RENTAL AGREEMENT
Equipment must be reserved at least 2 weeks in advance by calling the Resource
Specialist at 456-3978 or submitting a request online. If equipment is not reserved we
cannot guarantee that it will be available. Please give at least 24-advance cancellation
notice.
MANIKIN RENTAL: Manikin rentals are on a per class basis. Multiple classes will incur multiple
rental fees.
LATE FEE: An additional charge of $10.00 per day will be assessed if equipment is returned after
the designated “Return Date” indicated on the invoice or the front of the Equipment Request Form.
Equipment is expected to be returned within 3 working days after completion of class or certifying
event.
MAINTENANCE FEE: A cleaning and maintenance charge of $20.00 may be assessed for each
item that is returned which requires cleaning and/or maintenance which exceeds that resulting from
normal wear and tear. If the item is not repairable due to damage through negligent use you will be
responsible for the full replacement cost of the item and will be billed accordingly.
INSTRUCTOR/RENTER: It is YOUR responsibility to inspect the condition, quantities and
index numbers of all equipment and supplies prior to removing them from IREMSC. It is also essential
that you re-inspect each item with an IREMSC staff member upon return.
I, the undersigned, agree to return IREMSC equipment in the same condition as when it was checked
out, and to ensure that it is returned on the date indicated or coordinate other arrangements with the
IREMSC staff. I am aware that I will be held accountable for the full replacement cost of the equipment
if it is lost or damaged by my negligence.
