Introduction

The World Health Organization defines eHealth as the use of information and communication technologies for health. It supports the delivery of health services and management of health systems to become more efficient and effective. eHealth is described also as a means to ensure that “the right health information is provided to the right person at the right place and time in a secure, electronic form to optimize the quality and efficiency of health care delivery, research, education and knowledge. The application of information and communication technologies in health has rapidly increased for the past years and gained significance not only in the Department of Health but in the entire health sector. The DOH has continuously addressed the challenges and demands to further improve health care service deliveries and outcomes. Many countries have recognized the importance of adopting information and communication technology in health, also called as eHealth, to optimize processes and improve data collection, processing and analysis. The adoption of ICT has provided concrete foundation for health investments and innovations. Countries have formulated their own eHealth agenda to establish direction and plan the necessary steps to achieve their intended vision, mission, and goals.

The Department of Health has recognized the crucial role it plays in formulating and implementing the eHealth agenda for the Philippines. It has embarked on developing and implementing computer-based systems since 1988 and played a pivotal role in maintaining the health systems. In the same year, eHealth has been in the health systems and services from the time the Field Health Services Information System was developed and implemented through the support of World Health Organization. The DOH has gone through experimentation and early adoption phase where systems development using ICT are project based and time limited, proof of concept pilots, systems not connected to each other and rarely sustainable. Most systems failed because of the lack of infrastructure, skills and expertise in implementation, and lack of ownership. System development was driven by aids or supports from development partners donors, non-government organizations, and consultants resulting to proliferation of several vertical systems. overburdening service providers and bringing about sustainability issues and wastage of resources.

The application of eHealth in the Philippines has continuously advanced and yielded considerable benefits to an individual and public health. ICT has been used to improve the timeliness and accuracy of health reporting to facilitate monitoring and surveillance of diseases and injuries, among others. As DOH learned from the results of experimentation and early adoption phase, it has moved to increasing development or building up of application or information systems, began to adopt the use of standards to address the weaknesses of experimentation and early adoption of ICT, started addressing the issues on enabling environment, and promoted increased awareness of ICT in the health sector. Further, the DOH is also in its scale up and mainstreaming phase where there is a broad uptake of ICT by the general public and health professionals as well. Critical challenges are faced by the DOH as the general public became more aware and exposed to different services using ICT, or e-services. There is greater demand for efficiencies and high expectation from the DOH to deliver quality care, services and outcomes. The DOH and different stakeholders of the health sector have recognized the important roles they play from planning to utilizing eHealth in the country.

The National Objectives for Health, 2005-2010 and the 2011-2016 prioritized the use of ICT in various reforms areas, critical health programs, and specific areas in health administration. In 2005, the Philippines was signatory to the 58th World Health Assembly Resolution that advocated the following:

o Draw up a long-term strategic plan for developing and implementing eHealth services in the various areas of health sectors including health administration which includes an appropriate legal framework and infrastructure and encourage public and private partnership;

o Develop the infrastructure for ICTs for health as deemed appropriate to promote equitable, affordable and universal access;

o Build on closer collaboration with private and non-profit sectors in ICTs;o Reach communities, including vulnerable groups, with eHealth services appropriate to their

needs;o Mobilize multi-sectoral collaboration for determining evidence-based eHealth standards and

norms and to share the knowledge of cost-effective models, thus ensuring quality, safety and ethical standards and respect for the principles of confidentiality of information, privacy, equity and equality;

o Establish national centers and networks of excellence for eHealth best practice, policy coordination, and technical support for health-care delivery, service improvement, information to citizens, capacity building, and surveillance; and

o Establish and implement national electronic public-health information systems and to improve, by means of information, the capacity for surveillance of, and rapid response to, disease and public-health emergencies.

With this as input, together with the national development plans, government-wide ICT plans, assessments of ICT programs and health information systems, and priorities of the health sector, the DOH has updated its e-health framework to support Universal Health Care or Kalusugan Pangkahalatan. The National Objectives for Health 2011-2016 shows the commitment of the DOH to make KP works. It sets the health program goals, strategies, performance indicators and targets for the health sector to achieve KP by 2016, or transport the health sector to its desired outcomes or results. The said NOH includes the various roles that different stakeholders play in shaping the future of the health systems in the country and bringing better health outcomes to the Filipino people.

