11. Issues regarding particular injuries sustained by individual deceased.

In November 2007, relatives of the 52 victims of the July 7 bomb attacks were sent by the

coroner at the time, Dr Andrew Reid, graphic and explicit details of how their loved ones

died. The documents were sent, unsolicited, and without prior warning, causing a great

deal of distress to bereaved family members. Dr. Andrew Reid was accused of gut-

wrenching insensitivity and Graham Foulkes, the father of 22 year old victim David

Foulkes, noted that there were "fundamental" differences between what they saw when

they viewed their son's body and what the report said.1

How many other bereaved family members have noted fundamental differencesbetween what they saw and what was written in the post-mortem reports delivered to

them? How did this major discrepancy arise? What has been done to address these

discrepancies such that the post mortem report accurate reflects the reality upon which

it is meant to be based?
