Integrity inspection On

Structure, Floating Buoy and wire rope, sling and shackle

of Snap on Buoy (SOB) and Gin Pole

Location : Matak , Jetty

Date : 30 April - 2 May 2005

1. Inspection Summary

1.1 VISUAL INSPECTION

1.) Performed Visual Inspection on structural components and floating buoy of

SOB to see any visible defects such as corrosion, dent, bent, etc.

During Visual Inspection on Structure found :

1. Female end Collect Connector/stubbing Cone at 4 Bottom Leg of SOBHeavy Corroded (See Picture – 2 )

2. 2 ea Lifting Pad eyes on structural Framing ( No: 1 and 2 ) Heavy

Corroded at reinforcement Pads (See Picture – 7 )

3. 2 ea lifting pad eyes on Floating Buoy ( No: 5 and 6 ) Heavy Corroded,

Fitted and Thin remaining thickness

4. One section of Floating Buoy at 6" dia support position Heavy Corroded

(See Picture – 10 )

5 All "U" bolt of 3" dia riser support corroded (See Picture – 6 )

6 Structural frame on bottom SOB found dented (See Picture – 4 )7 Hoses Guard Heavy Corroded (See Picture – 12 )

8 Gin Pole Frame Leg 2 elevation frame 1 and Leg 3 elevation frame 4 and

3" diameter of leg frame near elevation frame 1 need to be weld-out

(See Picture – 12 )

9 All Bolt Rubber fender Corroded (See Picture – 9 )

10 Connection flange joint SOB to Gin pole need to be tightened at leg 4 and

leg 3 ( See Picture – 11 )

11 Inside floating buoy for riser penetration found Heavy corroded

( See Picture – 11 )

12 Some area of SOB Framing found Heavy corroded ( See Picture – 5 )

2.) Perform visual inspection on all lifting gears such as sling, wire rope and

shackles for frayed lines, kinked areas and corrosion areas. Put color coding for

final acceptance process.

1 Wire rope on 4 ea winch found corroded and kinked specially wire sling outside

the winch. (See Picture –1)

note : only wire rope that outside from the winch that can inspected

2 Shackles on pad eyes at bottom SOB need to be replace. (See Picture –3)

3.) Perform visual inspection on Hoses

1 Found 3 location damages on hoses winch no:1 and no:2 ( See Picture – 15 )

1.2 NDT EXAMINATION

1. Clean all defect surfaces and welding at the structural frame, then inspect by

using Magnetic Particle Inspection

- Some corroded area cleaned and checked by MPI and not found any significant defect

2. Inspect the floatation buoy (tank) remaining wall thickness by using Ultrasonic.

- Minimum remaining thickness on floating buoy wall = 12.4mm

3.) Clean and inspect all weld area at floating buoy by using MPI .

- Removed paint on weld area and checked by MPI and not found any significant defect (See Picture –8)

4.) All the lifting pad eyes and the pad eye guides to be cleaned and inspected by

using MPI.

- Removed paint on weld area and checked by MPI and not found any significant defect

- Found Heavy Corroded and recommended to be replace

- Not possible to used ultrasonic test Cause all fillet weld joint designed

5.) Perform random NDT inspection to all lifting aids

- Performed Visual inspection on shackles on redeye's at Bottom SOB found Corroded

- Performed Visual inspection on wire rope of 4 ea winch found corroded and kinked

- Not found any broken wire during checked by wire rope tester.

4. Conclusion and Recommendation

4.1) SOB Structure Framing1 Female end Collect Connector/stubbing Cone at 4 Bottom Leg of SOB Need to

be Replace ( Picture – 2 )

2 ea Lifting Pad eyes on structural Framing ( No: 1 and 2 ) Need to be Replace atleast the Reinforcement Pads ( Picture – 7 )

3 All "U" bolt of 3" dia riser support Need to be Replace ( Picture – 6 )

4 Structural frame on bottom SOB Need to be Repair or Replace ( Picture – 4 )

5 Hoses Guard Heavy Need to be Replace ( Picture – 12 )

6 Gin Pole Frame Leg 2 elevation frame 1 and Leg 3 elevation frame 4 and 3"diameter of leg frame near elevation frame 1 need to be weld-out ( Picture – 13 )

7 All Bolt Rubber fender Need to be Replace ( Picture – 9 )

8 Connection flange joint SOB to Gin pole at leg 4 and leg 3 bolt need to bereplace and tightening ( Picture – 11 )

9 Corroded SOB Framing replace with additional stiffener plate ( Picture-5 )

4.2) Floating Buoy

1 2.ea lifting pad eyes on Floating Buoy ( No: 5 and 6 ) Need to be Replace

2 One section of Floating Buoy at 6" dia support position Need to be installdoublers plate and support to be replace ( Picture – 10 )

3 Inside floating buoy for riser penetration need to be blowing and cleaning prior toLeak test. ( Picture – 14 )

4.3) Wire rope, Shackle and Winch

1 All Wire rope of 4 ea winch Need to be Replace ( Picture – 1 )

note : only wire rope that outside from the winch that can inspected

2 Shackles on pad eyes at bottom SOB need to be replace. ( Picture – 3 )

3 Hoses damages of winch no:1 and no:2 Need to be Replace ( Picture-15)