Reference is made to the introductory part of the NOH in terms of vital information about the Philippines, Philippine Health System at a glance, and the Universal Health Care or Kalusugan Pangkahalatan (KP). The Philippine Health Information System 2010-2016 Strategic Plan presented the major sources of health information; highlights of the assessment of the PHIS using the Health Metrics Network framework, assessment of the Field Health Services Information System and Vital Registry through a survey, and the National Statistics Office assessment of its Civil Registration System. By this point, the national eHealth vision, mission, priority focus areas or strategic goals/objectives, and strategies have been formulated by the DOH.

The Department of Science and Technology (DOST), through Executive Order 128, provides central direction, leadership and coordination of scientific and technological efforts and ensure that the result there geared and utilized in area of maximum economic and social benefits for the people. The functions and responsibilities of DOST expanded correspondingly to include the pursuit of declared state policy of supporting local scientific and technological effort, development of local capability to achieve technological self-reliance, and encouragement of greater private sector participation in research and development. There were e-health initiatives undertaken by the Philippine Council for Health Research and Development of the DOST, like the Health Research and Development Information Network (HERDIN) of the, eHealth Philippines (Philippine Electronic Health Information Village), eHealth portal and others.

To implement a national eHealth in the country for greater efficiency in health care, workforce productivity, and optimized use of resources, the DOH and DOST signed a Joint National Governance on eHealth. This shows the commitment of both national government agencies to unite towards finalization and implementation of the country’s eHealth Strategic Framework and Plan or Roadmap. Activities, programs and/or projects shall be undertaken collaboratively to maximize government resources, achieve convergence of infrastructure, and deliver quality health care.

Opportunities and Challenges

ICT has become the cornerstone to improve data collection and processing. It has been applied to health in various areas like epidemiology, demography, hospital processes, data collection on TB, malaria and other diseases. The use of ICT could lead to a safer, more efficient health care system, but there remain key OPPORTUNITIES and CHALLENGES to achieve organizational efficiency and if not properly addressed, lead to increased investment. Opportunities and challenges are sound basis for formulating the action lines to group the activities for better prioritization, focus, targeting and resource requirements in the eHealth Plan.

Opportunities are as follows:

1. There are improvements in the DOH e-government capabilities as identified in the ICT Competency Maturity Model Assessment Report - January 2010 conducted with the Asian Development Bank in collaboration with the Commission on Information and Communication Technology.

2. There is strong leadership and management support in the DOH.

a. The Aquino Health Agenda or KP is being implemented with ICT as a major enabling tool and the DOH has begun the use of ICT in several application or information systems like web-based disease registries reporting systems, hospital and clinic information systems, telemedicine, mobile applications, and others.

b. Health Information is one of the six (6) strategic instruments that shall be optimized to achieve the Aquino Health Administrations’ strategic thrusts. It is an instrument to establish a modern information system that shall provide evidence for policy and program development, and support for immediate and efficient provision of health care and management of province-wide health systems.

c. Increased budget to fund ICT infrastructure, systems development, and other projects.

d. Presence of Health Enterprise Architecture, eHealth Strategic Framework, National Health Data Dictionary, and DOH Information System Strategic Plan to guide the development and implementation of application or information systems for better sharing and access to data, facilitate consolidation of systems and architecture, promote interoperability by providing a means for

communication and coordination of electronic data among the various health domains, increase accountability and ownership of data and information, harmonize and optimize processes and workflows to achieve integrated health care, serve as reference in the development of integrated information systems, and promote the use of standards to support the objectives of the Department.

3. Improved collaboration with the different government agencies, institutions, and multi-sectoral groups as evidenced by the creation of the Philippine Health Information Network, Philippine Network for Injury Data Management System, Philippine Council for Health Research and Development – Department Of Science and Technology e-health initiatives, ICT4H Consultative Group, PhilHealth and DOH ICT Committee, National Statistics Office - DOH Collaboration, Memorandum of Agreement with the National Statistics Office for access and harmonization on mortality data and improvement of Civil Registrations on deaths and births, Human Resource for Health Network Philippines, National Law Enforcement Coordinating Committee, Joint DOH-DOST National Governance on eHealth, ongoing Memorandum of Agreement with the Information and Communication Technology Office – Department of Science and Technology iGovPhil, Memorandum of Agreement with National Mapping and Resource Information Authority for Health Facilities coordinates gathering and access to data and maps of the Philippine GeoPortal, among others.

4. Presence of standard policies, procedures and guidelines governing all ICT works in the DOH that directs all central and regional offices, and retained hospitals to submit all ICT-related works to IMS for evaluation. The evaluation process checks on compliance to the ICT standards of the DOH to ensure continuing maintenance and/or sustainability, reduce duplication of information systems, maximize resources, and implement efficient and effective solutions to address existing problems, issues and concerns.

