading to other areas of the
organization. Several physician care sites now offer
primary-care navigation for chronically ill patients.
How much of a difference does navigation make?
To quote one cancer patient: “To know you have a
person besides your family in your corner to make
sure you understand and can navigate the system
is just fabulous!”

CommunitySUPPORTING THE UNITED WAY • • • Community is traditionally one of the
biggest supporters of United Way—and if you’ve not supported United Way
before but agree to provide at least a dollar a week, Community matches your
gift! There are also match opportunities to help amplify new or increased pledges
from leaders. Our campaign supporting the United Way of Central Indiana is
ongoing, and we’re making good progress toward our goal of raising at least
$350,000. Meanwhile, Community Anderson has completed its campaign
benefiting United Way of Madison County, and Community Howard’s recent
campaign has collected donations of more than $50,000 for United Way of Howard County.
NEW YEAR, PLENTY OF SERVICE OPPORTUNITIES • • • You may have already
made a number of resolutions for improving your health and your life…perhaps more
exercise, maybe better eating or an earlier bedtime. How about adding a resolution to
help others more often, too? Our Serve360° program needs helping hands all year, and
while participating once a year is great, volunteering on a regular basis is even more
rewarding. In fact, the Indianapolis-area food pantries we regularly support are busier
than ever this time of year, so the need for volunteers is high. To learn more or register for

NORTH CANCER CENTER GETS NEW LEADER • • • Jim Hart, PharmD, BCOP, has been named executive director
for Community Cancer Center North. He joined Community in 2013 and most recently served as director of oncology
pharmacy. He also provided interim operational leadership at Community Cancer Center South. Prior to joining the
Community team, he spent five years as a clinical pharmacy specialist at MD Anderson.