5. The usefulness of ICT in the DOH and the health sector is widely recognized and its utilization has greatly improved over the past years. There are increases in the number of computerized application or information systems developed and implemented to address the critical or frontline services, and significant increases in number of personnel trained on use of ICT and ICT equipment deployed.

6. Availability of cheaper technologies using biometrics, barcode, Short Messaging System, and other mobile technologies, Voice Over Internet Protocol, video conferencing, and others that can address the requirements of ICT in Health. The DOH can capitalize on relevant emerging cost-effective information and communication technologies to support the strategic goals and thrusts of KP.

7. Certification of the DOH in ISO Quality Management System to institutionalize good governance systems.

8. The Health ICT Industry is now active and has a lot of solutions and services to offer. There is a wide uptake of ICT by the general public and health providers are already aware of the benefits or advantages of using ICT. The general public is now exposed to e-services and expects results in terms of efficiency and effectiveness.

9. Signed Joint DOH-DOST National Governance on eHealth to finalize and implement the Philippines eHealth Strategic Framework and Plan/Roadmap.

Challenges are as follows:

1. Though the usefulness of ICT in the DOH and the health sector is widely recognized and its utilization has greatly improved over the past years, the momentum has to speed up to ensure that it becomes a responsive strategic instrument to KP.

2. The use of ICT in the DOH has to be further exploited to enhance productivity, improve efficiency, and ensure greater transparency, effectiveness and accountability in operations. There is an urgent need to implement or adopt the Health Enterprise Architecture and National Health Data Dictionary to facilitate and/or improve access to and sharing of health, medical information, knowledge and locally relevant content resources for strengthening public health research, disease prevention and control, regulation of services and policy development.

3. With additional funds from government and development partners to support the ICT strategic goals and direction, including the availability of contractual personnel, the DOH has to adopt ways to better cope up with the challenges of duplication and overlapping, integration or harmonization of data/information/systems, implementation technical support, and maintenance and sustainability including rational use.

4. Continuing harmonization and scaling up of information systems on hospitals, routine public health service statistics reporting, drug price monitoring, disease registries, health human resources and information management systems and internal management support systems such as e-NGAS, expenditure tracking, procurement and logistics management.

5. Health care and health information and/or knowledge have to be extended to remote and underserved areas and vulnerable populations to promote equitable, affordable and universal access to health care and knowledge.

6. The DOH has to continuously promote and/or expand further its collaboration efforts in using ICT with other governments, planners, health professionals and other agencies along with the participation of international organizations.

7. Priority initiatives have to be decided upon with the many possible use of ICT in health. The rapid innovations in ICT and exposure to international health information technologies have resulted to a big demand to use ICT in the health sector.

8. Establishment of a knowledge hub for exchange and utilization of data/information, and use of ICT to provide better health services to reach Geographically Isolated and Disadvantaged Areas and to support MDGs attainment especially in high maternal and neonatal mortality areas.

9. In a devolved setup, management support at the national, regional, and sub-national levels is critical to ensure acceptance and commitment to support implementation

of health information systems. Continuing orientation, education and training, progressive status reporting, monitoring and evaluation have to be institutionalized.

10. The DOH will continue to harness web technologies for public health surveillance and reporting, access and networking with experts, and access to relevant databases and even reaching citizens and clientele. The ICT strategy to meet the requirements of the health sector will continue to shift to web-based technology, and will incorporate biometrics, barcode, Short Messaging System, and other mobile technologies, Voice Over Internet Protocol video conferencing other cheaper appropriate technologies.

11. Continuing upgrades, enhancements or rehabilitation of ICT-infrastructure especially at sub-national levels (centers for health development and DOH hospitals) to improve intra and inter communication, data management and facilitating regulatory and service delivery functions.

12. Increase in storage capacities, performance and security to accommodate the requirements of the entire health sector.

13. Updating of laws and regulations on health information, formulation of overarching policy for health information system, and harmonization of plans geared towards use of ICT or eHealth.

14. Ensuring that ICT is considered as just one of the primary tools and not a total solution to most problems by over and eagerly appreciative techie management and technical enthusiasts; and thus ensuring that e-health activities are coherent, harmonized, cost efficient and serve its intended purposes.

15. Implementation of Health Information System Standards / Health Information Technology Standards towards Health Information Exchange (National Health Data Dictionary, Health Enterprise Architecture, National Implementation of Health Data Standards for Interoperability, and Implementation of Software Certification in compliance to National Health Data Reporting).

Purpose

The Philippines eHealth Strategic Framework and Plan (PeHSFP) describes how the eHealth vision will be achieved to guide national coordination and collaboration, and sets clear direction and guidance to the ongoing and future eHealth activities in the country.

Section 1.0 - eHealth Context

The DOH has learned from the results of experimentation and early adoption phase since 1988, the start of developing software for Field Health Services and Information System, and has continuously developed or built other application or information systems. In Chapter VI of the DOH Health Enterprise Architecture – Version 1.0, “the use of ICTs in the DOH has remarkably supported and improved some of the functions of the Department. ICTs have been used in the areas of innovative technological changes, networking and infrastructure, office automation, development and implementation of computer-based systems. From the limited resources in terms of ICT personnel and funds, the DOH Management has augmented the budget on ICT to fully accomplish and support the ICT strategic goals and direction. Contractual ICT personnel are hired to render technical assistance or help desk support in the implementation or operationalization of several information systems in the DOH”.

In terms of standardization, the DOH through the Information Management Service has formulated standard policies, procedures, and guidelines governing ICT works in the Department through Department Order (DO) No. 2005-0032. The Order directs all central and regional offices, and retained hospitals to submit all ICT-related works to IMS for evaluation on compliance to the ICT standards of the Department. This aims to ensure continuing maintenance and/or sustainability, reduce or eliminate duplication of information systems, maximize resources, and implement efficient and effective solutions to address existing problems, issues and concerns.

Existing information systems and data sources are being integrated or harmonized to eventually address other challenges like establishment of the DOH data warehouse, quality database and establishment of a more responsive information system and access to and sharing of knowledge products. For remote and underserved areas and vulnerable populations, the DOH has implemented telemedicine in selected pilot areas through the National Telehealth Center, National Institute for Health, University of the Philippines, Manila. The DOH has also developed and implemented mobile technology solutions in reporting cases through the Health Emergency Management Staff’s - Surveillance in Post Extreme Emergencies and Disasters (SPEED) with support from WHO. There are several mobile technology applications developed and for implementation, e.g. Text TB for reporting inventory of tuberculosis drugs, maternal and neonatal death reporting, and routine health data reporting.

The DOH has created networks for increased collaboration on health information systems and eHealth applications. These are the Philippine Health Information Network, Philippine Network for Injury Data Management System, and ICT4H The networks consist of representatives from the different government agencies, private firms or organizations, local government units, non-government organizations, academe, research institutions, international organizations, and others with varying roles and responsibilities as far as the health agenda is concerned. The networks have one common goal, i.e. to create a more reliable, timely, high quality and affordable health care and useful health information systems, promote continuous medical training, education, research, and others through the use of ICT with respect and compliance to security and protection of citizen’s right to privacy. The networks vary from the degree of engagement , e.g. PNIDMS for injury related cases (DOH DPO No. 2011-0047), PHIN for health data collection, sharing and exchange, systems harmonization and other collective actions to improve health information systems (DOH DPO No. 2006-0452 - Creation of Management Structure for the PHIN) , and ICT4H for the development of the health sector enterprise architecture, HIS/HIT standards and compliance and capacity building. (DOH DPO No. 2010-5245 Technical Working Group on ICT for Health). There are some eHealth solutions or applications at various levels of maturity and/or stages of development and implementation.

Several opportunities were identified from different undertakings and perspectives. In the case of Health Information System, a forum was conducted in Manila last June 2011 and participated by nine countries and development partners. Seven (7) priority gaps or opportunities were identified, i.e. government and multi-sectoral engagement, policy and regulation environment, strategic planning/financing, Human Capital Development, Systems and Data Interoperability, Infrastructure, and Information Use. The PHIN with WHO and USAID support have also brought down the interagency HIS Forum at sub-national levels in two DOH Operations cluster and hoping to cover the two others this 2013.

At present, the context of National eHealth in the Philippines are the continuous development and build-up of eHealth systems like health information systems, telemedicine, adoption of electronic medical record systems, mHealth, knowledge management systems among others; and the scaling up and mainstreaming, as evidenced by the creation of the Philippine Health Information Network, ICT4H, Philippine Network for Injury Data Management System;

development of the Information System Strategic Plan, Philippine Health Information System Plan, and Health Enterprise Architecture; development of health application and information systems; KM hubs and works done to define and implement national health data standards.. The health ICT industry is now active with new business models and competition, new businesses and economic opportunities, new platforms for innovation and services, integration or harmonization of health information systems, eHealth systems in hospitals, chronic disease management applications, among others.

Since efforts were already started in implementing ICT in health as well as developing and building up eHealth solutions, eHealth can now move to expansion and mainstream services to further improve consistency in the use of standards, efficiency and quality outputs. More focus will be on establishing or updating interoperability and adoption standards, software certification, innovating services and reporting systems to meet the expectations of the citizens for more people-centered, efficient, and effective services, and undertaking monitoring and evaluation to ensure that eHealth delivers as expected.

The PeHSFP is the result of comprehending what the Philippines needs to achieve in order to address its health goals and challenges. The contents can be regularly updated to remain applicable or appropriate. The key to keeping updated the PeHSFP, as well as success in implementing the national eHealth agenda, is the continuing collaboration and communication with the different stakeholders to gain their support, guidance, cooperation and commitment.

Section 2.0 – Philippines eHealth Strategic Framework

The DOH and DOST shall endeavor to effectively use ICT to support planned improvements on health care delivery, administration and management, and in providing health services and communicating health to the Filipino people. Such intentions are contained in the National eHealth Strategic Framework, 2010-2016 which defines the five strategic goals on how the DOH will use ICT. The first goal is the needed inputs, conditions and configuration to support achievement of the four other goals (ISSP, 2011-2013).

Strategic Goal 1: To provide a rational and accountable eHealth agenda, legal and normative framework and structures to implement eHealth. This is translated into major inputs and process such as:

o A legal, policy or normative framework, and a definite road map to guide development and use, including the authority to impose certain standards and policies and compliance to national agreed standards. Currently, there are pending bills in Congress that are envisioned to support these such as “An act providing policies and prescribing procedures and systems on surveillance and response to reportable diseases, epidemics, and other public health threats and emergencies, and health data and information management and providing funds therefore and for other purposes,” “Computerization of Philhealth” “Philippine Statistical System” and Executive Order for Philippine Health Information System and the Philippine Health Information Network. Also, organizationally in the DOH Rationalization Plan, there will be a creation of National Center for Knowledge Management for Health. There is also a need to align this policy and strategic framework with KP.

o A long-term eHealth strategic road map that will provide structured time-frame, outcome, process and input indicators and annual targets must be developed. Translation of the road map to three to five years (medium term) plan that will state strategies and activities, timelines for each strategy/activity and indicators to track progress is needed. Also, conversion to annual plans pinpointing specific stakeholder accountabilities and priority programs and projects is in order.

o Standards development in terms of technical infrastructure, health informatics, statistics, interoperability, patient identifier, provider identifier, procedures, patient record, drug, security, data exchange, diagnosis, data aggregation, among others. There is a need to review, develop and adopt these relevant eHealth and related standards for interoperability (i.e. data content, vocabularies, terminologies (ICD 10, SNOMED, others), information content standard, information exchange standards such as messaging and HL7, EHR/EMR, National Health Data Dictionary (semantic and syntactic), identification and identifiers). Moreover, core data sets, specific data set, data quality, data use, data privacy and security protocols must be established.

o Increased and/or sustained financing or budget must be in place. Rationalizing use, having definite plans and definite outputs must also be in place.

o Health human resources availability and e-maturity. The following must be defined and put in place: Definition of basic ICT-competency requirements for each HHR; Definition of specialized e-health competencies for various application systems, HIS, KM, Telehealth and mhealth; development of capability training or capability programs including accreditation or adoption of ICT/eHealth/ medical informatics programs and institution and experts certifications.

o Multi-stakeholder collaboration and networks. This includes strengthening and/or expansion of the Philippine Health Information Network; public private partnership; networks of data producers or generators such as local civil registrars, public health nurses and midwives, association of municipal heath officers and systems implementers such as Health Information Technology Professionals, Systems Administrators, and user-groups such as for specific systems like for injury, PWD, VAWC, disease surveillance, geomaps/GIS, drug price monitoring; and KM workers.

o Continuous provision of technical support in implementing and sustaining systems. For the local government units, certain required systems must be incorporated in the Province-wide Investment Plan for Health and their Service Level Agreements with DOH; There is a need to develop sustainability plans with implementing units; social preparation and continuous advocacy and ensuring systems utilization and ensuring value added benefit to local health agencies.

o ICT infrastructure. This will include identification of relevant, responsive, simple and cheap technology solutions. More so, replacement and upgrading of the old ICT infrastructure at the central office; phased regional offices and hospitals’ ICT upgrading; and providing sub-national health facilities with an initial set of

computer system to implement required reporting or information systems. Social protection support initiative with DSWD and PhilHealth to support CCT beneficiaries and universal health insurance coverage and disease surveillance likewise are to put in place. There is also need to improve communication system for daily health administration and service delivery, support health emergency preparedness and response, and increase office automation with a ratio of 1:1 for management and technical staff and 1:2 for administrative staff. The central office and CHDs must have an enhancement of storage, redundancy and security capacities and electronic connections. The infrastructure must primarily support requirements to implement KP priorities and health related laws such as the hospital reform on facility enhancement, fiscal autonomy and services enhancement; health care financing and good governance, health emergency, Dangerous Drugs Act, Generics Act, National Voluntary Blood Service Act, Universally Accessible Cheaper and Quality Medicines. Most of these requires a national initiative and private sector participation to make it happen and affordable for DOH and local health facilities.

o Research and development of what are applicable and what work best in terms of technology solutions, technology standards and related health domains and in system development and e-health related standards.

o Monitoring and evaluation system for the road map and return of investment. This will require development of Monitoring and Evaluation Framework and putting up the system; development of eHealth programs and specific office score cards; harmonization and alignment of the eHealth M&E with the ME3 and KP score card and Performance Governance System. Focus will be on: management of quality, IT performance, internal control, ensuring compliance with external requirements and providing IT governance

Strategic Goal 2: To increase efficiency of processes and systems in health care delivery and administration, and create new processes and forms of doing things. This is applying ICT in Critical Health Application Systems that are vital in achieving the strategic objectives of the DOH. Within the planning period, ICT will be used to make more efficient health systems based on health sector reforms, health related laws and governments and sector requirements: Social Health Insurance; Disease Surveillance; Essential Drugs Management and Price Monitoring; Health Emergency Preparedness and Response; Health Facilities Management including Blood Supply and Other Ancillary Services; Health Services Referrals; Health Regulation of Facilities, Products and Services; Health Records and Document Management; Health Promotion; and Financial, Procurement, and Material Management.

Strategic Goal 3: Establish a unified and coherent health and management information systems. This is establishing the Philippine Health Information System which is a collaborative effort to increase availability, access and use of timely, relevant and reliable health information through shared agreement on goals and coordinated efforts and investments in health information and knowledge system. It involves formal HIS assessments, inventory, integration of systems and policy development, enhancement of various data and knowledge sources/systems/structures, and improving access and use. This will establish an integrated or harmonized health information systems that includes various data sources for routine data reporting and vital registration as well as special surveys; adoption of a data management protocols; enhancing health services statistics reporting

and from public health facilities especially the LGUs and the private sector; scaling-up and improving implementation of information systems for health centers/rural health units, hospitals, PhilHealth, health human resources, and other information systems under KP; expanding data access and utilization for evidenced-based decisions and systems’ sustainability; and instituting a formal information governance within DOH initially with the necessary tools and procedures by providing a framework, requirements, and standards in handling of information such that it is obtained, recorded, used and shared in a manner that is appropriate and effective, secure, accurate and reliable, ethical, and lawful.

Strategic Goal 4: Institutionalize knowledge management systems to promote knowledge exchange and utilization especially at sub-national levels. A Knowledge Management for Health framework and plan shall be updated and further implemented. Capacities for K production, use and application will be improved in particular in health research, knowledge translation and other means. Continuously improving access, exchange and sharing will be prioritized with K portal and dashboard, Intranets, call center; data warehouse; e-library; shared and better access to national and international databases and publications, and best practices and establishing CoPs or K networks. Tools, K hubs or learning and resource centers will be further improved and standards, and repeatable processes and procedures will be continuously promoted.

Strategic Goal 5: Capitalize on ICT to reach and provide better health services to GIDA, support MDGs attainment and dissemination of information to citizens and providers. Telemedicine and mhealth services will be fully tested and scaled–up once technologies and systems had been efficiently feasible and cost-effective. Limited use at this point is experienced mainly because of limited ICT infrastructure and organizational financing and technical structures. A policy and strategic framework will have to be developed and implement systems to reach GIDA, and to support MDGs attainment such as patient monitoring or tracking (WOMB-Watching Over Mothers and Babies), referrals, health providers networks, connecting health providers and patients, diagnostic services, and others.

The overall strategic goal toward which this PeHSFP is endeavored is to:

Establish harmonized, quality, relevant, responsible and responsive eHealth services to provide the necessary tools, data, information, and knowledge for evidence-based policy and program development towards the provision of accessible, quality, affordable, efficient, and safe health services and attainment of better health outcomes for all Filipinos.

eHealth Objectives

The major reasons for using ICT in health are summarized below. These have been the focus of the ICT works and will continue for the next five (5) years, concentrating also on integration/harmonization, and standardization within the health sector in general:

o Provide accurate and timely information, and feedback on performance for decision support through better environmental scanning and program planning, development, implementation and monitoring. This could be done by online reporting and exchange of data, utilizing common databases, shared networks, better user access to internal and external data/databases/information/knowledge and networking with international and local experts, and other stakeholders; and establishing more communities of practice, and health data warehouses.

o Provide better, accessible, appropriately delivered and quality health services, and accessible and effective medical products and technologies especially among the poorest quintile and marginalized sector through better targeted programs, outcome monitoring, up-to-date health products, services, facilities, and providers; regulation and monitoring; health facility and providers networking; vertical integration of different levels of health care, horizontal integration of health care within economic and social structures; better coordination and implementation of disease surveillance and epidemic, disaster preparedness and response; provision of clinical information support; multimedia health promotion; including telemedicine and mobile health; global connectivity for disease surveillance, and access to new products, markets and technologies; monitoring and response mechanisms for disease outbreaks and disaster management; better development, management and implementation of various health services.

o Promote administrative efficiency and productivity or good governance of the health system by increasing staff productivity, providing tighter internal controls, displacing material and operational cost, providing organization flexibility in managing health programs, better management accounting of resources available to DOH and providing value-added services and administrative support to health program development and provision of preventive, promotive and curative services. Examples of these are providing access to alternative quality learning processes and dissemination of information and knowledge where affordability and geography are barriers.

Increase efficiency of processes and systems in health care delivery and administration through new and better application systems and processes for Philhealth claims, health records, disease surveillance, drugs management and price monitoring, financial and procurement management and other regulatory support services.

3 Philippine Health Information System

The Philippine Health Information Network shall work towards harmonization and integration of data sources and information systems using acceptable data management standards and protocols, and support initiatives that will enhance health service statistics reporting especially from LGUs and the private sector. This will also include improvements and scale-up programs for information systems in health centers, hospitals, and other critical information and service delivery centers and support service groups.

4 Knowledge Management for Health

Greater and better capacity for exchange and utilization of knowledge resources and systems especially at the sub-national levels based on KM4Health framework. The latter encompasses health research, knowledge translation, including knowledge sharing and exchange programs such as call centers, e-library, best practices, Communities of Practices or K networks among others.

5 Telemedicine/mhealth Services

ICT capacities are maximized in reaching GIDAs, attaining MDGs and in collecting data and disseminating health information to policy makers, provider and citizens. Patient monitoring and tracking programs for MDGs such as maternal and neonatal events and TB treatment compliance, and disaster response shall be given priority focus.

Other Terms: eHealth Development Plan, eHealth Roadmap

The Department of Health has an existing eHealth Strategic Framework where the over-all goal, strategic objectives, indicators, targets, and strategies for 2011-2016 are defined under Chapter 6, Section 6.2 Health Information System of the National Objectives for Health – Philippines 2011-2016. There is a need to reinforce the framework, revitalize its contents, and fortify the national plan to ensure greater success in achieving the eHealth goals and objectives. The methodology used to enhance the existing eHealth framework and plan is as follows:

1. Review of the national health priorities of the country; current eHealth context; assessment, findings and recommendations; planned strategies and activities; and environment to gain better understanding and focus (Document Sources – Universal Health Care or Kalusugan Pangkahalatan, NOH 2011-2016, DOH HEA Version 1.0, PHIS Strategic Plan 2010-2016, Information System Strategic Plan 2011-2013, ICT4H Findings and Recommendations, and National HIS forum country commitments.

2. Review of the existing or published eHealth Framework, vision, mission, goal, and objectives.3. Review of the existing priority focus areas and identification of activities required to deliver

the national eHealth vision.4. Formulation of action lines to group activities for better prioritization, focus, targeting and

resource requirements.5. Consolidation of various assessments, evaluations, recommendations, findings, and

commitments into activities for each of the action lines.

Section 4.0 - eHealth Action Lines

The PeHSFP has five (5) components to support KP, i.e. Enabling structures and resources, mission critical health application systems, Philippine Health Information System, Knowledge Management for Health, and Telemedicine/mHealth Services. The existing components were reviewed, together with the current national eHealth context, and accomplishments done by the DOH and DOST as far as the eHealth agenda is concerned. In developing the PeHSFP, action lines were formulated to group activities of similar focus and intent that are necessary to deliver the eHealth vision. Action lines and descriptions are as follows:

1 Governance This is the exercise of authority to lead and manage the entire eHealth agenda. The eHealth vision is national in context and requires sustained leadership and commitment from the health sector leaders like government officials and senior officers. A governance structure and mechanisms are needed for coordination of efforts, identification of functions and expertise, and clear roles, duties and responsibilities in achieving the eHealth vision.

strong emphasis is placed on Health Sector Architecture Governance. The HEA serves as guide to ICT development for the health sector and it is necessary to consider also architecture governance. Architecture governance is the management and control of the HEA at an enterprise or health sector level and requires the involvement and active participation of high level officials from the DOH.

A governance structure is also defined in the Draft Philippine Health Information Network and other existing networks like the Philippine Network for Injury Data Management System. Existing governance structures will be reviewed and harmonized into one Organizational Structure. Included in governance is networking for collaboration with the national agencies, local government units, private, non-profit sectors, and other concerned stakeholders in ICT to improve further the health services to the public.

2 Legislation, Policy and Compliance

This is the formulation of relevant proposals and discussions with concerned legislators to support the eHealth agenda with the end objective of enacting laws and executive orders (e.g. Draft Executive Order of the PHIN on governance); and formulation of relevant policies to serve as principles, rules, procedures or protocols to guide the development and implementation of eHealth. This supports the implementation of the national eHealth agenda and environment like national legislation, regulations, policies at the national level and specific to applications and services, and compliance to national eHealth standards for interoperability and data exchange, among others.

3 Human Resources These are the workforce needed to carry out the activities of the eHealth agenda, i.e. the people needed to act, sustain, enhance, operate or implement the activities. They can be the health personnel or the health ICT personnel. The primary objective of this action line is to develop, share and implement eHealth literacy and capability building programs to achieve the eHealth strategic goals at all levels or categories, i.e. from the national, local government units, health education institutions, private groups or organizations, corporations, non-profits/non-government organizations, those involved in health delivery services.

4 Standards and Interoperability

Health data can be moved from one location to another with the sender and recipient having the same understanding and analysis throughout. To achieve this kind of scenario, data must be standardized as well as communication standards.

i.e. communication standards ensure that health data can be sent from one location to another in its original form and complete in contents. Data standards ensure that data has similar meaning and understanding to the sender and recipient.

This enables accurate and reliable collection and exchange of health data or information across entities or the health sector, and use of varied technologies that are able to send standard common data sets to the DOH. Standards include data codes, data structure, terminologies, messaging, security, system/software compliance, among others. One gadget can be used for several applications utilizing different technologies but still will be able to comply with the standard data requirements.

5 Infrastructure This refers to connectivity, computing infrastructure (e.g. computers, servers, and mobile phones), core services like identification and authentication, data sets, information systems, and data repositories, among others. This supports innovative and improved ways of delivering health care services and information, and the sharing of data/information across the health sector.

B Solutions Development and Harmonization

This refers to the development and harmonization of services and applications to address the health system goal of establishing a harmonized, quality, relevant and responsible eHealth services to provide the necessary tools, data, information, and knowledge for evidence-based policy and program development towards the provision of accessible, quality, affordable, efficient, and safe health services and attainment of better health outcomes for all Filipinos.

This supports the eHealth Resolution as recommended by the World Health Organization Executive Board to the 58th world health assembly that has recognized the potential impact that advances in ICT could have on health care delivery, public health, research, and health-related activities for the benefit of the both low- and high- income countries. This includes the establishment of national centers and networks of excellence for eHealth best practice, policy coordination, and technical support for health service delivery, improvements, information dissemination, capacity/capability building, and national public health information systems to improve surveillance and rapid response to disease and health emergencies through quality information.

This monitors the progress and impact of the activities to ensure that eHealth delivers according to plan or priorities. This also shows if outcomes have been achieved, measures the outcomes that are being delivered, identifies and corrects those that are not being realized.

The adoption or use of eHealth will be monitored and evaluated. Set of indicators, called as outcome indicators, will be defined to provide insights into the tangible results for implementers and stakeholders.

This refers to continuing research and development of new products and services, innovative eHealth solutions in electronic health records, information systems, disease registries, health informatics, m-health, and others that can be done in a collaborative or independent way. Results of the research and development process can serve as basis for new and continuing enhancements and upgrades to existing eHealth solutions.

The National Objectives for Health 2011-1016 has defined six (6) major strategies to achieve the intended vision, mission, and goal of eHealth in the Philippines, namely:
